Please find enclosed original copy of Bond #2SB100408590-145 in the amr,_^t of $6,240.00, dated
September 6, 1991, covering the construction of monitoring -veils on S.W. Greenbury Road and S.W.
Cascade Avenue. Also find enclosed a copy of the signed permit application (permit #442-91) regarding the
construction of these weals.
Rittenhouse-Zeman & Associates, Inc. (RZA), on behalf of BP Oil, submitted an application to the City of
Tigard on Augus: 19, '991, to install up zo five monitoring wells on Tigard City streets adjacent to the sul,ject
site. One requirement of the permit was that a refundable surety bond or security check be issued to the
City of Tigard covering the full cost of well construction plus a non-refundable fee of 4%. The estimated cost
of each well was$1,200.00 1.0nging the total required security to$6,240.00 (5 X$1,200 plus$240 fees).
To expedite the permitting process, RZA issued fts check #24153 to the City of Tigard on August 29, 1991,
with the understanding that this amount, minus application fees, was refundable upon receipt by the City or
1 igard of a surety bond in like amount.
Please have your department remit a check In the amount of $6,000.00 payable to Rittenhouse-Zeman &
Associates at its earliest possible convenience.

Please note that the original permit application was for the construction of up to five monitoring wells,
numbered #1 -5 on plate 1 of t'ie letter accompanying the permit application. In fact, only three or the five
wells have been installed: wells #1, #2 and #4 (now referred to as MW9, MWT and MW8, respectively).
Consequently, RZA shall recommend to BP Oil that it reduce the amount of the surety bond to $3,600.00,
covering the installation costs of only three wells.

Please find enclosed a completed permit application for the drilling and installation of five groundwater
monitoring wells near the Intersection of S.W. Greenbury Road and S.W. Cascade Avenue Described below
are our proposed work and construction plans, as wei; as a traffic control plan and estimate.; costs of
construction.
Work Plan
Rittenhouse-Zeman and Associates (RZA) has been contracted by BP Oil Company to rurner♦late impacted
groundwater at the BP service station located at the intersection of S.W. Greenburg Road and S.W. Cascade
Avenue In Tigard. As a first step in the remediation process, groundwater monitoring wells need to be
installed downgradient from the site. In this case, all downgradient well locations fall on either S.W.
Greenburg Road or S.W. Cascade Avenue.
RZA proposes to drill and install up to five monitoring wells as soon as approval is granted by the City of
Tigard. Drilling has tentatively been scheduled for August 22 and 23. The drilling and installation will take no
more than two days. However, RZA requests a 30 day window (August 22 to October 20, 1.991) in which to
perform the work. Drilling will be scheduled for normal working hours (8 am - 5 pm) unless oti-lerwise
directed by the City of Tigard.
Construction Plan
The locations of the drilling sites are shown on Plate 1. Each well will be drilled to a depth of approximately
15 feet and cased with 2" schedule 40 PVC screened pipe (Figure 1). The casing will be sandpacked below
approximately four feet in depth and sealed above four feet with bentonite and concrete. The PVC casing
will be fitted with a locking well cap. The well will be fitted with a tamper resistant, traffic-bearing, flush-
mounted steel cover according to ctendard BP Oil Company well specifications.

Traffic Control Plan
American Barricade will provide traffic control services during all times that drilling operations are In effect.
Only one drilling site will be occupieo at one time. The following traffic controls will be instituted for the
various drilling locations:
1) For drilling of site #1, both the centermost southbound lane of Greenburg Road and the adjacent left turn
lane (Into the Shilo Inn motel parking lot) will be closed. Southbound traffic will be re-routed Into the
curbside southbound lane of Greenburg Road. Southbound access to the Shilo Inn will be provided via a
traffic flagger.
2) Fcr drilling site f, e, the curbside eastbound turning lane of S.W. Cascade Avenue (south of the traffic
islard) will be closed. Traffic wishing to turn either north or south onto Greenburg Road from Cascade
AvE nue will be routed through the centermost eastbound lane of Cascade Avenue (no+ih of the traffic island).
3) For drilling Site #3, bcth the curbside eastbound turning lane from Cascade Avenue to Greenburg Road
and the curbside southbound lane of Greenburg Road will be closed. Traffic wishing to turn south onto
Greenburg Road from Cascade Avenup will be routed onto the centermost eastbound lane of Cascade
Avenue to the centermost southbound lane of Greenburg Rcad,
4) For sites #4 and #5, the centermost southbound lane of Greenburg Road will be closed with southbound
traffic routed onto the curbside lane of Greenburg Road.
Given the large amount of traffic on Greenburg Road, bath flaggers and flashing arrow signs will be located
on either side of the drilling operation. Hazard warning signs indicating "FLAGGER AHEAD" wiil also be
posted, as appropriate. American Barricade will provide all traffic control services in accordance with
standard DOT regulations.
Construction Costs
P,ZA estimates the cost of installing one off site monitoring well at approximately$1,200. This price includes
the cost of providing traffic control services by American Barricade. A security bond in the amount of$6,000
will be provided by BP Oil Company by August 22, 1991. However, _given the likelihood that less than five
wells wil! be installed, the security bond will be reduced accordingly following completion of the wells.

