Engineers,LLC (NSE). We have reviewed the mix design and find that it conforms with the intent
of the design documents as to the specified concrete strength, slump,and air entrainment
requirements.
NSE cannot respond to the remainder of the concrete mix design(i.e. amounts of cement,water,
crushed rock,sand equivalent,etc.). It is the responsibility of the concrete supplier to proportion
these items so as to meet the requirements of the design documents.

Conducted site observation to review foundation/stem wall reinforcement. At this time, all
foundation reinforcement and stem wall dowels are in place and the contractor will be ready to pour
concrete by 6:00 a.m.tomorrow morning.
To the best of our knowledge, all work observed appears to be in conformance with the design
documents.

Arrived on site at the request of the contractor to observe the removal of the loose debris in the uncovered areas, and to observe
the backfill of the voids.
The exposed void at grid point E, 1.25 had loose material that was removed with a mini-track hoe. During the removal, a void
directly north was exposed (along gridline E). The void extends a minimum of 5 feet outside the building footprint. Loose fill
(spoils)was 18 inches to 2 feet below the footing subgrade and slopes down into the void as far as visible. There is a 5 to 6 foot
span of unsupported footing visible.
It may be possible to install helical anchors on either side of the unsupported span to support the increased footing loads in this
area. This solution would need to be re-engineered and approved by the project structural engineer. Filling the void with CDF is
also a possible solution. It is important that the CDF/concrete used make contact with the entire exposed subgrade surface of the
footing.
A '/2 inch diameter steel rod was used to probe the footing subgrades at A, 5 to A,7 to B,7 and B,4 to B,7. The spread footing
subgrades at B, 5;B,6; C, 5; and C,6 were also checked. All footing subgrades were found to be firm and unyielding and seem
to be competent material and is suitable for further construction.
The material that was being removed during excavation remains unsuitable for use as backfill due to the high debris content.

Arrived on site at the request of the contractor to observe footing excavations and void clean-up.
The void at E, 1.25 was filled with 4 inch minus rock to approximately 2 feet below the subgrade of the existing footing. The
rock was then sloped so that CDF would flow beneath the footing and into the void behind the footing. The CDF was poured
into the void and above the subgrade of the footing until a static position was reached. A rock"dam" was pushed towards the
footing so head pressure could be built and CDF was then added.The rock was then compacted and CDF was added to the entire
void to 32 inches below the existing grade.
The void at C.8, 3.5 was filled with 4 inch minus rock to approximately 6 inches above the subgrade of the existing footing and
compacted.The void subgrade was approximately 10 feet below the existing grade.
A '/2 inch diameter steel rod was used to check the grade beam footing subgrades at C, 7 to F, 7 to F, 6; and the spread footing
subgrades at D,6 and E,6.All footing subgrades were found to be firm and unyielding with exception of the spread footing at D,
6 which required a 3'x9'x10" over-excavation to reach competent material that was suitable for further construction. An over-
excavation of 4'x8'x2' was required at approximately E,6.5 to reach competent material.
A Geocon Northwest representative will return to the site tomorrow,as requested.

Arrived on site at the request of the contractor to evaluate the elevator pit and footings.
The voids at E, 1.25 and C.8,3.5 were backfilled with compacted 4 inch minus rock to approximately subgrade.
A 1/2 inch diameter steel rod was used to check the spread footings at D, 5 and D.4, 3; and the elevator pit between gridlines D
and D.5. All subgrades were found to be firm and unyielding. The material appears to be competent and suitable for further
construction.
A Geocon Northwest representative will return to the site on Tuesday,as requested.

Arrived on site at the request of the contractor to evaluate the subgrade after the site was stripped of vegetation.
Upon arrival,the contractor was still stripping the site and the stripping material was stockpiled in the middle of the site.Geocon
met with Bones Construction and stated that in order to properly evaluate the subgrade, the large stripping pile will need to be
removed.
Darren (Bones Construction) stated that the material for fill placement is out behind Coffee Creek Correction at the North
Wilsonville exit. Darren requested that Geocon evaluate the material today. Brian Compton Trucking was supposed to be at the
site with excavation equipment. Geocon visited the source site;however, nobody was on site. Geocon called Darren and stated
that the material was not available to look at. Darren called back and stated that Brian would be on the site over the 2°d and 3`d of
July to clear the stockpiles of tall grass and steel debris setting in the borrow area.
A site visit was conducted on 7/2/11 at the source location,however,nobody was on site and the area still had grass and debris in
stockpiles.
A Geocon Northwest representative will return to the site upon request.

