I This letter is in response to our conversation regarding the status of the subject building permit. A.
discussed, ;n lieu of actual receipt of the permit.we intend to perform the following steps in order to
maintain out schedule for the foundation scope Lf work. To the best of my knowledge,these steps do 'lot
in any way circumvent any governing codes or standard processes associated with the con_nrucdon aad
inspection thereof. It is understood that PGB is liable for the foundation design,in that,if the plans
reviewer finds the design is unacceptable,or that performance in unable to meet the applicable standards,
we will assume 411y expenses to render it acceptable.

• CONCRETE RE'NFORCEMENT
The reinforcement cage shall be inspected by City of Tigard Inspectors prior to placement in the
drilled hole. This should be done in the AM.

• TOWER ANCHOR BOLT CAGE
i As required,the tower anchor bolt cage may be inspected by City of Tigard Inspectors prior to
placement in the drilled hole. This should be done in the Ai.'. i
• GEO17ECHNICAL INSPECTION
A geologist will provide observation and conformation of design parameters during construction.
The same firm,Northwest Geotech,Inc.,that performed the initial investigation will be present.

• CONCRETE TESTING
Testing of cotrrrtte shall be done in accordance with applicable ASTM standards. The minimum
acceptable compn,,ssive strength shall be 4000 psi. Test results will be given to the City of Tigard
for their review and approval prior to tower erection.

Dear Mr. Livingston:
This letter is in response to your request for a Minor Modification to the approved Conditional
Use Permit for the PGE Substation sit,;. "The Director has approved the proposed addition of a
monopole to i',e PGE Substation of a cellular communication tower as a Minor Modification to
the original Conditional Use Permit. Based on she findings which you provided, the Director has
approved this request subject to the Conditions of Approval indicated below.
The following revisions to the site and landscape plan shall be made prior to the issuance of
Building Permits:
1. As required under Section 18.100.080.E (1-3) of the Commu;iity Development Code, the
plan shall be revised to provide screening materials such as; an arborvitae hedge or other
similar type landscaping to screen the equipment structures from view of the adjoining
freeway and from adjoining properties. Where a hedge is provided, the plantings shall be a
minimum of fo jr-feet tall at planting.
2. Where applicable, the applicant shall demonstrate that as a part of the lease agreement that
they are entitled to plant landscaping outside the leasehold area.
3. The applicant shall alsc provide irrigation to any landscape plantings.
4. As required under Section 18.106.050.J (1-3) of the Community Development Code, the
plan shall be revised to provide a paved driveway from the street to the proposed pad site.
Areas within the pad site which are to accommodate maneuvering of a service vehicle shall
also be paved.

20. 1995. Provide new documentation with a current Oregon seal.
