Cascade Environmental has prepared this Pre-Demolition Asbestos Containing Material (ACM)
Survey report for the location referenced above.
SCOPE OF WORK
The purpose of this survey was to determine the presence of asbestos-containing material that
may be impacted during the planned demolition activities at the location referenced above.
Cascade Environmentafs Asbestos Hazard Emergency response Act(AHERA)Certified Asbestos
Inspector,Mr. Robert Bums,conducted the survey on July 12,2016.
ASBESTOS SURVEY
Sampling and Analysts
Cascade Environmentals asbestos survey was performed following the sampling protocols
outlined under applicable United States Environmental Protection Agency (EPA) and Oregon
Department of Emironnriental Quality(DEQ)regulations.
Bulk samples of suspect ACMs were collected and sent to QuanTEM Laboratories for analysis
using Polarized Light Microscopy(PLM) in accordance with EPA"Method for the Determination of
Asbestos in Bulk Budding Materials" (EPA/600/R-931116, July 1993). QuanTEM Laboratories is
located in Oklahoma City, Oklahoma, and is registered as an American Industrial Hygiene
Association-accredied and National Voluntary Lab Accreditation Participation (#101959)-
accredited laboratory. Routinely, the amount of asbestos is determined using a semi-quantitative
technique, which involves calibrated visual area estimation. This quantitative method expresses
the amount of asbestos as percent area and has a ink of detection of less than one percent
When required, the amount of asbestos can be estimated using another semi-quantitative area
estimation technique called point counting. In combination with gravimetric sample reduction
procedures, point counting can achieve limits of less than one percent(<1%),when 400 points are
counted and less than a tenth of a percent(<0.10%)if at least 1,000 points are counted.

ACM's as defined by the EPA and the State of Oregon are materials with an asbestos concentration greater than
one percent(>1%). However, the Occupational Safety and Health Administration (OSHA) regulates aft materials
containing any detectable levels of asbestos.
FINDINGS
A summary of Cascade Environmentals asbestos inspection, including analytical results, sample numbers,
material descriptions, and sample locations is provided in the following table. The analytical results and chain-of-
custody documentation is included in Appendix A.

Sold—indicates at least one layer of the material sampled contains greater than one percent asbestos.
ResuitslDiscuss
The laboratory analytical results for this survey indicated that none of the samples collected during this survey
contained greater than one percent asbestos and are not considered ACMs.
Caution should be used during renovation activities as concealed building materials, that have not previously
been analyzed for asbestos' may be encountered. If suspect ACMs located in accessible andior inaccessible
areas of the facility are encountered during renovation activities, further sampling will be required to characterize
those materials, prior to any disturbance.
Other Miscellaneous Materials
Cascade Environmental sampled all suspect asbestos containing materials identified in the areas to be impacted
during demolition activities. Materials that are considered non-suspect asbestos containing materials may be
included in the waste stream from the renovation activities. These materials may include wood, metal and
fiberglass insulation.

LWTATIONS
Reasonable effort was made by Cascade Environmental personnel to locate and sample suspect materials at the
location. However,the existence of unique or concealed ACM, other hazardous materials or associated debris is
a possbdity within any facility. If additional suspect ACM or other hazardous material is discovered during
demolition it should be assumed to be hazardous until sampling and analysis proves otherwise. Cascade
Environmental does not warrant, guarantee or profess to have the ability to locate or identify all hazardous
materials within a facility.
