Field Observations
1. The footings, stems walls and garage piers were set up for a monopour for Units 1 -8. All
vertical, horizontal reinforcing bars, pier ties and footing stirrups were found to be installed
in accordance with the plans. The footings for the garage piers on Lots 1 & 8 had not been
completed yet.
2. The garage piers are being poured 10" thick instead of the 8" thick as shown on the plans
allowing for nailers to be embedded in the concrete and to match the 10" width of the beam
seat at the top of the wall.
3. One of the vertical dowels on each beam seat on top of the garage piers was installed outside
the #3 ties.
4. The anchor bolts for the holddowns were installed as indicated on the plans. The strip
footing at the HHDQ11 holddowns on the back wall was thickened to at least 12" as required
on the plans except for the left corner of unit 2.
5. The plans had a detail added to bridge over tree roots to protect two trees. No roots were
encountered so this detail was not required.
Field Conflicts Resolved
1. The footings and stem walls were all found ready to pour, except for the garage pier footings
on Lots 1 & 8.
2. The garage piers are acceptable to be increased to 10" thickness.
3. The beams seats on top of the garage piers are acceptable as installed with one of the vertical
dowel rods outside of the #3 ties.

Field Observations

Mark E Butler P.E, S.E. the Engineer of Record from Butler. Consulting, Inc. performed the
special inspection for the installation of seven threaded rods in adhesive into the foundation at
the Barrows Road Estates project under construction on Lots 2 through 8 in Tigard, Oregon. The
rod required "fora Simpson HTT16 that left off "the plans.- ..�- - -
A 5 /8" 4) x 24" long threaded rod was to be installed in a 3 /4" 0 by 10" deep hole using Simpson
"SET" adhesive into the 6" concrete stem wall on the left side of each unit on Lots 2 through 8.
The end unit on Lot 1 does not have the footing poured yet and will use a different detail. Three
tubes of Simpson "SET" adhesive with were to be used all with serial number EZ- 35046 -RR and
an expiration date of 4/10. The holes were all blown out with compressed air prior to installing
the rod. The installation was found to be in accordance with the manufacturer's installation
instructions and ICC report ESR -1772 except on two units as follows:
1. The hole on Lot 7 was only 9W' deep.
2. The hole on Lot 2 was only 8W' deep due to encountering reinforcing steel in the wall.
The uplift load on the HTT16 holddown is only 3,321 lbs, less than the rated capacity of full
embedment. The holddown is installed on a framed wall sheathed with OSB supporting three
stories above which will reduce the net uplift load. Therefore, the two rods installed with 9W
and 8 of embedment depth were found to be acceptable as installed.

Rather lengthy field report from yesterday's site visit. A.couple things to note:
1. The proposed fix for the parallam beam stopping short of the stairwell of adding a 1 %" LSL
adjacent to the existing beam works.
2. The straps on the front shear walls can be moved to clear the beam scat on top of the concrete
pier provided that out -to -out dimension between the straps is at least 26
3. The HHDQ 1 1 holddowns on the back wall installed some 6" from the end of the shear wall
are acceptable as installed (The inspector request verification of that).
4. A serious issue that I did not catch until I got back to the office is that the 31/2 x 1.1 /s" LSL
header over the front porch supports has a significant uplift load at each end that needs to be
resisted. The left end needs an ECCQ44 post cap with an HTT16 holddown at the foundation.
This holddown was not called out on the plans. The holddown will need to be installed on a
'/s" x 10" embed threaded rod in epoxy with special inspection required. Since this holddown
should have been on the plans, 1 will offer to perform the special inspect at no charge rather
than having to pay Carlson Testing to do it. On unit 1 where the footing has not been
installed yet, the footing needs to be 24" x 24" x 12" deep with the post installed on an
embedded CB44 post base. The right end of the LSL porch header is more difficult. 1
provided a design sketch with 2 options. Look them over and let me know if either will
work.

