The holding tank will be for an office trailer located at 13935 S.W. Pacific Highway, Tigard, Oregon. We hay:: reviewed your proposal and ari in receipt of your $100 permit tee. The Department herewith approves the installation of a hold- ing tank as authorized under Oregon Administrative Rules (OAR) Chapter 340, Section 71-037 (3) (B) . We are therefore issuing you an alternate sewage dispose; permit, No. 115-77, under the following conditions: 1. The holding tank shall be a minimum of 1 ,000 gallons capacity. 2. Evidence must be provided from a licensed, bonded sewage pumping company that the tank will be pumped periodically, at regular in- tervals, and the contents disposed at an ap- proved treatment facility. We are in receipt of L & M Sanitary Service's letter. Records of pumping dates and amounts pumped are to be maintained and made available to the Depart- ment of Environmental Quality upon request. 3. Inspection of the installed system by the City of Tigard must be accomplished prior to put- ting the holding tank Into use. 4.
