This is a Fire and Life Safety Plan Review and is based on
the 1991 editions of :he Uniform Fire Code (UFC) and triose
sections of the Unifor"'m Building Code (UBC) and Uniform {
Mechanical Code (UMC) specifically referencing the fire
department, and other local ordinances and regulations.
Revised plans are conditionally approved subject to Tigard

i The Washington County Department of Health and Human Services has
obtained the plans for the proposed Rrr,n(l '''able Pizza to be located at 12085
S.W. Hall Boulevard, Suite 12.0 i- )regon.
It is our understanding that community water and community sewer will be
utilized at this structure.
The following is understood to be planned with necessary changes for approval
noted:
1) The plans show a commercial dishwasher that sanitizes with a
chemical sanitizing rinse. Machine or water line mounted
thermometers must be provided to indicate water temperatures of
the wash and rinse cycles. These thermometers must be accurate to
+3 degrees F. The dishwasher ;nest be capable of reaching proper
wash and rinse temperatures. Chemical sanitizers used must meet
the requirements of 21 CFR and be dispensed improper
concentration. An accurate test kit is required to test sanitizer
concentration of the final rinse.

2) The plans show a culinary sink. Please be aware that this sink can
not be utilized for noncompatible uses such as handwashing or mop
washing. This sink is shown to waste indirectly to a floor sink.
3) The plans show a utility mop sink. Please supply a mop hanging
device so mops and similar floor cleaning equipment can be •
cleaned and hung between uses.
4) There must be a handsink desip-nated in each of the food or drink
preparation and food or drink dispensing areas. Handsinks are
shown the front and back preparation areas and are convenient to
all areas. Note: A partition may be required between the mixer
and handwashing sink if handwash splash could contaminate the
mixer or dough in the mixer.
5) All handwashing sinks including toe restroom handsinks must be
equipped with dispensed soap and dispensed sanitary trowels or
approved hand drying devices. Com non (cloth) towels cannot be
used to dry hands. If disposable towels are used, easily cleanable
waste receptacles must be conveni:mtly located near the
handwashing facilities. The handwashing sinks must be equipped
with hot and cold tempered water. If self-closing, slow-closing, or `
metered faucets will be used, they must be designed to provide a
flow of water for at least 15 seconds without the need to reactivate
the faucet.
6) The restaurant plans indicate seating for 90. Two restrooms are
shown with a total of th-ee toilets, one urinal and two handsinks.
7) The restrooms must meet all the requirements as described in the
1987 Oregon Food Sanitation Rules for design, construction and
operation. Be aware that restroom doors must self-close and that
there mist be at least one covered waste receptacle in the women's l
restroom.
8; The dishwasher, food preparation sink, icemaker, soft drink
machine and salad bar are shown to waste indirectly. If any other
piece of equipment is utilized to hold food or ice and is equipped
with a drain then it must waste indirectly to an approved drain.
Where air gaps are required, the distance betwe^n the bottom of
the waste pipe and the top of the floor sink or drain must be at
least one inch or two waste pipe diameters (whichever is greater),

9) Any refrigeration unit which does not come equipped with an
evaporator pan for its liquid wastes must have its liquid wastes
drain indirectly to a floor drain or floor sink.
10) Floor sinks and floor drains must be located so they are accessible
for cleaning and maintenance.
R 11) Self-service beverages, soup and salad/condiment areas mint have a s
smooth, nonabsorbent floor covering such as vinyl, tile or the
equivalent extending 30 inches on each side to which the public has
access.
12) The plans submitted show a self-service beverage area. Please be
aware that beverage drinking containers gan not be refilled on i
dispensing units that require the container to come into contact with
the beverage machine. The lip of used beverage containers shoLld
never come in contact with a beverage dispensing unit or an ice
dispensing machine.
13) Base coving at least four inches in height will be needed on all
wall/floor junctures that require wet mopping.
14) Any gaps in floors, walls, or ceiling around plumbing or electrical
work must be filled into prevent rodent and insect access and
entrance.
15) Exposed utility lines and pipes can not be installed horizontally on
'he floor.
16) All lamps over or within food storage, food preparation, and food
l display facilities and facilities where utensils and equipment are
cleaned and stored shall be shielded, coated cr otherwise shatter
resistant.
17) Each refrigeration unit not equipped with an accurate built-in
thermometer, must have a spirit stemmed thermometer locatod on
the top shelf or door.
18) A metal probe thermometer accurate to ±2 degrees F must be
provided to assure attainment and maintenance of proper internal
cooking, holding or refrigeration temperature of potentially
hazardous foods.

