Hello Chris,
Wendy Uptegraft with National Permit Services asked that I contact you regarding the status of the above project
located at 9009 SW Hall Blvd, Ste 150, Tigard, Oregon.

According to our records,the permit was submitted on 1/4/2012 for plan review and a deposit of$627.07 was
received. Plan review was completed on 1/25/2012. The permit was made ready to issue on 1/31/2012 and the
applicant, Ms. Uptegraft was contacted and notified of the remaining permit fees due.
Since that time,we have called and/or emailed Ms. Uptegraft for a status update on this project on numerous
occasions. Because the permit has not been issued within 180 days of the 1/31/2012 plan review approved date,the
permit is technically expired and all remaining plan review fees are due and payable by the applicant. Ms. Uptegraft
indicated that she only submits the plans for review and once approved it goes to a project manager,therefore she gave
us your contact information.
Please provide me with status of this project so that we may proceed with one of the following options:
1. If project was cancelled,we will send you an invoice to collect payment for balance of plan review fees due and
expire the permit, or
2. If project was completed, we will send you an invoice for entire balance due, including permit fee. You will
provide us with payment in full and the name of a licensed contractor so that the permit can be issued.
Please note that if during a site visit we confirm that work was completed without a permit,we will begin code
enforcement action upon the applicant, building owner and the tenant occupying the space, and may assess fines, if
applicable.

expired 180 days past the submittal date of 1/4/2012.

It has been nearly one year since I last emailed you, and have received no response. I also left you a vmail message
earlier today to ask you about the status of this 2-year old project, and my call has not been returned.
I have attached an invoice for plan review fees past due on this project. As the applicant, you are responsible for
payment of the fees due. You may pay these fees by sending a check to City of Tigard — Building Division, Attn: Dianna
Howse, 13125 SW Hall Blvd, Tigard, OR 97223, or you can complete the attached payment authorization form with your
credit card information and fax (do not email)this form back to 503-598-1960 to my attention as soon as possible.
Because a permit has not been obtained within 180 days of the 1/4/2012 application date and numerous attempts to
contact you have gone unanswered, we will expire this permit application. We will also conduct a site visit to confirm
that the work outlined in the permit application has not been performed without a permit being issued. If so, we will
begin building code enforcement action upon the applicant, building owner and tenant occupying the space.
