Reference: GeoPacific Engineering, Inc., Geotechnical Investigation, Fern Hollow Court Lot 5,
Tigard, Oregon, October 4, 2012.
GeoPacific Engineering, Inc. is please to submit this summary report of geotechnical compliance
for the above-referenced project. We conducted on-call construction observation and testing
services during the construction of the project. Our services were performed August of 2014.
Field reports summarizing our observations and testing are attached.
CONSTRUCTION OBSERVATION
During construction, we provided periodic observation as the earthwork contractor, Endicott
Woods (EW), installed the vertical pipe piles, in order to verify that adequate depths into
competent soils were obtained. We observed that driving conditions in undisturbed native soils
was relatively similar for piles in all portions of the site. Based on our observations, the size and
spacing of the piles, and the depths of embedment into stiff soils were observed to be in general
accordance with the recommendations presented in our report.
We observed as EW performed compression testing on two of the piles in different areas of the
site. At 150 percent of the design load, one pile deflected a total of 0.035 inches and the other pile
deflected a total of 0.100 inches, which meets our recommendation of 0.3 inches of maximum
deflection.
CLOSING AND LIMITATIONS
Based on our observations, the vertical piles have been installed in general accordance with the
recommendations presented in our report, and with additional recommendations provided in the
field during construction.
Our conclusions and opinions as to whether the work essentially complies with the project
specifications are based on our observations and experience. The conclusions contained herein
represent conditions at the date of this report and apply only to our work with respect pile

8/22/14: GeoPacific Staff Engineer Ben Anderson on site to observe pile
installation. Earthwork subcontractor Endicott Woods (EW) on site installing 6"
diameter round steel piles 21 feet long. EW is starting the piles with a
smaller hammer (about 250-500 lbs) and plans to finish them off with a larger
hammer. After conversation with the geotechnical Engineer Jim Imbrie, Riverside
Homes, and EW, GeoPacific will return to the site to observe the finishing of
piles with the larger hammer. EW has installed several piles to 21 feet beneath
the existing ground surface in the central portion of the site but plans to
excavate to footing depth and drive the piles 3 to 4 feet further. 4 piles have
currently been started.
8/25/14 : GeoPacific Staff Engineer Ben Anderson on site to observe pile
installation. EW has installed all piles on the site to depths of approximately
20 feet below the footing depth, leaving 1 foot above the ground surface for
the pile caps. I observed as EW applied driving force to the piles with a
hammer weighing approximately 1000 lbs. I observed the driving of 5 piles in
various areas of the site, at different elevations. The piles consistently
moved approximately 3 inches in 10 to 15 seconds. The movement exceeds the
anticipated refusal criteria recommended in the geotech report. However, the
piles may exhibit adequate capacity. I recommended that two of the piles be
compression tested before evaluating other options.
8/26/14: GeoPacific Staff Engineer Ben Anderson on site to observe pile
installation. I observed as EW performed compression load testing on two piles.
The first pile was located at the SE corner of the proposed residence and the
second pile was located 9 feet east of the NW corner of the proposed residence.
The piles were loaded in increments to 150 percent of the design load, which
is 16 kips. At 150 percent of the design load (24 kips) , the first pile
deflected 0.035 inches total and the second pile deflected 0.100 inches total,
which meets our recommendations of 0.3 inches maximum deflection. Based on our
observations of driving conditions and load testing, all piles on the site have
been installed in general accordance with project specifications and our
recommendations and are able to support 16 kips per pile.

foundations. Observations by GeoPacific were performed to determine, in general, if the work is
being performed in accordance with applicable geotechnical recommendations. The fact that any
particular work has been observed does not waive the contractor's responsibility for the means
and methods of construction, job site safety, or to comply with the contract documents.
Locations indicated in our field reports are generally based on construction staking placed by
others, or information provided by the contractor at the time of our site visit. Surveying of test
locations or locations of site improvements, and establishment/verification of line and grade, are
beyond the scope of services provided by GeoPacific.
Within the limitations of scope, schedule, and budget, GeoPacific executed the scope of services
in accordance with generally accepted professional principals and practices in the field of
geotechnical engineering at the time the report was prepared. No warranty, expressed or implied,
is made.