This field report is for all 8 units of building 1. The observations and comment below are typical
• for all units except as specifically noted otherwise.
Field Observations
1. No headers were provided over the front and back windows on the 3' floor. The plans had
called out typical 4x8 headers.
2. The drag truss located 2' from the back wall did not have the A34 clips installed between the
bottom chord and the shear,wall below.
3. The type "DD" shear wall on the back of the 3` floor at the mechanical chase had not been
sheathed on the inside face yet. A 2x4 nailer was installed at the ceiling to support the fire
blocking at the top of the chase covering the wall top plates.
4. The H2.5 hurricane ties were observed to be installed on every truss as required on Unit 8.
The rest of the units were not checked.
5. The City of Tigard Building Inspector verified that the fire walls properly extended up to the
top of the crickets and that he had given prior approval to cover.
6. A 3 x 11 %" PSL spans across the unit supporting the 3' floor joists. The beam was
supposed to extend to a 4x6 post in the stairwell wall but on units 4 to 8, the beam stopped
• about 8" short with a 4x4 post was added beneath the end. The only support below is
provided by the 1'/4 x 11' /R" LSL beam in the 2" floor that supports the stair header and hung
from a 3 x 11'/8" PSL with an HI1 hanger. In units 1, 2 and 3, the 31/2 x 11' /s" PSL
extended to the 4x6 post with 2" of bearing provided.
7. The inspector questioned the load path of the MSTC52 holddown straps on front wall of the
3` floor. The straps at each end were wrapped around a cantilever 3 x 11' /s" LSL rim
beam. The two interior straps did not have any cantilever beam to wrap around.

8. The 2" floor closet door was relocated from the landing to the living room side as shown on
previously issued design sketch SK-10. The double -sided type DD shear wall was installed
with holddown straps installed on double 2x or 4x studs below.
9. No solid bearing is provided to support the stair landing beyond legder nails into the
perimeter framing. Solid bearing need to be added.
10. The holddown straps on the rear shear walls do not all line up from floor to floor.
11. The straps on the 2 floor front wall were called out to wrap around the garage door glulam
header. However, the steel beam scats embedded in the tops of the concrete piers prevent the
strap from being installed. An alternate design is required.
12. The HHDQI 1 holddowns on the rear wall were installed on 4x4 studs facing inward up to 6"
. from the ends of the shear walls. The loads will need to be checked for the shortened
moment arm to resist overturning. 1n some cases, the holddowns were installed on built -up
2x studs.
13. The 3'/2 x 11' /a" header over the front porch was to bear on top of the glulam garage door
header. However, this beam was raised up to allow for an 8' tall garage door. The end of the
glulam beam was notched to provide partial bearing for the LSL porch header with a 2x6 PT
squash block installed on top of the concrete pier to provide full bearing. The porch header
supports the 2 floor shear wall above. A Load path for the uplift from the shear wall
reaction is not provided.
14. At the front corners where the ECCQ columns caps were installed the double top plate had to
be cut. A Simpson strap was bent and installed on the inside corner to tie the walls together.
15. The door between the front entry and garage was shifted 8" to provide space for the electrical
panel. This resulted m an 8" long section of wall with a single sill plate anchor at the corner.
This 8" long wall section will be on the strike side of the door hinge and does not provide
much support for the door.
16. The 4x4 posts in the wall between the garage and bonus room were not installed. This post
would land on top of or adjacent to a plumbing stack. The post supports a 4x4 post with
double 2x4 trimmers extending from the 3" l floor.
17. On Unit 6, one of the rear deck pier pads had to be moved 12" to clear a plumbing cleanout.
This will result in the deck beam cantilevering 24" instead of 12" as designed.
18. Unit 3 ha.d a vertical cut in the exterior sheathing on the 1 " floor back wall. The cut was
adjacent to the holddown and weakens the wall.

19. Unit 1 at the front corner has no support for the 31/4 x 11' /R" LSL porch header. The footings
have not been pourcd yet with the intent of pouring the footings monolithic with the porch
slab.
Field Conflicts Resolved
1. 4x8 headers need to be installed over the front and back windows on the 3 floor.
2. The Simpson A34 clips need to be installed between the drag truss and the type DD shear
wall on the 3 floor per detail 11 /S7.
3. Sheathing will need to be installed on the inside face of the type DD shear wall on the 3rd
floor at the mechanical chase. Solid blocking will bc required at the top of the wall to
receive edge nailing for the sheathing.
4. The installation of the glulam beams reinforcing the top chord of the bowstring trusses was
acceptable as installed. The revised configuration of the through -bolts maintained the
minimum 3" edge distance as required.
7. The holddown straps on the front shear walls are to be wrapped around the continuous P/4 x
1 1 %" LSL rim joist spanning between the 31/2 x 11 LSL cantilever rims as shown on detail
8 /S7. Blocking needs to be added on the back side of the LSL to provide a nailing surface
for the straps.
8. The 1“ floor shear wall beneath the shortened type DD shear wall on the 2„d floor needs to
have edge nailing added the full height of the studs that the straps from above are attached to.
In addition, the mechanical room side of the wall needs to be sheathed between the strap
from above and the wall.
9. The 2 "d floor stair landing can bc supported by adding (2) LSTA24 straps from the landing
joists up to the 1'/4 x 1 1'/8" LSL beam above.
10. Where the holddown straps on the rear shear walls do not all line up from floor to floor, the
straps need to be attached to a double 2x or 4x stud on the floor below with edge nailing
extending he full height ofthc studs.
12. The built -up 2x studs that the holddowns are installed on need to be stitch - nailed together
with (2) 16d nails spaced at 12" on center.
13. The support of the 31/4 x 11'/8" LSL porch header with the notched glulam and squash block
was acceptable as installed for vertical support. However, a load path to resist uplift is still
required.

