Foundation Subgrade Inspection Showers
THE FOLLOWING WAS NOTED:
NGI arrived on -site, as requested, to inspect the pier foundation excavations for the two monopole towers on the west
side of the building. The excavations were checked for diameter and depth of embedment. Both excavations were
constructed with 3 feet diameter steel casing in the bottom 9 feet of the excavation and formed with 3 feet diameter
sonotube in the upper approximate 4 feet. Embedment depths were measured to be approximately 10 feet and 11 feet
below top of pavement for the south and north pier, respectively. The foundation plans call for a 3 feet diameter pier with
a minimum embedment depth of 9 feet. NGI probed the bottom of the pier excavations and found on the order of 8 to
10 inches of loose soil (slough) in the excavations. Standing water was also present in both excavations. The contractor
reported that the excavations were dry at the time of drilling and that the water was from runoff from the roof of the
adjacent building. NGI recommended that the standing water and loose soil be removed prior to placement of the
reinforcing steel and concrete. The contractor will call NGI for re- inspection of the excavation once the holes are cleaned '
out. NGI will also provide reinforcing steel inspection at that time.
NGI returned to the site in the afternoon following cleaning of the excavations with a vacuum truck. The contractor
reported that they removed as much loose material as they could, but water and soil subsequently washed into the
excavation from below the casing during heavy rainfall. Probing of the excavations indicated approximately 12 to 16 •
inches of loose soil was present in addition to several feet of water. The contractor attempted to dewater the excavations
with a sump pump; however, this did not address the slough present in the excavation. The contractor elected to cancel
the pour and will reclean the excavations tomorrow morning.

THE FOLLOWING WAS NOTED:
NGI arrived on -site, as requested, to provide reinforced concrete inspection and a follow -up inspection of the subgrade
conditions for the pier foundations. The subgrade conditions remained to be brought in accordance with the
recommendations of the geotechnical report and the contractor elected to cancel today's concreting. While on -site, NGI
provided an inspection of the cage reinforcement prior to its insertion into the foundation excavation. The reinforcing cage
hoops had 4" extensions rather than the 5" extensions specified. This condition was discussed with and found acceptable by
Jim Jarrett, Valmont Microflect. To the best of NGI's knowledge, the subject reinforcing cage was fabricated in general
accordance with the project plans, specifications, pertinent codes, and the acceptance criteria of the design authority. NGI
informed Cole Graves, Site Tech, and Dave Fisher, VoiceStream Wireless, of all observations.

THE FOLLOWING WAS NOTED:
_ NGI arrived on -site, as requested, to provide reinforced concrete inspection and subgrade inspection for the pier foundations.
Loose soils and water were removed from the foundation excavation. NGI provided fresh properties tests and cast one set
of four compressive strength test cylinders on the load delivered. The test cylinders were stored in insulated cure boxes
on -site, overnight. The slump of the delivered load was 6 % ", exceeding the mix design requirements of 4 ± 1. The lower
portion of the south pier anchor bolts were wet set. The batch to discharge time reached 21/2 hours; however, the concrete
remained workable without a significant slump loss or temperature increase. To the best of NGI's knowledge, with the above
noted exceptions, the reinforced concrete operations occurred in general conformance with the project plans, specifications,
pertinent codes, and the acceptance criteria established by the design authority. NGI informed Cole Graves, Site Tech, of all
test results and observations.
