Alpine View Subdivision
Northwest GEO Consultants provided geotechnical special inspections of home foundations and
segmental block retaining walls constructed at Alpine View in Tigard, Oregon. The observations were
conducted on an on -call, intermittent basis to confirm that construction was completed in general
conformance with the recommendations provide in our project geotechnical evaluation and
Segmental Retaining Wall (SRW) design.
Our involvement began on May 14, 2012 and ended on November 27, 2012. We completed 13 site
visits, observing foundation subgrade preparations for 11 homes and SRW construction of 2
permitted walls on lots 3/22 and 6/29.
Based on our observations, as documented in the attached Field Observation Reports, it is our
opinion that the construction was completed in general conformance with the engineering design
and project specifications and will perform as intended.

I made this site visit-as requested by Perry Wagoner to observe the conditions of subgrade for single family
homes that will be constructed on Lots 32 and 35. I spoke with Mr. Wagoner over the phone.
At the time of my visit, the footings had been excavated to subgrade. On lot 35, subgrade was covered with a
thin layer of uncompacted crushed rock. I observed and probed the subgrade using a 1/2-inch diameter steel
foundation probe. The subgrade in both lots consisted of very stiff to hard silt with trace fine sand.
Based on observation and probing, the subgrade excavations have been prepared in general conformance with
the project specifications and my recommendations and will perform as intended to support single family home
foundations. The areas are ready for placement of footing forms and reinforcement.

1 made this site visit as requested by Perry Wagoner to observe the conditions of subgrade for a single family
home that will be constructed on Lot 1. I spoke with Mr. Wagoner over the phone.
At the time of my visit, about 3/4 of the site had been excavated to subgrade. The contractor had departed for
the day and will return tomorrow when the dump site for disposing of excess soil is open.
I observed and probed the subgrade that was completed using a 1 /2 -inch diameter steel foundation probe. The
native subgrade in most of the lot consisted of very stiff to hard silt with trace fine sand. In the eastern portion
of the lot the soil was soft and appeared to have been disturbed by removal of a large stump. I spoke with Mr.
Wagoner about the need to overexcavate to remove the soft soil in this area.
1 will return to the site tomorrow to check the remaining areas and the area where the soft soil is located.

home that will be constructed on Lot 19. I spoke with Mr. Wagoner over the phone.
At the time of my visit, the contractor had removed the large tree stump and excavated the soft soil that was
located on the front east portion of the lot. The excavation was approximately 2 -feet below subgrade elevation
where the stump had been. The contractor expected about two loads of Tigard Sand & Gravel %" -minus crushed
rock will be used as fill. Tigard Sand & Gravel is to saturate the crushed rock at the pit before delivery.
The rock should be compacted until well keyed using a hoe -pac on the excavator.
Based on observation and probing, the subgrade exposed subgrade is firm medium stiff to hard native silt. After
the crushed rock is placed and compacted the area will be prepared in general conformance with the project
specifications and my recommendations and will perform as intended to support single family home
foundations.

that will be constructed on Lots 23 and 29. I spoke with Mr. Martin over the phone.
At the time of my visit, the contractor had been excavated to subgrade elevation. I observed and probed the
subgrade using a 1/2-inch diameter steel foundation probe. The subgrade in the lots consisted of medium stiff
to hard native silt. !observed an area of loose soil approximately 7 feet in length located underneath the footing
line In the southeast corner of lot 29. The soft area appeared to be from a tree stump hole that was loosely
backfilled. I recommended and observed the contractor excavate the soft soil to a depth of 1- foot and
recompact with crushed rock.
Based on observation and probing, the subgrade excavations have been prepared in general conformance with
the project specifications and my recommendations and will perform as intended to support single family home
foundations. The areas are ready for placement of footing forms and reinforcement.

home that will be constructed on Lot ?92 spoke with Mr. Russell over the phone,
At the time of my visit, the contractor had been excavated to subgrade elevation. I observed and probed the
subgrade using a 1 /2 -inch diameter steel foundation probe. The subgrade in the front of the lot consists of very
stiff to hard native silt. In the back of the lot, subgrade consists of very stiff to hard residual soil.
Based on observation and probing, the subgrade excavations have been prepared in general conformance with
the project specifications and my recommendations and will perform as intended to support single family home
foundations. The areas are ready for placement of footing forms and reinforcement.

I made this site visit as requested by Don Durst to observe the conditions of subgrade for single family homes

1 made this visit at the request of Don Durst (as communicated to Brad Hupy PE, GE) to observe foundation
excavation subgrade conditions for single - family home construction on Lot 5 located at 13369 SW Ouzel Lane.
While on site, I spoke with Mr. Durst over the phone.
At the time of my site visit, the foundation excavation had been completed to an approximate depth of 2 to 4 feet,
and foundation forms were partially set. I observed the subgrade soils visually and probed the near surface soils
with a steel shaft probe. The subgrade consists of native, clayey silt soil with a very-stiff consistency.
Based on our observations and probing, the subgrade has been prepared in general conformance with the project
specifications and NW GEO Consultants LLC recommendations, and should perform as intended for support of
single - family home foundations as specified in our geotechnical report. dated March 5, 2012. The excavation
subgrade is ready for placement of foundation forms. /

I made this visit as requested by Kristian Ingebrigtsen to observe foundation excavation subgrade conditions for
an Allen Block retaining wall. I spoke with Mr. Ingebrigtsen over the phone, while on site.
At the time of my site visit, the foundation excavation had been completed to an approximate depth of 1 to 4 feet,
and the lower two courses of blocks had been placed on a crushed aggregate base. I observed the subgrade soils
visually and probed the near surface soils with a steel shaft probe. The subgrade consists of stiff to very- stiff,
clayey silt soil.
The contractor representative reported that the wall is to be constructed six blocks high with a 2H:1 V slope above.
Geotextile reinforcing grid 3 to 4 feet long is placed between first and second blocks, and consists of STRATAGRID
350 per label on roll. 3 inch diameter perforated plastic subdrain pipe is placed behind wall. Block cells on
northern end of wall below reinforcing grid were not filled with crushed aggregate. I informed contractor
representative that cells should be filled. Observed contractor begin unstacking this portion of wall to perform
recommended corrective work.
Scheduled NGC soil technician, John MacDonald, to visit the site in afternoon to perform compaction testing, and
confirmed compaction test visit schedule with Mr. Ingebrigtsen and Trademark representative.
Based on our observations and probing, the subgrade has been prepared in general performance with the project
specifications and NW GEO Consultants LLC recommendations, and should perform as intended for support of
flexible, segmental block retaining wall systems.

CONTRACTOR ACTIVITY, OBSERVATIONS, CONCLUSIONS. RECOMMENDATIONS:
I made this site visit as requested by Kristian Ingebrigtsen to observe the condition of subgrade for an MSE wall
that will be reconstructed in the back yard of Lot 6. The retaining wall is being reconstructed to add about 30
feet in length to the existing wall. Wall total height will be 3 blocks high with a 2H:1 V backfill in the back.
At the time of my visit, the contractor had prepared the granular pad and first layer of geogrid had been
constructed. I observed and probed the subgrade in the front of the wall using a 1 /2 -inch diameter steel
foundation probe. The subgrade consisted of medium stiff to stiff silt. We will re -visit later today or tomorrow
morning to test density of backfill and check geogrid /drains placement.
Based on observation and probing, the construction of the MSE wall at this point have been prepared in general
conformance with the project specifications and my recommendations and will perform as intended to support
MSE wall.
