Purpose: Building slab subgrade preparation
I arrived onsite at the request of Perlo McCormack Pacific to observe subgrade preparation in
the building pad. I met with Mr. Dean Hickok, the project superintendent, during my visit. I
also met with representatives of Wilbur West during my visit. I departed the site at
approximately 0930 and returned at approximately 1600. The following is a summary of my
observations.
At the time of my arrival, I observed the existing concrete pond had been removed, along with much
of the associated utility piping on the north end of the pond. I observed subgrade conditions to be
variable, with stiff to very stiff native silt exposed in some in some areas, while I observed soft to
medium stiff silt and gravelly silt fill in others. I observed several trenches exposed during the
excavation process. I also observed several gravel wells at the terminus of the building downspouts.
The downspouts have been directing water into the area, resulting in wet subgrade conditions
throughout the area. Much of the areas where soft silt and gravelly silt fill was observed, particularly
where the soil is wet, will likely not support construction equipment during grading. For •
constructibility purposes, we recommend these areas be over -excavated and backfilied with
compacted crushed rock. The recycled aggregate used for an access road to the building pad would
be acceptable for use as well. We recommend soft subgrade areas be over - excavated no more than
18 inches below bottom of slab, at which time we may reevaluate the exposed subgrade and provide
further recommendations as necessary. I also recommended the project owner be involved in the
decision as to what depth we may over - excavate.
Upon my return in the afternoon, I observed excavations in the northern half of the new addition
extending at least 3 feet below subgrade elevation. According to the contractor, soft, saturated fill
and some existing utilities were removed from these areas. The exposed subgrade in this area
consists of stiff native silt and stiff silt and gravelly silt fill. I spoke with Dean and with Mr. Wilbur
West about soil conditions in this area. Over - excavation to this depth may not be necessary, and we
were not allowed to reevaluate subgrade conditions at 18 inches below bottom of slab, as we had
requested. However, based on observation, in our opinion, the subgrade observed has been
prepared in general accordance with our recommendations and project specifications.
I observed Wilbur West excavating near existing spread footings_ These footings are supporting the
second story overhangs of the existing building, which are planned for demolition sometime in the

next few weeks. However, these sections are still occupied at this time. I observed Wilbur West
excavating up to two of the four sides of these footings while over-excavating soft spots in the slab
area. We recommend the excavations do not expose the soil column directly beneath the footings.
Excavating the surrounding soil adjacent to the footings in this manner is unsafe, and may result in
catastrophic foundation failure. These areas should not be left open without temporary shoring,
underpinning, or other methods to assure the foundations are stable. I recommended to Wilbur
West and Dean Hickok the soil around the footings already exposed be immediately backfiiied and
compacted with gravel a distance of at least 4 feet out from the edge of the footing.

I arrived onsite at the request of Perlo McCormack Pacific to observe paving operations. I met
with Dean Hickok, the project superintendent, during my visit. I also met with Jerry Mortenson
of Mort's Construction during my visit. The following is a summary of my observations.
At the time of my arrival, I observed the planter had been removed from the northwest corner of the
existing parking lot, and a Keystone wall was being constructed in this area. I observed 4 rows of
blocks already in place on the eastern half of the walk No blocks had been placed for the western
half of the wall. Vertical cuts up to approximately 4 feet have been excavated as part of the wall
construction. I observed very stiff, damp native silt in the excavation walls. The excavation appears
stable. I observed a gravel pad approximately 6 inches thick being placed below the first level of with a
Keystone blocks for the western half �of observed a stockpile of clean, being

be used for wall backfill. Based on observation, In our opinion, the retaining wall Is being
constructed in general accordance with our recommenda$1lons and project specifications.
Several additional parking spaces are being created around the wall noted above, and throughout the
existing parking lot. I observed the subgrades for most of the spaces had been prepared arfd
covered with gravel prior to my visit. Where subgrades where exposed, I observed very stiff, damp
native silt. I probed the subgrade with a »Inch steel foundation probe, and was unable to penetrate
more than »inch with difficulty. Based on observation and probing, In our opinion, the subgrades
are being prepared in general conformance with our recommendations and project specifications.

of Mort's Construction during my visit. The following is a summary of my observations.
At the time of my arrival, i observed the Keystone wall has been constructed. I observed clean,
angular drain rock has been placed as wall backfill. Based on observation, In our opinion, the
retaining wall is being constructed in general accordance with our recommendations and project
specifications.
The additional parking spaces around the wall noted above and throughout the existing parking lot
have been rocked and the base rock compacted. The gravel consists of 1 -inch minus gravel supplied
by Tigard Sand and Gravel. I performed density tests In several of the parking stalls with a Troxler
3430 nuclear density gauge, comparing the test results to a modified proctor value of 135 pcf. The
test results indicate dry densities of approximately 129.2 to 131.3 pounds per cubic foot, which
translates to approximately 96% to 97% of the maximum dry density. Based on observation and
compaction test results, in our opinion, the subgrades are being prepared in general conformance
with our recommendations and project specifications.

grading operations. I observed native silt soil exposed throughout the area. I observed scattered
thin roots less than Winch in diameter In the subgrade. However, the vast majority of vegetation has
been removed in accordance with our recommendations and project specifications.

At the time of my arrival, I observed the Keystone wall along the eastern property boundary being
constructed. I observed clean, angular drain rock has been placed as wall backfill within an
approximate 1.5 -foot drainage zone along the back of the wall. I observed silty sand fill being
placed behind the wall beyond the drainage zone. The silty sand is being Imported from another
construction site. I had obtained a sample during my previous visit for laboratory testing.
Approximately 2 feet of fill has been placed. I performed density tests in the fill with a Troxler 3430
nuclear density gauge, comparing the test results to a modified proctor value of 126 pcf. The test
results Indicate dry densities of approximately 122.2 to 125.3 pounds per cubic foot, hi ch on
translates to approximately 97% to 99% of the maximum dry density (ASTM
observation and compaction test results, in our opinion, the fill is being placed in general
conformance with our recommendations and project specifications.
