Comments: Structural observations have been completed for this project as required by Section
1710 of the 2010 Oregon Structural Specialty Code and per the Statement of Special Inspection Plan
shown on the Drawings. To the best of my knowledge, the construction conforms to the requirements of
the contract documents and all reported deficiencies have been successfully resolved.
The above statement is based on:
• Structural observation site visits
• Special Inspection reports, including Carlson Testing Inc.'s final summary letter dated May 17,
2016.
