1 METRO Safety & Fire, Inc. 14324 SE Stark St. Portland, Oregon 97233 503-231-2999 Description of Scope of work for: Providence Health Systems — Scholls — 12442 SW Scholls Ferry Rd. - Tigard, Oregon — Fire Alarm Panel Emergency Replacement. As part of an emergency fire panel replacement, we changed the existing Edwards 5723B conventional fire panel with a FCI 7100 Addressable fire alarm panel. We were able to utilize the existing devices by using a AMM- 4SF zone module for the 3 zones and continue to use the existing Silent Knight 5495 notification appliance panel. When an alarm, or trouble condition is detected at the new fire alarm panel it will be retransmitted to the Central Station via a digital communicator. The system is monitored by a UL listed fire monitoring station. CI j O Approved7T]d 'GARD [ ►"`' Conditionally Approved [ ] See Letter to:Follow [ ] Attac ed ..
