(CTI) has performed observations and testing associated with the above- referenced site. Since the governing jurisdiction did not provide the specific requirements for special inspections, our observation and testing services were provided in general accordance with the City of Portland OPDR requirements. Our site visits were made between October 11 and 12, 2007. Copies of our field inspection reports, report of in -place density tests, and laboratory results are attached. CTI has not performed a geotechnical investigation of this site and is not aware of a geotechnical report by others for this particular project. Accordingly, CTI is not the geotechnical Engineer -of- Record and has no knowledge of subsurface soil conditions of this site. Our observation and testing services for this project are provided as called for in City of Portland Title 24 requirements for Soils Special Inspection. CTI performed the following Special Inspection items: 1) Verify removal of unsuitable material or soils prior to the placement of fill. 2) Approve the fill material prior to placement of fill. 3) Test the compaction of the fill every two feet of elevation or every 250 cubic yards, whichever provides the greatest number of tests. With respect to item 1: On October 11, 2007, CTI observed that the basement excavation was flat and free of demolition debris. The concrete slab and walls from the former basement were removed from the site. Based on our observations, the basement excavation has been prepared in accordance with the City of Portland — OPDR guidelines. With respect to item 2: % -inch minus crushed rock imported from Tigard Sand and Gravel, which met the requirements listed in The City of Portland's Title 24 for fills (24.70.080), was used to backfill the excavation. With respect to item 3: A total of 4 in -place relative density tests were performed on the backfill material for the excavation on October 12, 2007. The tests were performed at 4 feet, and 1 feet below the surrounding grade. Results in dry density for the tests ranged from 90% to 98% of the materials maximum dry density as determined by American Society for Testing and Materials (ASTM) D -1557. Based on the City of Portland Title 24 requirements, and our discussions with our client, Forrest Johnson, the minimum compaction percentage is 90% of Final Summary Letter T0705749 Permit No. Unavailable November 7, 2007 Page 2 of 2 ASTM D -1557. Therefore, all the in -place relative density tests met the minimum compaction specification. In addition to in -place relative density tests, CTI observed a proof roll of the backfill at an elevation of about 1 -foot below the surrounding grade. Based on our observations of the proof roll on October 12, 2007, the backfill was placed in general accordance with the referenced requirements. The purpose of my visit was to observe the excavation resulting from the demolition of the previous residence. I met with Forrest and Kelly (excavator) on site. CTI has not performed a geotechnical investigation of this site and is not aware of a geotechnical report by others for this particular project, Accordingly, CTI is not the geotechnical Engineer -of- Record and has no knowledge of subsurface soil conditions of this site. Since our client or the jurisdiction did not provide specific special inspection requirements for the demolition project, our observation and testing services for this project are provided as called for in City of Portland Title 24 requirements for Soils Special Inspection. While on site, I observed the excavation resulting from the demolition of the previous residence. The excavation measured approximately 40 feet (north - south) by 25 feet (east -west) and was about 5 to 5'/2 feet deep. The northeast corner of the excavation was located approximately 60 feet south and 80 feet west of the northeast property corner. The excavation was free of demolition debris, and the exposed subgrade within the excavation consisted of sandy silt. I left the site at about 11:20 a.m. Kyle Smetana M.
