Geotechnical recommendations for design and installation of the micropiles were provided in our May 15, 2014, report to you entitled, "Geotechnical Consultation Services, Apple Store at Washington Square, Tigard, Oregon." The general contractor for the project is Sajo Construction, Inc. of Montreal, Canada, and the specialty foundation contractor was PLi Systems, of Hillsboro, Oregon. Nine micropiles with a design load of 100 kips in both tension and compression were installed to provide a minimum 15-ft bond zone in the basalt rock that underlies the site. Installation and testing of the micropile foundations were observed on a part- to full-time basis by a geotechnical engineer from GRI between July 15 and 22, 2014. Our observations and findings for each site visit were summarized in a daily Site Visit Report. A copy of each report was provided to Apple and Sajo. In our opinion, the work observed by GRI was completed in conformance with the recommendations provided in our report and the project plans and specifications. Please contact the undersigned if you have any questions. Submitted for GRI, it% PRO1t 1 GINF - a 5453 W"..445, ' - , 1%, 1A4 23, le: QV ggAl G. K110 Renews 6/2015 Michael W. Kimball, PE,GE Client: Apple, Inc. Contractor: Sajo(general) PLi Systems(micropiles) Site Address: Washington Square Mall Building Permit No. City of Tigard#BUP2014-00130 GRI visited the site to observe verification testing on the production micropile at grid 36-U. We understand a total of 9 micropiles, with a design load of 100 kips will be installed to support the planned tenant improvements for the new Apple store. Bob Collins with Sajo (general) and Luis Maciel with PLi Systems (micropiles)were both present during the site visit. Based on our conversation with PLi, we understand the pile was installed early on July 12, 2014. After coring the concrete, the contractor pre-drilled approximately 13-ft below the elevation of the existing concrete slab using an 8-in.-diameter auger affixed to a portable drill. A 13-ft long,6-in.-diameter pipe was then placed in the hole; and drilling continued through the pipe with the auger until rock was encountered at about a depth of 14.5 ft. A down-hole hammer was then used to advance the boring to a depth of about 30 ft below the concrete. A full-length, epoxy coated, threaded #18 bar was inserted, and the hole was tremie grouted with neat cement grout. We understand a total of 8 bags,or about 9.2 ft3 of grout was placed in the hole. Verification testing consists of loading the pile in tension in increments of 25% of the design load, up to a maximum load of 200% of the design load, with pile head displacement measured at each increment. At 150% of the design load, the load is held for 10 minutes to check for creep, and readings are taken at 1, 2, 3,5, 6 and 10 minutes. The load is then returned to the alignment load and a final reading taken. The verification test is considered successful if creep is measured at less than 0.04 in.,total deflection at design load is less than 0.5 in., and no pull-out failure occurs. During the test, no creep was measured, total deflection at design load was 0.187 in., and no pull-out failure occurred. A copy of the verification test data form is attached. Based on our observations and the results of the verification test, it is our opinion the pile has been installed in conformance with project plans and specifications, and is suitable for foundation support. GRI discussed findings on site with Bob and Luis, and a copy of this report was rovided electronically to Bob 7/K...644.:40 Collins and Roberto Botelho with Apple, Inc. / ter Reviewed by: Date: Comments: Copies to: 5587-A SVR 07-15-14.docx G 1. 9750 SW Nimbus Avenue Beaverton,OR 97008-7172 pl 503-641-3478 fl 503-644-8034 PIN PILE VERIFICATION TESTING Job Name: A-ppl,_ S-fan-e... We understand a total of 9 micropiles, with a design load of 100 kips will be installed to support the planned tenant improvements for the new Apple store. The attached sketch shows the approximate location of the piles referenced in this report. Based on conversation with PLi, we understand piles located at gridlines S-36, T-36, V-36, and W-36 were predrilled yesterday to reach the top of the basalt rock which underlies the site at estimated depths of 13 to 17 ft. Drilling was accomplished using 8-in.-diameter auger affixed to a portable drill. Examination of the drilling spoils indicates the upper soil unit consists of brown silt with varying percentages of clay and fine-grained sand. The approximate depth to rock at each of the above-mentioned pile locations is summarized below: Pile Location Depth to Rock 00 S-36 13.5 T-36 13.0 V-36 14.0 W-36 14.5 Following pre-drilling to the rock, the permanent casing which extends from the existing slab to about the top of rock was installed. The permanent casing consists of a13-ft long, 6-in.-diameter steel pipe. GRI observed drilling to final tip elevation of pile S-36,and we understand T-36,V-36 and W-36 will also be drilled to final tip elevation tonight. A down-hole hammer inserted through the permanent casing was used to advance the boring from the pre-drill depth of 13.5 ft to a depth of about 31 ft. Drill cuttings were observed to consist of gray rock fragments/dust. GRI will return to the site tomorrow to observe placement of the reinforcing bar and grout. A total of 9 micropiles, with a design load of 100 kips will be installed to support the planned tenant improvements for the new Apple store. The attached sketch shows the approximate location of the piles referenced in this report. Based on conversation with PLi, we understand the piles at gridlines T-37, U-37, V-37, and W-37 were previously drilled to top of basalt rock.Additionally, the 13 ft., 6-in.diameter permanent steel pipe casings were also placed. PLi informed GRI that drilling in these locations was stopped at the top of basalt, which they reported to be between 14 ft.and 15 ft.from top of concrete. Tonight, GRI observed PLi drill to final tip elevation for pile T-37. A down-hole hammer was used to advance the boring from the pre-drilled depth to approximately 30 ft. below the top of the concrete. We understand that drilling for micropiles U-37, V-37, and W-37 will be performed with the same procedure. Drill cuttings were observed to consist of gray rock fragments and dust.
