This letter is to certify that all requirements of building permit BUP2004- 00454, issued
for a building shell, have been completed. The final inspection was performed and
approved on November 9, 2005, by inspectors from the City of Tigard. No tenant
spaces are included in this permit, nor shall any tenant improvement be occupied until
such time as each space is approved by final inspection of its specific permits, approved
for the use intended and provided with a Certificate of Occupancy.
The City neither guarantees nor warrants to the owner, occupant or any other person
that this letter evidences strict and complete compliance with each and every ordinance
or regulation of the City or the State of Oregon affecting the construction or use of said
structure or the land upon which it is situated. Such compliance is the responsibility of
the owner and /or occupant of the premises.
This letter certifies only that the work covered under the permit number listed above has
been completed. It is not permission to occupy tenant spaces.

Clark Pacific has a contract with Howard S. Wright Construction Company to manufacture at our West
Sacramento plant, Precast Prestressed Concrete products for the Washington Square Mall Parking
Structure Expansion "B." Production will begin this week at our plant in West Sacramento, California.
UBC Section 1701 requires that the owner provides special inspection for concrete and steel materials .
manufactured offsite. However, section 1701.7 allows for this inspection to be waived if the plant is
classified as an "approved fabricator" by the Building Official. This is also outlined in the Tigard Municipal
Code section 14.06.080.B.2.
Clark Pacific has been granted this "approved fabricator" status on specific projects in various cities. San
Diego, San Francisco, Seattle, Sacramento, Phoenix, and Pleasanton are just a few of the many cities
that have granted this approval based on our comprehensive in plant inspection program and PCI plant
certification. We have been classified as an "approved fabricator" in the City of Los Angeles for over 15
years on a continuous basis for all projects. The outlying cities in the Los Angeles area such' as Burbank,
Santa Monica, and Glendale all recognize the City of Los Angeles approval for work in their cities. We
were contracted for the work on the previous phase of this project and had inspection services by Carlson
Testing Lab, which can verify our, qualifications in this area. I have enclosed materials for documentation
of Clark Pacific's qualifications as an "approved fabricator" and copies of other City certificates for your

1701.7.1 QC Manual - We have submitted a copy of our QC Manual. I have
enclosed a copy of our current personnel list with certifications for your
reference.
1701.7.2 Approved Inspection Agency — We submit the enclosed information for
Ross Bryan Associates. This agency inspects our plant twice per year
for 2 days each visit. Their PCI inspection program is approved by ICBO
for use in this section. Their PCI ICBO certification is enclosed.
1701.7.3 Periodic Inspections — I have enclosed our latest PCI inspection report
documenting our PCI plant certification granted by PCI based on Ross
Bryan Associates' inspection.
1701.7.4 Notification. — Clark Pacific agrees to notify the City of any changes to
the Manual.
We request approval of Clark Pacific as an "approved fabricator" for the City of Tigard under section
1701.7. We look forward to working with the City of Tigard on this project.

The plan review was performed under the State of Oregon Structural Specialty Code (OSSC)
2004 edition; and the Tualatin Valley Fire & Rescue Ordinance 99 -01 (TVFR99 -01) 1999

1. This permit is for construction of the Parking Garage and the Shell of the Mall.
Separate permits will be required for the interior build out of tenant spaces. Addresses

2. The Mechanical sheets are approved under MEC2004- 00636. Although included,

3. All exit doors shall be openable from the inside without the use of a key or any
special knowledge or effort. 1003.3.1.8 OSSC
4. Exit signs shall be illuminated at all times and shall be provided with a second source
of power that will provide 1 '/2 hours of illumination in case of primary power loss.
1003.2.8.5 OSSC FYI Additional exit signs may be required by the city field
inspector to clearly indicate the path of egress.
5. The means of egress shall be provided with illumination of not less than 1 foot candle

at the floor level anytime the building is occupied. The means of egress illumination
shall be provided with a second source of power in case of primary power loss.

• Deferred submittals such as Fire Alarm, Fire Sprinkler, Truss Joist Shop
Drawings, etc, will be charged a deferred submittal fee based on the valuation
of the portion of the work being deferred. The minimum fee shall be $200.00.
Special Inspection: Special inspection is required for items listed on sheet SO.1. The
special inspection agency of record, Carlson Testing, shall furnish inspection reports to the
Engineer of Record, Dan Munn @ DLR Group, the General Contractor, Howard S.
Wright. and the City of Tigard, Building Division, attention Hap Watkins. All
discrepancies shall be brought to the immediate attention of the general contractor for
correction. The special inspector shall submit a final signed report stating whether the work
requiring special inspection was, to the best of the inspector's knowledge, in conformance
with the approved plans and specifications and the applicable workmanship provisions of the
code. 1701.3 OSSC
American with Disabilities Act (ADA): It shall be the responsibility of the Architect,
Engineer, Designer, Contractor, Owner and Lessee to research the applicability of the ADA
requirements for the structure. The City of Tigard reviews the plans and inspects the structure
only for compliance with Chapter 11 of the OSSC which may not include all of the
requirements of the ADA.
Approved Plans: 1 set of approved plans, bearing the City of Tigard approval stamp, shall
be maintained on the jobsite. The plans shall be available to the Building Division inspectors
throughout all phases of construction. 106.4.2 OSSC
Certificate of Occupancy: No building or structure shall be used or occupied until the
Building Official has issued a certificate of occupancy 109.1 OSSC
Premises Identification: Approved numbers or addresses shall be provided for all new
buildings in such a position as to be plainly visible and legible from the street or road
fronting the property.
When submitting revised drawings or additional information, please attach a copy of the
enclosed City of Tigard, Letter of Transmittal. The letter of transmittal assists the City of
Tigard in tracking and • ocessing the documents.

Utilities Under Building Slabs
Dear Mike
Per cur conversation of January 18. 2005 we are proceeding with pouting abandoned existing utilities
under the building slabs rather than full removal. As indicated these utilities are not connected to live
systems and removal would be disruptive and expensive.
We will be issuing a change directive to modify note 5 on sheet C4.OB to allow either removal or filling
with grout as a means of dealing with abandoned utilities under the slab. It is my understanding from our
conversation that you concur with this approach. Please sign and fax back your agreement with this change.

Comment 1:
The wind girts (pages CCF1.34- CCF1.39) were checked for bending stress and deflection at
100 mph wind speed. See the additional talc attached showing the bending stress check at
the worst case girt. The stress in the girts did not control, and all other girls were checked
for deflection. For deflection, we used a service level load. For this, the importance factor
was 1.0 and an 85 mph wind speed corresponding to a 10 second wind gust was assumed.
Comment 2:
The revised braced frame talcs (pages CCF1.47- CCF1.53) are for the critical case for the
single brace at the lower level. See the additional talc attached for the HSS6x6x3 /8 braces
at the upper level. The X -brace is checked for the current condition as well as the future
loads. The axial loads are given in the original FastFrame models. See original frame
member calculation page CCF1.53.
Comment 3:
The diaphragm shear has been conservatively calculated in the attached sheets. There will
be future changes to the drawings as follows: Drag strut plates will be added along grid
40.7 between MM and RR. The diaphragm attachment plan will be revised to include type
B attachment between grids 40.7 and 44.7 and grids MM and RR.
