To whom it may concern:
This letter is to certify that all requirements of building permit #BUP2003- 00445, issued
for a building shell, have been completed. The final inspection was performed and
approved on 08- 20 -04, by inspectors from the City of Tigard. No tenant spaces are
included in this permit, nor shall any tenant improvement be occupied until such time as
each space is approved by final inspection of its specific permits, approved for the use
intended and provided with a Certificate of Occupancy.
The City neither guarantees nor warrants to the owner, occupant or any other person
that this letter evidences strict and complete compliance with each and every ordinance
or regulation of the City or the State of Oregon affecting the construction or use of said
structure or the land upon which it is situated. Such compliance is the responsibility of
the owner and /or occupant of the premises.
This letter certifies only that the work covered under the permit number listed above has
been completed. It is not permission to occupy tenant spaces.

TRAFFIC IMPACT FEE FOR WILLOWBROOK, BUILDING F — DENTAL CLINIC
Enclosed with this letter you will find a calculation sheet showing the computation that has
been performed to determine the amount of the Traffic Impact Fee (TIF) to be paid for the
project noted above. The amount of the TIF is $31,595.00.
You have three payment options available to you. The first is to pay the TIF at the time you
are issued a building permit. The second is to arrange for payment over time by signing a
promissory note (if you wish to exercise this second option please contact me for additional
details). The third option is to defer payment until occupancy. Please note with the third
option, that Traffic Impact Fees are subject to an annual increase of up to 6% if not paid or
financed prior to July 1 of each year.
You may appeal the discretionary decisions made in determining the appropriate category
and the amount of the fee based on that category. A notice of appeal must be received by
the City Recorder no later than 5:00 p.m. on August 7, 2003 and must be accompanied by
the $1,170.00 appeal fee required by Washington County. Although filed with the City
Recorder, an appeal would be heard by the Washington County Hearings Officer.
Also enclosed with this letter, you will find a "Countywide Traffic Impact Fee Payment Option
Form ". Please choose a payment option, obtain the necessary signatures, and return to me
as soon as possible. We must receive this form before permits can be issued.

The plan review was performed under the State of Oregon Structural Specialty Code (OSSC)
1998 edition; and the Tualatin Valley Fire & Rescue Ordinance 99 -01 (TVFR99 -01) 1999
edition. The submitted plans are approved subject to the following conditions.
• Provide 2 -A rated fire extinguishers so that no usable space has a distance greater
than 75 feet to the extinguisher. Standard 10 -1, Chapter 3 TVFR99 -01.
• Deferred submittals such as Fire Alarm, Fire Sprinkler, Truss Joist Shop Drawings,
etc, will be charged a deferred submittal fee based on the valuation of the portion of
the work being deferred. The minimum fee shall be $200.00.
Special Inspection: Special inspection is required for items listed on sheet S4.2 The special
inspection agency of record, shall furnish inspection reports to the Engineer of Record,
Afghan Associates, Inc., the General Contractor, and the City of Tigard, Building Division,
attention Hap Watkins. All discrepancies shall be brought to the immediate attention of the
general contractor for correction. The special inspector shall submit a fmal signed report
stating whether the work requiring special inspection was, to the best of the inspector's
knowledge, in conformance with the approved plans and specifications and the applicable
workmanship provisions of the code. 1701.3 OSSC
American with Disabilities Act (ADA): It shall be the responsibility of the Architect,
Engineer, Designer, Contractor, Owner and Lessee to research the applicability of the ADA
requirements for the structure. The City of Tigard reviews the plans and inspects the structure
only for compliance with Chapter 11 of the OSSC which may not include all of the
requirements of the ADA.

Live Loads Posted: The live loads for which each floor has been designed shall be
conspicuously posted by the owner in that part of each story in which they apply, using
durable metal signs, and it shall be unlawful to remove or deface such notices. The occupant
of the building shall be responsible for keeping the actual load below the allowable limits.

Approved Plans: 1 set of approved plans, bearing the City of Tigard approval stamp, shall
be maintained on the jobsite. The plans shall be available to the Building Division inspectors
throughout all phases of construction. 106.4.2 OSSC
Certificate of Occupancy: No building or structure shall be used or occupied until the
Building Official has issued a certificate of occupancy 109.1 OSSC
Premises Identification: Approved numbers or addresses shall be provided for all new
buildings in such a position as to be plainly visible and legible from the street or road
fronting the property.
When submitting revised drawings or additional information, please attach a copy of the
enclosed City of Tigard, Letter of Transmittal. The letter of transmittal assists the City of
Tigard in tracking and • •. cessing the documents.