14. The Simpson straps installed at the corners where the top plates were cut out due to the
E.CCQ column caps were acceptable as installed, although to be most effective, the straps
should be installed on the outside face.
15. The 8" long section of wall adjacent to the door between the entry and garage can be
stabilized by removing the trimmer stud and replacing with a 2x4 PT stud bearing on the
floor slab. The end can then be nailed into the mud sill to help stabilize the wall.
16. The missing 4x4 posts in the back garage wall can be replaced by double 2x6 studs stitch -
nailed together. The double studs need to be located beneath the 4x4 post or double 2x4
trimmers supporting the 3' floor beam.
17. Moving the one deck pier on Unit 6 resulting in a 24" cantilever on the deck beam is
acceptable.
18. The section of sheathing with the vertical cut on the back wall of unit 6 needs to be removed
and replaced. Blocking will need to be added at the horizontal joint with edge nailing
installed on all sides of the replaced sheathing.
Outstanding Items
6. The 31/2 x 11 PSL beam on the 3"' floor of units 1 — 3 with just 2" of bearing on the 4x6
post at the stairwell wall was found to be adequate as installed. On units 4 — 8 where the
beam stops short of the stairwell wall, a 1 Va x 1 1 /B LSL will need to be added adjacent to the
cxisting 1' /a x.11' /a" LSL in the 2 " floor below supporting the stair landing. This double LSL
beams will need to be stitch- nailed to the the adjacent LSL with 16d nails spaced at 12" on
center top and bottom and hung from the PSL beam with an HGUS4110 hanger.
11. The straps on the front 2 " floor shear walls that cannot be installed because of interference
with the embedded steel beam seats can be moved over to clear the beam seat provided the
minimum out -to -out dimension of the pair of straps is at least 26V2". Double studs will need
to be added in the shear wall above with edge - nailing extending the full height of the studs.
See attached design sketch SK-11.
12. The HHDQ1 I holddowns on the rear wall installed up to 6" from the ends of the shcar walls
were found to be acceptable as installed.
13. The 3'/2 x 11 LSL porch header needs to be supported on the left end on a 4x4 post with an
ECCQ44 post cap installed. A Simpson HTT 16 (or PHD2) needs to be installed at the post
base on a Ye" 4) x 10" embed threaded rod in Simpson "SET" adhesive. Special inspection
will be required. The right end needs to have uplift resistance provide by one of the
following methods. See the attached design sketch SK-11.

A) Install an HDA8 holddown on a ' /e" 4) threaded rod through the LSL beam with (2) ' /s x
4" embed concrete expansion anchors drilled into the end of the concrete pier.
B) Add a 4x4 PT HF #2 post on the end of the concrete pier with (5) VA" x 3" embed
concrete expansion anchors into the pier with an MSTC28 strap on each side of the post.
19. The 31/2 x 11'8" LSL porch header on Unit 1 needs to be supported on the left end on a 4x4
post with an ECCQ44 post cap installed as on item 13 above. The post will need to bear on a
24" x 24" x 12" footing with a CB44 embedded post base.

This Project Memorandum is issued to Milestone Homes, LLC for the Barrows Road Estates
project under construction on SW Barrows Road in Tigard, Oregon regarding relocating a closet
door on the 2" floor.
The door opening for the coat closet at the top of the stairs is being relocated from the stair side
to the living room side. This will shorten the type "D2" shear wall on the 2 " floor requiring it to
be upgraded to a double- sided type "DD /6" wall. The door will. need to be shifted slightly
towards the party wall for the smear wall to be a minimum of 2' -7" long. A CMST14 strap will
be required on each side at each end of the shear walls nailed to the double 2x stud.s. Studs will
need to be added in the 1'' floor wall below as required.