Arrived on site at the request of the contractor to evaluate an inflow water issue at the roadway expansion on 72nd Avenue and
the corner of 72nd Avenue and Dartmouth Street.
At the 72'Avenue expansion, a considerable inflow of water on the excavated subgrade for the roadway was observed.At the
corner of 72nd Avenue and Dartmouth Street there was water inflow from an existing trench line that runs to a manhole on the
property. The inflow off of Dartmouth Street is in a structural area of the Dartmouth roadway expansion and sidewalk. It is
Geocon's opinion that some type of drain system be installed at both locations prior to any base rock placement.
The contractor had approximately 12 to 15 cubic yards of material that was excavated on site and wanted to use it as fill material.
Geocon stated that the material is unsuitable for any structural fill on site.
The stripping stockpile was still on site;Geocon will not be able to evaluate the stripped subgrade until it is removed.

Arrived on site at the request of the contractor to evaluate the stripped subgrade at selected areas of the site.
A fully loaded 10 to 12 cubic yard dump truck was used to proof-roll the stripped subgrade on a portion of the north 1/3 of the
site,the south 1/3 of the site,and the roadway extension footprint.The subgrade was observed to be firm and ready for continued
construction with the exception of an area that was 30 feet by 30 feet in the western end of the north 1/3 of the site. This area
requires an over-excavation of one foot to remove soft material that has some organic material.
The stripping stockpile remains on site;therefore,Geocon was not able to evaluate the stripped subgrade.Additionally, Geocon
was not able to observe a proof-roll of an area in the north 1/3 of the site due to the stockpile of unsuitable material that was
mentioned in the previous field report.
The contractor has not yet determined the fill source material.

Arrived on site at the request of the contractor to evaluate the stockpiled material in the building pad footprint for use as fill
material.
Approximately 100 cubic yards of material was observed to be satisfactory for fill placements. The contractor has not over-
excavated the west 1/3 of the building footprint or the area in the new roadway alignment.
Geocon conducted a site visit at Bones Construction site in Tualatin,Oregon to obtain a soil sample of the proposed fill material
to be used at Red Rock Center,Two,5 gallon buckets and three sealed bag samples were collected for testing purposes.
Geocon will obtain the in-place moisture range from the sealed bag samples. It appears that the material has a high moisture
content in-field.The material was observed to be satisfactory for use as fill placement with optimum moisture testing to follow.
The results of the soil moisture tests ranged from 23.8%to 30.0%.

Onsite today to observe general construction process on building pad footprint pa pri»t and roadway alignment footprint.
We collected the moisture content bag samples of fill material at Bones site in Tualatin. The in-field moisture content ranged
from 23.8% to 30%. The max thy density and optimum moisture results of the proctor test performed at our laboratory were
determined to be 107 pcf at 18%. Therefore moisture conditioning will need to be performed prior to any placement of this fill
material.This information was communicated to Darren with Bones Construction.
We conducted preliminary in-place density testing of the fill material that had been placed and compacted in the existing ditch-
line at the north end of the building footprint.The test results revealed the fill placed to be high of optimum moisture at 25.9%
and at 89% max dry density. This further indicates that the import fill material will require moisture conditioning prior to
placement.
We conducted an afternoon site visit at the request of the contractor to evaluate fill placement after mixing the import material
with drier onsite soil in an attempt to reduce the moisture content. In-place density testing revealed compaction to range from
88%to 91%with moisture content ranging from 24%to 25%.This does not meet the design specification requirement.
Had phone conversation with Matt with Bones and conveyed the results of testing today and was advised that Bones would halt
trucking this afternoon until issues with moisture can be resolved,

