Dear Mr. Robinson.
Once again we find ourselves in the position of having one of your
projects completed without your firm having obtained the required
building and mechanical permits. The plumbing permit was obtained,
but only after a stop work order was placed on the job.
The particular job this time is the woL-k for the Lakeside Chiropractic
Health Center. The plans for this project were submitted October
13, 1988, by Space Concepts. A Dlan review letter, listing comments
and conditions, was mailed October 19, 1.988, to Space Concepts.
No response from either the design firm or your company has beer,
received to date.
The permits will have to be purchased, and all inspections completed,
before a Certificate of Occupancy can be issued for the tenant.
The fees for the Building Permit are $251.23. We have not received
any plans for changes or additions to the mechanical system.
We request that you purchase the required permits at once to avoid
further action as required in these instances. if you have any
questions, or if we may be of assistance, contact us at any time.

Gentlemen:
A fire and life safety plan review was conducted on the above captioned
project for compliance with the 1985 editions of the Uniform Building Code
(UBC) , Uniform Mechanical Code (UMC) , and Uniform Fire Code (UFC) as amended
by Tualatin Rural Fire Protection District's Ordinance 86-5.
Submitted plans for the above captioned project are -conditionally approved
subject to the following items:
1. Fire Alarm Plans: Plans referred to and examined by this office
contain no provision for the alteration or installation of fire alarm
system. Not less than three sets of plans for the installation shall
be submitted to this office for approval prior to installation. (UBC
Sec. 302(b))
2. Firestopping: In all wood framed walls and partitions, firestopping
consisting of 2-inch nominally-sized lumber or other approved
materials must be installed at all floor and ceiling levels.
Penetrations in this prescribed firestopping to accommodate wiring,
plumbing, and other similar utility runs must be packed with
noncombustible materials in an approved manner so as to prevent the
passage of flame. (UBC Sec. 2516)
3. Exit Door Hardware: All doors shown on the drawings must be openable
from the inside for immediate exit. at all times without the use of a
key, special knowledge, or effort. (UBC Sec. 3304)
4. Rproved Plans on Job Site: One set of approved plans bearing the
staiips of the Tigard Building Department and this office must be
maintained on the project site throughout all phases of construction
and must be made available to building and fire inspectors for
reference during required construction inspections. (UBC Sec. 303)

A fire and life safety plan review was conducted on the above-
captioned project for compliance with the 1985 editions of the
Uniform Building Code (UBC) , Uniform Mechanical Code (UMC), and
Uniform Fire Code (UFC) as amended by Tualatin Rural Fite Protec-
tion Distr;.ct's Ordinance 86-5.
The plans are approved subject to the following conditions.
1. Corridor Doors: Doors located in common corridors
shall be of not less than 20-minute fire resistive
rated assemblies and shall either be automatic or
self-closing. Relight assemblies into corridors
shalt be of not less than 114-inch wired glass set
in steel frames.
(ULC Sec. 3305(g)(h))
2. Exit Door Hardware: All doors shown on the drawings must
be apenable from the inside for immediate _-xit at all times
without the use of a key, special knowledge, or effort.
(UBC Sec. 33U4)
3. Insulation Flame Spread: The insulation, including breather
papers and vapor barriers which are not in contact with the
upper surface of the ceiling and under surface of the floor,
as the case may be, must have a flame spread rating of not
to exceed 25 and a smoke development classification of not
greater than 450 as measured on the Steiner Tunnel Test
scale referred to as UBC Standard No. 42-1.

4. Mechanical Plans Required: Plans referred to and examined by
this office contained no plans for heating or air condition-
ing systems. Unless electric baseboard heat is employed,
complete mechanical system plans for the HVAC equipment and
duct work must be submitted to and approved by this office
prior to installation.

5. Fire Extinguisher Required: A fire extinguisher having a
minimum rating of. ZAIOB:C must be placed in an accessible
location within plain view.
(UFC Sec. 10.301(x))
NOTE: If extinguishers already exists in the main corridor
area and they are within 75 feet travel distance to all
parts of this occupancy, disregard this item.
6. Automatic Smoke Detection- Plans referred to and examined
by t.his office contain no provisions for the alteration or
installation of automatic smoke detection. Not less than
three sets of plans for the installation shall be submitted
to this officE for approval prior to installation.
7. Approved Plans on Job Site: One set of approved plans bear-
ing tilt. Stamps of the Tigard Building Department and this
office must be maintained on the project site throughout all
phases of co-istruction and most. be made available '.o build-
ing and fire inspectors for reference during required con-
struction inspectiOns.

B. Inspections Required: Inspection and approval of ^onstruc-
tion by a representative of this office is required: (a)
prior to the cover of any new framing elements following the
installation of all utility- runs which will. be concealed
within wall and partition cavities; (b) upon completion of
consttvction ano prior tc occupancy of the tenant space.

9. Certificate of occupancy Required: Prior to the use and
occupancy of the project (space) , a certifica`.e of occu-
pancy or other written instrument of approval must be
