We have two AT &T permits that have been ready to issue since April and one since May. Normally plan review approval
is only good for 180 days. You can request another 180 day extension in writing and the first request will be granted. If
you are not going to proceed with the project we would appreciate it if you would process a request to void the
permit(s). If you want to void the permit(s) here is the link to the form: I:\Building\Forms\ReqPermitAction.doc.
If I do not get a response back within two weeks I will be expiring these permits. Let me know if you have any questions.

Hi George,
We just received payment for the AT &T antenna permit located at 8777 SW Burnham St. (permit number: BUP2011-
00059). However, we do not have a contractor listed on the application, so we will not be able to issue the
permit. Furthermore, someone will need to pick up the permit once the contractor has been provided since the plans
are too large for us to mail. If you have any questions for me, feel free to let me know.
