We were surprised to receive the enclosed letters from the property owner at the above
address.
The above permits were taken out by AT & T Wireless Services (formerly Cellular One)
in 1994 for a cellular phone cell site. We were the general contractor and completed
construction early in 1995.
Ms. Letha Thomas was . the inspector on the project and was at the site (both at our
request and of her own volition) on numerous occasions for inspections.
If a "final" final inspection is required, I would be happy to meet your representative at
the site. Arrangements will need to be made in advance, however, with AT & T to get
access to the site, since it is a secure area. Please call me with a date and time and I
will make the necessary arrangements.

4) Completed form for Structural Special Inspections.

6) Receipt for application of erosion control plan to USA.

The plans for this project were reviewed for conformity with applicable
codes. Please submit the following items for completion of the plan
review process at your earliest convenience:
1. Submit Stamped caics for the 100 foot wood pole from an Oregon
registered structural engineer.
2. Submit an erosion control plan which complies with all of the
requirements from USA (Unified Sewage Agency)- location of
silt fence, detail for silt fence, general notes, etc.
3. Submit a letter of approval for the pre -fab unit from the
Building Codes Division in Salem Oregon.
4. Please complete the following form and return to the Building
Division.
5. Per sheet SD2, the SW wall of the pre -fab unit is
approximately 7 -8 feet from the property line. Table 5 -A (B2
occupancy) requires a one -hour fire - resistive wall less than
20 feet from property lines.
6. Please submit an extra copy of a site and erosion control plan
for USA.
Please make these corrections on the appropriate pages of the drawings
and resubmit three copies of each page to the City of Tigard for review.
This plan review does not include electrical or plumbing plan reviews.
Electrical concerns can be directed to Washington County at 640 -3470 and
plumbing concerns to Mike Sheehan at the City of Tigard at 639 -4171
extension 312.

This letter with enclosures is to certify that the equipment shelter being furnished to
Cellular One for : the referenced site has been plan checked and approved by the
Building Codes Agency. A plan approval number of M115-93-0798D was assigned by
(your) state plan reviewer.
An application for insignia and inspection has been filed with the state officials. An
inspection should be as soon performed as possible after receipt at the Cellular One
warehouse on or about October 4, 1994. Upon completion of a satisfactory inspection,
an official insignia will be affixed to the exterior wall near the door by the inspector.
If you have any additional thoughts or questions in regard to this unit, please feel free

AST! E119 Fire Test of Building Construction and Materials

Dear Mr. Raman:
This letter will serve as an interim report of the E119 fire resistance test which we
performed recently on your precast wall system, - consisting of a nominally 314°
layer of epoxy - concrete over 2 inches of polyurethane foam, with a nominally 1-
114' thick layer of epoxy-concrete (with embedded aggregate) on the exterior and
• with reinforced, cast columns and beams on the inside of the panel.
The wall was received as a 10' x 10' assembly and was directly mounted into a
masonry lied, restraining test frame. After being allowed to dry to equilibrium
(lees than 75% r.h. within the wall), the wall was placed in front of our large scale
E119 fire resistance wall furnace (with the interior side towards the flame, since
that was the weakest side), instrumented with thermocouples and exposed to the •
E119 time • temperature curve until failure by heat transmission. The wall was
then removed from the furnace and exposed to the standard hose stream test,
. which was successfully met.
The test wall achieved a fire resistance rating of 102 minutes. A full report of this
project will be issued following receipt of the final drawings of the wall interior.
