Meeting Summary 4:00 PM to 5:30 PM:
Meeting held on -site to discuss the status of the Project and to determine jurisdictional
responsibility.

The scope of this Project as defined by the City of Tigard is the conversion of a single -
family dwelling to business occupancy (R3 to B Occupancy), dentist office.
Permits issued from City of Tigard on December 15, 2003, include Building, Mechanical,
Site Plumbing & Plumbing. The Site Plumbing permit has been withdrawn.
Additional square footage that was not originally permitted by City of Tigard has been
designed and is in the process of being constructed.
The following is a summary of the discussions held and the action required by all parties in
attendance.

pulled. Permits for any new scope of work, not
already permitted by City of Tigard, will need to
be permitted by the City of King City. It appears
as indicated on the March '04 submitted plans
and calculations that, that the plans indicate the
removal of the siding and installation of new
lateral bracing / sheathing.
07.01.04 Engineering Use and design methodology has been altered, Dr. Sepp Submit with

1)Third floor (attic) will not be converted to an Plans
office space. The stairs will be removed and
the attic will be an unoccupied — unfinished
space.
2) Larger windows were installed in patient
lobby area.
3) Additional square footage to the permitted
floor plan as been added above the garage.
4) Steel beam shown on plans was constructed
with a wood glue -lam beam.
5) Removal of stair case (pending).

Based on the above noted design changes, Dr.
Sepp will need Engineer / Designer to revise
structural calculations and IFC drawings. Once
documents are completed, submit 4 -sets of
copies to the City of Tigard. City of Tigard will
determine what needs to be permitted by City of
King City and will forward accordingly. Once
the determination of what City of King City will
be reviewing and inspecting of the new scope of
work, CLAIR will contact Dr. Sepp and
coordinate the permits.

• The scope of this project is the conversion of a single - family dwelling to business
occupancy, dentist office. This review was performed under the provisions of the
Uniform Code for Building Conservation (UCBC), 1997 edition, Oregon Structural
Specialty Code (OSSC), 1998 edition and Uniform Fire Code (UFC), as amended
by Tualatin Valley Fire & Rescue, 1999 edition. The plans are approved subject
to the following
Plan Review Comments
1. The proposed use is a dentist office. The main floor will be utilized for
customers and the top floor will contain an administrative office for the
dentist. The lower floor and the remainder of the top floor will not be used
and may be future space. Any other modifications of this future space will
be under the jurisdiction of the City of King City.
2. The lateral analysis by Tim Covert, Structural Engineer, requires Simpson
Set Epoxy Anchors to be installed. Carlson Testing, Inc. has been
identified as the special inspection agency that will perform these
inspections. Please send reports to Hap Watkins, Inspection supervisor at

3. The frontage road improvements required by the Oregon Department of
Transportation shall be completed and approved prior to use or occupancy
of the building.

4. All land use conditions required by the City of King City shall be completed
and approved by King City prior to the use or occupancy of the building.
5. A minimum of (1) fire hydrant shall be located within 250 feet of all
portions of the building and at least (1) additional hydrant shall be located
within 500 feet of the closest portion of the building. UFC 903.4
6. Fire lanes as required by the Fire Marshal shall be marked and maintained
as required by ORS 98.810 through 98.812. UFC 901.4.5.1
7. An accessible entry with an accessible route connecting to the accessible
• parking space is required. If the existing door and threshold are to be
used, the threshold may be 3 /4 inch high if a beveled edge is provided on
each side. OSSC 1108.1.1.2 and 1113.3.6.2 -
8. The accessible entry doors and doors on the path of travel to the
• accessible restroom, as well as the restroom, shall have lever type
hardware that does not require tight grasping, pinching or twisting to
operate. OSSC 1109.3.2 and 1109.9.8
9. The accessible restroom shall be signed with the international symbol of
access as required by OSSC 1108.4.12.1
10.A minimum 2A:10BC fire extinguishers shall be provided in the building so
the travel distance to an extinguisher does not exceed 75 feet. UFC

11.A copy of the approved plans shall be on the job site at all times and
available to the City of Tigard inspectors for inspection purposes. OSSC
Section 106.4.2.
12.A final inspection and Certificate of Occupancy is required prior to
occupying for the intended use of this building or parts thereof. OSSC
Section 109.1.