19) Each hot holding facility storing potentially hazardous food must be
provided with a numerically scaled indicating thermometer accurate
to ±3 degrees F, located to measure the air temperature in the
coolest part of the facility and located to be easily readable.
Recording thIrmo,re,ers accurate to 3 degrees F, may be used in
lieu of indicating thermometers.
20) Where it is not practical to install thermometers on equipment such
as bain-maries, steam tables, steam kettles,heat lamps, cal-rod
units, or insulated food transport carriers, then the product +•
thermometer must be available and used to check internal food
temperatures.
21) If perishable foods will be reheated, a method to reheat this food to
165 F within one hour must be provided. Steam tables, bain
maries and crock pots are not allowed for rapid reheating or
cooking of foods. {
22) If perishable food will be cooled then a method to rapidly cool this
food must be provided. Commercial air cooled refrigerators or ice
baths are recommended for cooling foods. When foods are cooled
in the refrigerator, they must be cooled in shallow containers no
more than four inches deep with food no more than three inches in
depth in the container.
2.3) All equipment must be installed so as to be moveable or properly
sealed to facilitate proper cleaning.
24) Storage shelves must be smooth, impervious, and easily c' nable.
Unfinished wood is not acceptable.
25) To minimize manual contact of foods, please provide and utilize
i handled scoops and other appropriate utensils.
26) Food may not be stored under exposed or unprotected sewer lines
or water lines, except where automatic fire protection sprinkler
heads may be required �)y law.
27) All storage of food, food containers, and single service utensils i
must be on shelves at least six inches above the floor except where
storage is on wheeled platforms or four inch high sealed bases.

28) Ail floor mounted equipment, unless readily movable, must be
sealed to floor, installed on a concrete or otherwise smooth base at
fi least four inches high, or elevated on legs to pro, ,de at least a six
inch clearance between the floor and equipment.
29) Vertically mounted floor mixers need only be elevated to provide at
least a four inch clearance between the floor and equipment if no
part of the floor under the mixer is more than six inches form
cleaning access.
30) Be aware that all food or food items in the facility which are within
customer reach and not prepackaged, must be protected from
customer contamination by a sneeze shield or other approved
weans. Please see the NSF pamphlet that is enclosed for
information on sneeze shield requirements.
31) If food delivery is planned then deliveries must be made in
approved company vehicles with approved equipment that will keen
products at proper temperatures.
32) Outside storage areas or enclosures for garbage and refuse
containers must be large enough to store these containers in and
must be kept clean. Garbage and refuse containers, dumpsters and
compactor systems located outside must be stored on or above a
hard, nonabsorbent surface such as cement or machine-laid asphalt
that is kept clean and maintained in good repair.
33) Your plans show seating for more than 30 patrons and will need to
I conform with the Oregon Clean Air act designating smoking and
nonsmoking areas. for your convenience, a copy of this rule is
enclosed.
.34) The local plumbing authority may require a grease interceptor be
installed. If a grease interceptor is required, it must be located and
installed so that it is effective. A maintenance schedule must be
developed and followed to prevent grease from. going down the,
sanitary sewer.
35) All plumbing; must meet the requirements of the City of Tigard and
the Oregon Uniform Plumbing Ccde.

