There eight pages following this FAX memo. The original building permit is marked #1 in bold and
shows the date of issue. The revised permit marked #2 is for revisions and additional fees.
The letter from Dr. Gerig to Gary Lampella marked #3 and dated 3/15/02 is the request that the
building be granted a certificate of occupancy. It also has a hand written response by Mr. Lampella
dated 3/19/02 saying that a certificate of occupancy would be granted if all final inspections were
approved. I remember that though the walkway to the public way was not installed at this time, it
would be reviewed for exception when the plans came in for the first tenant improvement.
The inspection report dated 3/20/02 marked #4 showing final approval for the building permit
BUP2000 -00403 was subsequently revised to "fail" by the senior inspector Rick Bolen because
there was still the outstanding issue of the path to the public way. Somewhere along this line, there
was no official conditional certificate of occupancy issued. Dr. Gerig occupied the premises in the
last week of March 2002.
The permit BUP2003 -00691 marked #5 was issued for a tenant improvement for suite 103. During
plan review it was determined that the occupant load of that part of the building not occupied by
Dr. Gerig would not require the exit corridor and the path to the public way was not required.
The inspection report dated 2/3/04 marked #6 was generated after it was clear that a permanent
certificate of occupancy could, in fact, be granted for permit BUP2000- 00403.
The certificate of occupancy marked #7 was issued 2/4/04.

The agreement settled on is that the requirement for building a corridor exit to the city
sidewalk on the south property line of my property be postponed until application for
tenant improvements in my building are made. This will allow time to coordinate with
my neighbor to the south in making the required improvements to our shared property i

improvements, 1 would also like to request that any and all permit/review fees be
waived by the city for these improvements.