Arrived on site at the request of the contractor to evaluate the placement of fill on the building pad footprint and the new
roadway alignment.
Geocon received a call from Brian Keicher requesting a notification when Geocon would be on site so that he could make a visit
at the same time.Geocon received the call after the 8:00 AM visit.
Geocon called Brian Keicher at 11:00 AM and left a message that the contractor requested an afternoon visit at 1:00 PM to
conduct in-place density testing at the western 1/3 of the building pad footprint.
In-place density testing was conducted on the first 1 foot fill placement on the west 1/3 of the building pad footprint. The test
revealed moisture of 23.5%which does not meet design specification requirements,requiring additional moisture conditioning.
A site visit was conducted at 1:00 PM as requested by the contractor. Geocon observed that the contractor had trucked
approximately 15 loads of fill material in on 7/27/11 and spread out to condition the material's moisture.
In-place moisture testing was conducted at various locations in the farmed material. This material had been spread in the late
afternoon on 7/27/11. The moisture range was observed to be 18.5% to 20%, which is in the range of the specification
requirements.
During the afternoon visit,Brian Keicher did not visit the site.
The contractor requested a site visit on 7/29/11 in the morning, at which time there will be a meeting with Matt (Bones
Construction), Brian Keicher, Red Rock business associates, and Robert Pendell(Geocon Northwest). This meeting is about an
issue that Matt(Bones)has with the moisture content of the fill material coming on site.Matt believes the moisture to be closer

continue to rut the stripped subgrade due to the heavy loads.This was previously brought to Darren's(Bones Construction)
attention with a response from Darren that they would repair it at a later date.

Arrived on site at the request of the contractor to evaluate fill placements on the building pad footprint and roadway alignment,
and to attend a meeting with Matt(Bones Construction),Brian Keicher,Red Rock business associates.
Upon arrival, Geocon observed that Carlson Testing was on site to conduct in-place testing of the fill placement at locations
where Geocon took tests. Apparently this follow up of Geocon's testing pertains to the moisture contents of Bones
Construction's fill material.
In-place density testing was conducted on the fill placements at various locations on site.Test results and locations are shown on
the attached sheet. Test results revealed that tests 1 through 5 met the design specification requirements and tests 6 through 8 did
not meet the design specification requirements,which require additional moisture conditioning.
A fully loaded 10 to 12 cubic yard dump truck was used to proof-roll the center 1/3 of the building pad footprint after the
stockpiled material was removed.The subgrade was observed to be satisfactory with the exception of a small area that has drawn
moisture from the stockpiled material. The contractor plans to leave this area uncovered to dry back with the current high
temperature before fill placements.
Geocon met with Red Rock business associates, Brian Keicher, and Matt (Bones Construction) explaining Geocon's
observations to date in respect to the moisture of the fill material that was imported to the site.
Geocon explained that the moisture content of the material that was trucked to the site is high of optimum moisture content.
However, after moisture conditioning the material back to 18% to 20%, compaction of the fill placements meet or exceed the
required 95% compaction of the maximum dry density obtained in laboratory testing. Matt (Bones Construction) stated in the
meeting that they do not have the budget to conduct the moisture conditioning process.This issue is one that only Brian Keicher
can address.

moisture conditioning and compaction efforts before retesting.
Geocon understands that the wall is to be constructed at the west side of the roadway alignment. Bones Construction stipulated
that the center line of the wall is at the bottom of the slope on the current roadway fill. In Geocon's opinion, the roadway fill
placements should be placed and compacted past the center of the wall footprint for wall load support.

Arrived on site at the request of the contractor to evaluate the excavated footing subgrade at the building pad footprint.
A 1/2 inch diameter steel rod was used to probe the excavated footing subgrades at the west 1/3 and center 1/3 of the building pad
footprint. The footing subgrade was observed to be firm and satisfactory with the exception of the footing subgrade at the center
1/3 of the building pad footprint. During the probe,the conditions were observed to be very soft at the native subgrade portion of
the pad requiring an over-excavation to a firm, suitable condition. The areas of over-excavation are as follows: center 1/3 of
building pad: southeast corner-west 33 feet=33'x5.5'x1';northeast corner-south 45 feet=45'x55'x1.3'.
The contractor will use 3A inch minus crushed rock to restore the grades in the footing over-excavations.

building pad. A very firm satisfactory condition was observed.
The contractor was in the process of placing base rock on the west 1/3 and center 1/3 of the building pad footprint.
The contractor will continue footing excavation today and requested a site visit on 8/15/11 in the morning and a Geocon
