The Child Care Division has concluded its assessment of a complaint that alleged you were in
violation of the rules for Registered Family Child Care.
On April 12, 2012 I made an unannounced visit to your facility. The results of the assessment are
outlined below. •
Issue #1: The complaint alleged that your facility is operating child care without a license.
According to OAR 414 - 300 - 0010(1), "Unless exempted by Oregon laws governing child care
facilities, no person or organization shall operate a child care center without a valid
certification issued by the Child Care Division (CCD)."
The owner stated that you have a preschool program that runs 5 days /week, from 9am -noon.
There is a 45 minutes dance /gymnastics activity in the daily routine. She showed me the daily
schedule. She said that for the preschool program the children must be 3 years of age by
January. There were a few 2 314- year -old- children in the beginning of the school year in
September but none at this time.
The owner also said that you have an after school program. It runs from after regular school
hours to 6 pm. The staff pick up the children from different schools in the area to attend your
program. There is a dance /gymnastics activity for 1 to 2 hrs, in addition to home work, crafts,
games and outside time in the after -shool -care daily schedule.
Your child care operation is not exempt from regulation.
This allegation has a status of valid .
For violations that were found valid, you will need to take immediate steps to come into
compliance with the rules. You must send a written explanation of how you have come into
compliance: The Division must receive the explanation within 30 days of the date of this letter and
may make another unannounced visit to verify that you are in compliance. Repeated or serious
non- compliance may result in a civil penalty as discussed.
You may submit a written response to this letter that will become part of your record. It will be
included whenever information about your compliance history is disclosed. We encourage you to
limit any response to one page, so we can read your exact words to anyone asking about
compliance history in your file.

The Division occasionally receives calls from the public asking about compliance history in a
facility's file. Information disclosed includes:
- Licensing history (examples include, but not limited to adults enrolled in the
Central Background Registry, training, last health and safety review, etc).
- Information about invalid complaints will not be disclosed.
- Information about unsubstantiated complaints will be disclosed for two years from
the time of the complaint.
- Information about valid complaints or other non - compliance issues will be
disclosed for a period of 10 years from the time of the complaint.
Although we only disclose the information as described above, all complaints remain part of your
permanent file as required by law.

Westside Dance & Gymnastics Academy After School Program
Westside has 2 vans that travel to local schools and pick up children.
These children are coming to Westside Academy for the soul purpose of taking a dance
or gymnastics class.
Students arrive, have a snack and change their clothes for class.
Each day Westside transports about 22 students to our facility.
After class students are supervised and do homework, games and crafts, until their
parents pick them up by 6 :00
Classes range from lhour to 2 hours long, depending on the students level.
