FIELD INSPECTION REPORT
DATES COVERED: December 31, 1998
PROJECT: Children's Village Day Care
ADDRESS: 10400 SW Park Avenue, Tigard, OR
INSPECTOR: R. Andree
12- 31 -98: CTI representative was onsite to perform an epoxy inspection on 5/8" diameter threaded rods
embedded 8" into the existing concrete foundation. Holes were cleaned with a vacuum. Epoxy was not
put into holes as contractor had difficulty mixing epoxy and chose not to continue.
Representative used an R -Meter #00966 to confirm that rebar was placed in forms. Checked two
locations along south wall #1 at 40' from east end of wall and #2 at center of wall. R -Meter readings
indicated verticals are on 18" center and horizontals are at 9" on center.
Performed a rebound hammer test (see attached) on retaining wall that was 3' 10" high and 8" thick.
Concrete was a 3000 psi mix with 3/4" aggregate. Areas were formed with plywood panels and snap ties.
Wall is exposed to weather on one side and back filled on the other. Used a Schmidt Hammer #1643 on
0° position or perpendicular to wall. Air temperature was 50 °F. #1 west retaining wall 13' from north end
average number was 32, psi 3310, #2 south wall 9' from southwest corner 3' up from footing, average
number was 33, psi 3510. Test was done in accordance, with ASTM C 805 -97.
