STRUCTURAL STEEL WELDING INSPECTION REPORT
On the above dates, our representative arrived at Pacific Steel Fabricators, Middleton, ID to perform work on the referenced project as
reported below. Work was in progress and performed by two welders and one welder helper for clean up, pick up and staging
procedures. Weldments consisted of complete joint penetration, single /multi -pass fillet welds on continuity plates and connection
plates for wide flange moment frames.
The in- progress work had material identification checked. All phases of work was monitored, inspected and documented with
approval per AWS D1.1 and contract specifications and approved structural drawings.
Note: Work was inspected without plans marked "approved ". Upon arrival, MTI representative was informed that the 12 frames had
been shipped to Portland earlier that morning. Based on discussions with MTI representatives Ski Harris and Dan Colby, the visual
and non - destructive inspections were completed on frames 1 through 11. Frame 12 will be shipped back to Pacific Metal Fabricators
for visual and non - destructive inspections.
Work inspected was in compliance with project plans, shop drawings and specifications, except as noted.
