This is a Fire and Life Safety Plan Review and is based on the 1985 editions
of the Fire and Life Safety Code (UBC) , Mechanical Fire and Life Safety Code
(UMC) , Uniform Fire Code (UFC) , and other local ordinances and regulations.
Plans are conditionally approved subject to the following items:
1. Address Required: The tenant space number must. be prominently
displayed on the street front where it is readily visible to
drivers and officers of responding fire apparatus and other
emergency vehicles. UFC Sec. 10.208
2. Fire Extinguisher Requirements: Not less than one (1) approved
fire extinguisher(s) with rating of not less than 2AlOB:C shall be
provided for each 1,500 square feet of floor area or fraction
thereof. The travel distance to an exting+iisher from any portion
of the building shall not exceed 75 feet. UFC Standard 10-1
3. Second Story: Second floor of this building shall not be used for
any purpose other than storage. If other uses are contemplated,
then plans shall be submitted to this office and the City of
Tigard Building Department for review and requirements of any
recommended changes needed.
4. Approved Plans on Job Site: One set of approved plans bearing the
stamps of the building department issuing the construction permit
and this office mist be maintained on the project. site throughout
all phases of construction and must be made available to building
and fire inspectors for reference during required construction

5. Inspections Required: Inspection and approval of construction by
a representative of this office is required: (a) prior to the
cover of any new framing elements following the installation of
all utility runs which will be concealed within wall and partition
cavities; (b) upon completion of construction and prior to

6. Required Occupancy Certificate: Prior to the use and occupancy of
the project space , a certificate of occupancy or other written
instrument of approval must be obtained from the building
department issuing the construction permit. UBC Sec. 307

codes, and are approved, subject to clarification or inclusion of the

1. Additional structural details may be required should major
repair or replacement of floor members be necessary.
2. Code requirements for separation of mechanical equipment
from classroom occupancies may necessitate additional fire
separations or fire-resistive construction.
3. The second story must be limited to an occupant load of 10.
More than 10 will require a change in the stairway pitch as
well as the addition of a second exit.

1. SCOPE OF CONTRACT.
Contractor shall be fully responsible for as labor.materials, and equipment necessary for lite curnplete
construction of the Work as required in the Contract Documents.
2. SE t RINr ANQ COMPLIANCE WITH BIDDIN ' QOCUMENT
Drawings, Specifications. and other bidding documents may be obtained at the lime and place indicated on the
NOTICE TO BIDDERS. Bidders shall comply with all conditions stipulated in such bidding documents.
3. B_QaV1IRE-
The site and any building located thereon shall be thoroughly examined in relation to conditions which might
directly or indirectly affect the contract work. The bid sum shall reflect all such affecting conditions. Bidders
shall be responsible for verifying all dimensions which may affect the Work.
4. INTERPRETATI N OF DRAWINGS AND Q DOCUM NT :
a. If any bidder is in doubt as to the true meaning of any part of the Contract Documents or finds errors.
discrepancies,or omissions in them,he shalt at once request interpretation or correction thereol by the
Project Manager
b. The Project Manager will promptly clarify the area in question and issue written Instructions to all
prospective bidders. Verbal instructions or interpretations will have no validity regardless of source.
c. Request for such clarification must be in the office of Project Manager a minimum of live days before bid
opening.
5 PREPARING AND SUQMITTIN IDS:
a. Bids shall be prepared on Church Bid Forms.
b. Bid lorms may be obtained from the Project Manager.
c. The Bidder's legal name and business address shall be stated in full

1. The Contractor shall at his own cost and expense provide necessary permits,comply with local ordinances and
regulations, and provide arty surveys necessary to locate addibuns or structures and conjoint to zoning
ordinances and protective covenants governing said real properly

The Contractor shall not commence work under this Contract until he has obtained the insurance required
hereunder and such insurance has been approved by the Owner. The submittal of said evidence to the Owner

3. Per'ormance Bond and Labor & Material Payment Bond -
When required, Contractor shall furnish to the Owner a Performance Bond and a Labor & Material Payment
Bond.
Each bond shall be in an amount equal to 100% of the Contract sum and be security for all obligations arising
thereunder.
Both the forma of the bonds and the surety company issuing them shall be subject to the approval of Owner.

