FIELD INSPECTION REPORT
DATES COVERED: September 16, 1999
PROJECT: Air Touch Cellular — Sequoia
ADDRESS: 15575 SW Sequoia Pkwy, St- ; i — Tigard, OR
INSPECTOR: G. Seamster
09- 16 -99: CTI representative was onsite for periodic inspection of structural steel, bolting and welding. The
following was noted:
1. Performed visual inspection of base plates and cap plates to tube steel columns (2), beam to beam clips,
and (2) beam braces. All welding appears to conform to details and AWS D1.1.
2. Verified all bolted connections were completed and tightened to a snug tight condition per engineer (see
attached). --
3. Reviewed welder certificates for Mike Case SSN #532 -92 -3787 and Darrel Kelm. Also verified material
certificates for steel used.
Weld procedures, welder qualifications, base plates and bolts tested were reviewed and materials were
verified.
Our reports pertain to the material tested /inspected only. Information contained herein is not to be reproduced,
except in full, without prior authorization from this office.

Daryl:
As per our conversations, the structural bolted connections and wedge anchors for the
above mentioned project are not required to be tension tested. Snug tight connections are
acceptable with all bolted connections.
Additionally, per our conversations, you indicated that you have tightened the structural
bolts and wedge anchor bolts to 325 ft lbs. and 175 ft. lbs. respectively. This condition is
acceptable as installed.

additional information.