Dear Mr. Davies:
Please find enclosed a completed permit application for the drilling and Installation of five groundwater
monitoring wells near the Intersection of S.W. Greenburg Road and S.W. Cascade Avenue. Described below
are our proposed work and construction plans, as well as a traffic contr")l plan and estimated costs of
constructlon.
Work Plan
Rittenhouse-Zeman and Associates (RZA) has been contracted by BP Oil Company to remediate impacted
groundwater at the BP service sijtion located at the intersection of S.W. Greenburg Road and S.W. Cascade
Avenue in Tigard. As a first step in the remediation process, groundwater monitoring wells need !o be
installed downgradient from the site. In this case, all downgr3dient well locations fall on either S.W.
Greenburg Road or S.W. Cascade Avenue.
RZA proposes to drill and Install up to five monitoring wells as soon as approval is granted by the City of
Tigard. Drilling has tentatively been scheduled for August 22 and 23. The drilling and Installation will take no
more than two days. However, RZA requests a 30 day window (August 22 to October 20, 1991) in which to
perform the work. Drilling will be scheduled for normal working hours (8 am - 5 pm) unless otherwise
directed by the City of Tigard.
Construction Flan
The locations of the drilling sites are shown on Plate 1. Each well will be drilled to a depth of approximately
15 feet and cased with 2' schedule 40 PVC screened pipe (Figure 1). The casing will be sandpacked below
approximately four feet in depth and sealed above four feet with bentonite and concrete. The PVC casing
will be fitted with a locking well cap. The well will be fitted with a tamper resistant, traffic-bearing, flush-
mounted steel cover according to standard BP Oil Company well specifications.

Traffic Cor-trol Plan
American Barricade will provide traffic control services during all times that drilling operations are In effect
Only one drilling site will be occupied at one time. The following traffic controls will be Instituted for the
various drilling locations:
1) For drilling of site #1, both the centermost southbound lane of Greenburg Road and the adjacent left tum
lane (into the Shilo Inn motel parking lot) will be closed. Southbound traffic will be re-routed Into the
curbside southbound lane of Greenburg Road. Southbound access to the Shilo Inn will be provided via a
traffic flagger.
2) For drilling site #2, the curbside eastbound turning lane of S.W. Cascade Avenue (south of the traffic
Island) will be closed. Traffic wishing to turn either north or south onto Greenburg Road from Cascade
Avenue will be routed through the centermost eastbound lane of Cascade Avenue(north of the traffic Island).
3) For drilling site #3, both the curbside eastbound turning lane from Cascade Avenue to Greenburg Road
and the curbside southbound lane of Greenburg Road will be closed. Traffic wishing to turn sou'h onto
Greenburg Road from Cascade Avenue will be routed onto the centermost eastbound lane of C-;cads
Avenue to the centermost southbound lane of Greenburg Road.
4) For sites #4 and 415, the centermost southbound lane of Greenburg Road will be closed with southbound
traffic routed onto the curbside lane of Greenburg Road.
Given the large amount of traffic on Greenburg Road, both flaggers and flashing arrow signs will be located
on either side of the drilling operation. Hazard warning signs Indicating 'FLAGGER AHEAD' will also be
posted, as appropriate. American Barricade will provide all traffic control services In accordance with
standard DOT regulations.
Construction costs
RZA estimates the cost of installing one off site monitoring well at approximately$1,200. This price Includes
the cost of providing traffic control services by American Barricade. A security bond in the amount of$6,000
will be provided by BP Oil Company by August 22, 1991. However, given the likelihood that less than five
wells will be installed, the security bond will be reduced accordingly following completion of the wells.