SECTI�I 4 .IOLD HARMLESS AQREEMENT:
1. Contractor shall indemnify and save Owner, Owner's representative, and their agents and employees harmless
from anu against any and all liability. demands, causes of action,or claims thereof,whether well founded or
otherwise.including the cost of defending the same, for bodily injury to any persons whomsoever,or damage to
property of any persons in the course of construction as a result of the negligence of Contractor, subcontractor,
or matecals supplier, their agents or employees.
2. Contractor shall be liable to defend Owner in any lawsuit hied by any subcontractor or materialmen as a result y
of the project which is the subject matter of this Contract.
SECTION 5_J--XTRAS& CHANGE ORDERS:
1 The Owner may, without invalidating this agreement,order extra work or make changes by altering, adding to,
or deducting from the Work, the price herein being adjustod accordingly However, any changes from the terms
of the Contract or the Drawings and Specifications requued by the Owner shall not be valid unless the changes
ordered are in writing with the price specified and signed by both Owner and Contractor
QECTION 6 DELAYS& EXTENSION QF TIME.
1. All time limits stated in the Contract Documents are of the essence of the Contract
2. If Contractor is delayed at any time in the progress of the work by any act or neglect of Owner, or by changes
ordered in the work,cr by strikes, lockouts, unusual delay in transportation,unavoidable casualties or acts of
nature beyond Contractor's control, then lire time of completion shall be extended for Such reasonable time+as
may be agreed upon by Owner and Contractor, Such agreement shall be in writing and signed by Owner and
Contractor.
SECTION 7 CONTRACTOR RESPONSI IIT
1. Contractor is fully responsible for the Project and all materials and work connected therewith until the Owner
has accepted the completed Project in wrrfing The Contractor shall replace or repair at his own expense any
materials or work damaged or stolen, regardless of whether he has received payment for the work or materials
from the Owner.
SEQTION 6 RESPONSIBILITY FOR ACTS & OMt5SIQN�.

2. For All Items Suecified Above Contractor Shall.
a. Allow sufficient lead time for orderly procurement.
b. Schedule deliveries in harmony with construction schedule and give written notice to vendors and Owner of
any revisions to delivery dates at least 90 days in advance.
1) Maintaining Owner's integrity with national purchase agreerneni sources and selected vendors requires
that installation crews are not directed to commence work before lob conditions will permit their
performance.
2) Owner may back-charge Contractor for Owner's installer's actual expenses resulting from faulty
scheduling of work relating to items in this Section
c. Receive and unload during normal working hours.
d. Verify that number of packages received matches number listed on freight bill and check packages for
external damage. Report damage and loss on freight bili. Within one day of freight delivery, send freight
bills recording loss or damage to The Church of Jesus Christ of Latter-day Saints, Atin: Traffic Section,
13th Floor, 50 East North Temple Street. Salt Lake City. Utah 84150. Identify freight bill with name of
Project as listed in Contract Documents. r
1) Shortages and damages not recorded and reported as specified above shall be repaired or replaced, as
determined by Owner, at Contractor's expense.

1. Unless arranged differently, the Contractor shall furnish and maintain, at his expense and in accordance with all
local codes and ordinances, temporary walar, torl,-t facilities, heat, fight and power.
3 The Contractor shall provide proper storage facilities for storing and protecting materials on the site as well as
scaffolding. I_dders, ramps,hosts, etc, all to comply with Labor Law, safety regulations, and applicable Slate

3. Groups of trees and other vegetation shall have prolective fencing erected around the ontire group to the
satisfaction of ;he Architect.
4. Areas within the prolective fencing shall remain undisturbed and shall not be used for any purpose.
5. Vegetation that has died or has been damaged beyond repair shall be removed and replaced by Contractor to the
satisfaction of the Architect.
44_ SECURITY
1, The Contractor shall project all adjacent private and public property including sidewalks, streets, underground
and overhead utifilies, etc, and make any repairs thereto at his expense
2. The Contractor shall provide necessary precautions to protect all persons on the site,including members of the
general public, from injury or harm including postirrl of warning signs, erection of barricades,guardrails, lights
and any other protection necessary to meet lob condilions arid OSHA requirements.
3. The Contractor shall provide protection to excavations, building, materials, landscaping, etc, from any form of
damage. All openings in the building shall be provided with temporary waterproof enclosures as soon as the
walls and roof are erected. When necessary, work in process shall be covered each night Observe cold weather
precautions, and stop work and notify the Architect when necessary

catalog reference,only such item shall be used unless the Project Manager's approval for a substitution is
secured in writ+ng prior to bid.
3. Where the word "approved by". "satisfactory to". "as directed by". "submitted to "inspected by", or similar
phrases are used in this specification, they shall be understood to mean that the material or item referred to
shall be approved by, satisfactory to,oi inspected by the Project Manager.