Sanitation Rules and Statutes.
37) All employees must have current Washington County Food
Handler's Cards. For information call 640-3460.
38) A preopening inspecti-,n must be conducted by our Departmeat
prior to license approval and operation. Please contact Tim
Bunnell at 648-8722 at least one week prior to operation to
schedule this inspection.
39) The license fee of $370.00 and license application must be
submitted to this office prior to operation.
If any further changes are necessary, it will be required that those changes be

To Whom It May Concern:
CMA is a manufacturer of low temperature commercial dishwashers that are chemical
unitizing type mac%ines. Our dishwashers operate on 140' final rinse water temperature and
use chlorine rather than heat to sanitize. One of the main advantages of using a low temp
diahwasher is the fact that it keeps the kitchen much cooler,
The question that wait brought to my attention waA regarding the vent hood over the
dishwasher and whether or not a vent hood was necessary on a low temp Chemical Methods
dishwasher. CMA has been manufacturing dishwasher's for 30 years, and to my knowledge,
at no time has had to supply a vent hood over a low temp dishwasher. I don't know of any low
temp manufacturing company that has been required to supply a vent or to have vents supplied
along with the low temp machines L-i restaurant type applications. If putting a vent hood over
a low temp dishwasher is a new requirement, I would think that this would be of common
knowledge in our industry. I do know, however, that this V at requirement for high temp
machines (180' final rinse).

This is a Fire and Life Safety Plan Review and is based
on the 1991 editions of the Uniform Fire Code (UFC) and
those sections of the Uniform Building Code (UBC) and
Uniform mechanical Code (U!-1C) specifically referencing
the fire department, and rather local ordinances and

The plans for this project were reviewed for co.=form.ity with
applicable codes. Please submit the following items for completion
of the plan review process at your earliest convenience:
1. Since the occupant load is greater than 50, the occupancy
classification for this tenant improvement is an A-3.

2 . Submit complete plumbing and mechanical plans (including
gas piping) for review.
3. One in eight accessible parking spaces, but not less than
one, shall be served by an access aisle 96 inches wide
. minimum and shall be designated van accessible (Table 31-
A, Figure 9, and section 3104(g)2B) . Please submit a
signayc detail .for the handicapped parking stall.
4. Key-locking hardware may be used on the main exit only,
if there is a readily visible, durable sign on or f
adjacent to the door stating, "THIS DOOR MUST REMAIN
UNLOCKED DURING BUSINESS HOURS" (section 3304(c)
exception) . The east exit door to have panic hardware
(section 3317(d) ) .
5. The toilet room shall have a smooth hard nonabsorbent
surface which extends upward onto the wall at least 5
F inches (section. 510(c)l) . x

6 . Walls within 2. feet of the front and sides of urinals and
water closets shall have sm, oth, hard nonabsorbent
surface to a height of 4 feet. The material used in such
walls shall be of a type which is not adversely affected
by moisture (section 510(c)2) .

7 . The floor or landing at entrances to the building shall
not be more than 1/2 inch lower than the threshold of the
doorway (section 3304(i) ) .
6 . All foundation plates or Bills on the concrete slab to be
a
pressure treated wood (section 2516 (c)3) . '

9 . Submit a typical wall detail specifying stud size, gypsum
board size, and attachment to the floor and roof
structure.
10. Provide a draft stop above the suspended ceiling. In a
non-sprinklered building, the maximum distance between
draft stops is 60 feet with the maximum square footage of
3000 square feet (section 251F(f)4B(iii) ) .
11 . Double-acting doors shall be provided with a view panel
of not less than 200 square inches (section 3304(b) ) .
12 . Submit a seating diagram for the dining areas (section
3302(b) ) .
13. Submit handicapped details for the rest rooms which
comply with the current requirements of Chapter 31.
14 . Two accessible parking spaces are required for this site
(Table 31--A) .
15 . Provide curbed ramps at the sidewalks for the accessible
parking spaces (section 3103(b) and figures 11-13) . �
16 . Provide the type and thickness of glazing for review.
17 . Tempered glazing is required in fixed or openable panels
adjacent to a door where the nearest exposed edge of the
glazing is within a 24 -inch arc of either verticle edge
of the door in a closed position and where the bottom
exposed edge of the glazing is less than 60 inches above
the walking surface. Tempered glazing is also required
in the exit doors (section 5406(d) 1,6) .
18. The walk-in cooler to be Oregon State approved. Submit
the manufactures name and three complete sets of plans
for review.
Please make these corrections on the appropriate pages of the
drawings and resubmit three copies of each page to the City of
Tigard for review.
This plan review does not include electrical or plumbing plan
reviews. Electrical concerns can be directed to Washington County
at 640-3470 and plumbing concerns to Mike Sheehan at the City of
Tigard at 639-4171 extension 312 .