3. The Architect will reinspect the Work upon r.ceipt of notice that the Work,including inspection list items from
final inspection, has been completed,except r'ems whose completion has been delayed because of
circumstances acceptable to the Architect. Ufron completion of reinspection,the Architect will prepare a
certificate of final acceptann:.,or advise the Contractor of Work that is incomplete or of obligations that have not
been fulfilled but are required for final aeceptan:e. The cost for any inspections beyond the re-inspection

3) Sand lightly as necessary. Apply second coal of joint or topping compound, feathering
edges two to three inches beyond preceding coat.

a. In cold weather, provide thermostatically controlled heal 55 Degrees F to 70 Degrees F day

d. Use a broad steel finishing knife to butter joint compound into channel formed by tapered
edges'of board, filling channel Tully and evenly. Avoid heavy fills.
e. Embedding Tape -Center tap6 reinforcement and press duan into fresh joint compound. t7
Hold knife at an approximate 45 Degree angle to board and draw along joint with sufficient rt
pressure to remove excess compound. Leave sufficient compound under tape to provide a
propel bond but not over 1/64-inch under feathered edge. Do not use lopping or finishing
compound for embedding tape.
f. Covering Tape - Atter embedding tape, immediately apply skim coat of joint compound. This
cover coal reduces possibility of edge wrinkling or curling which may lead to cracking of h3
edge. Allow to dry twelve nours. j
g. Spotting Screw Heads- Apply first coal of compound over screw heads immediately prior to 1
or after tape has been embedded. Use sufficient prhssure on knife to finish compound level
with wall surfaces Spot screw heads in a similar manner during second coat application of
compound During application of third coat over jcints,sand screw heads lightly and apply
third finishing coat. Durile screenbak sanding cloth, 320 or 220 grit,is recommended for
best results
h Second Coat Application- Aher embedding and covering coat has completely dned (2a hours
under good drying conditions),apply a second coat, feathered approximately 1 1/2 inches (38
mm)bevond edges of first coal. Allow to dry Sand lightly.
I Third Coat Application- Atter second coat is dry, sand lightly. Apply a thin 'finishing' �t
coat to pints. Feather joint edges at least t-3 inches(50 mm)beyond second coat. Sand j)

a. Prepare substrates as lollows -
1) Over Concrete-
a) Remove paint, sealer,grease,oil.old carpel and adhesive, and other materials
incompatible with carpet adhesive.
b) Patch cracks,holes, and irregularities with specified material to provide a smooth,
level surface.
2) Over Existing Vinyl Asbestos Tile -
a) Remove all finishes on VA lite prior to installation of carpet.
b) Patch broken or damaged Ute with new tile where one or two tiles will provide
smooth surface. In areas of greater damage,remove broken tiles, clean and prepare
substrate as necessary, and feather rough edges with latex underlayment.

b. Frotecting Other Surfaces -
Good painting practice excludes splattering,dripping cr painting any surfaces not intended
to be painted. These items will not be spelled out in detail but special attention must be
[,aid to the following -
I) Do not paint finish copper, bronze,chromium plate,nicker, stainless steel, am dized
aluminum, or rnonel metal except as explicitly specified.
2) The cones of ceiling speakers must be kept completely free of pant Metal grilles over
ceiling speakers must be masked off, if the ceiling is being spraj painted. It it is
required that these grilles be panted, it shall be carefully done with brush or roller
after the spray painfinq is complete.

c. Execution -
1) Carefully follow Specifications and Color Schedule,painting completz surfaces to be
painted.
2) Secure Color Schedule for rooms before priming walls. Tint priming coat and undercoat
to approximate shade of final coat.
3) Wash metal surfaces with mineral spirits to remove dirt and grease before applying
materials. If rust or scale is present,clean with a wire brush or sandpaper before
oamling. Clean shop coals of paint that become marred,and touch up with proper type
primer.
4) Treat galvanized metal and zinc surfaces prior to applying first coal of paint

execute a joint use and maintenance agreement for
shared access drive or shall demonstrate that such

3. Use of the house shall be limited to no more than 20 students
at one time. The second floor of the building shall not be
used for classroom use. Classes shall be held between the
hours of 7: 00 a.m. and 4: 00 p.m. , Monday through Fr.•iaay,
during the Tigard School District school year . There shall
be no classes or other gatherings in the house during
evenings, weekends or during the summer . However, the
property may be used as an office for the teacher without
restriction as to the hours of operation, tVenings, weekends
or summer use.
4. Religious assembly uses of the property shall be limited
to the religious instruction of persons regularly enrolled
as students in grades 9 through 12 of Tigard School Disc..-ct.
However, for those students not enrolled at the Tigard High

School , there shall be no vehicular traffic directly to the
site for the purpose of either dropping off or picking up
those students. Any vehicles transporting students from
other locations skull use the Tigard High School lot across
92nd Avenue from this property.
5. The owner shall provide either on-site security system or
a security service to provide security for the building
during the hours that it is not being operated for classroom
use.
6. The church shall maintain the existing or an equivalent
fence along the east, north and west sides of this property.
Along the west side, of this property, subject to staff
approval, they shall complete the fence to the driveway, to
accomplish security for the lot. There shall be no student
access to this property along the shared access easement on
the west side of this property. All students shall enter and
exit the building from the main entrance to the sidewalk on
the 92nd Avenue side of the building .
7 . The approval shall be valid if exercised within one year
of the final approval date.
