Based on your letter and the subsequent telephone conversations, the following documents are enclosed: The signed Voluntary Compliance Agreement is enclosed. Please note that the mechanical permit MEC2004 -00725 listed as "requiring final approval" has been signed off as final by Dan Nelson. A copy of the City inspection document is enclosed. CUP Item Number Status 40. Tree Care Recommendations The report of final recommendations from Halstead Arboriculture Consultants was revised on 11/10/05 and is enclosed. All work on the school site has been completed. The report also references trees no longer on TTSD property, and/or not on the school site. These include a tree on private property west of 132 Terrace, trees on Ms. Rider's life estate, and at least one tree in the fenced off undeveloped NE corner (Halstead recommends a "no trespassing sign "). The requirements for trees on the school site are done. I recommend that Mat Stein and Dave Halstead jointly review the other trees that may affect City /County streets and the other referenced properties. The District will certainly do our part to comply with their recommendations within the stipulated time period. 41. Section 2, County Land Use Complete. Ed Murphy requested written response from Phil Healy at the County on 10/12/05. I have not received any further update on this item. 45. Final Grading Work Report Complete. Permit BUP2004 -00529 must be completed and approved, including all outstanding corrections, conditions, ancillary permits and fees. This report summarizes our construction observation and testing services during the construction of Alberta Rider Elementary School for the Tigard- Tualatin School District (TTSD), located in Tigard, Oregon. The site is located as shown on the site vicinity map, Figure 1. This work was performed as a part of our contract (dated May 18, 2004) with TTSD for construction observation and testing services for Alberta Rider School. SCOPE OF SERVICES The Alberta Rider Elementary School Project consists of the construction of one main school building, an adjacent parking area and an outside covered play area, all of which were covered by our services. The general layout of the site is shown on Figure 2. Construction observation and testing services provided by URS during the duration of the project focused on earthwork, including foundation excavations, parking lot aggregate and base density testing, floor slab subgrades, and backfill observation. This report certifies the conformance of backfilling and compacting to project specifications at the site. Between November 16, 2004 and March 17, 2005, URS made 61 visits to the site to perform construction monitoring of the aforementioned activities on an on -call basis. The purpose of this monitoring was to confirm that site conditions were as anticipated from the geotechnical borehole logs and to document the activities of the contractor in order to assess compliance with the project specifications, URS representatives were on site on an on -call basis during most of the construction period. Each representative filled out a daily field report for each day on site summarizing the activities monitored and inspections performed. The daily reports are numbered from 1 to 61, representing each day on site by URS. Copies of all daily field reports are included in Appendix A. JRS Ccrperation 113. SW Columola. Structures at this site were supported on spread footings with slabs on grade. URS observation and testing tasks included the following activities: 6 Monitoring of slab subgrade proof rolling O Monitoring of footing excavation and approval of footing and slab subgrades ® Directing local overexcavation where soft subgrade soils existed ® Performing proctor tests to determine maximum dry density and optimum moisture contents of backfill materials ® Compaction testing of backfill below footings and slab subgrades 8 A total of 121 nuclear density tests were performed during the duration of the project, the locations of which can be found on sketches in the daily reports, Appendix A. When materials did not meet the specified compaction requirements, the contractor was directed to rework or recompact failing areas until compaction requirements were achieved. A summary of all compaction test results can be found in Appendix B and telephone notes are included with the daily field reports in Appendix A. I DISCUSSION The construction activities observed were performed in general accordance with the standard of care of construction practice. No unanticipated complications were encountered during the observed construction activities, and general soil strength and stratification were as anticipated in the URS geotechnical report dated May 18, 2004. CONCLUSIONS It is URS' opinion that the Alberta Rider Elementary School foundation and adjacent play areas were constructed in a maimer consistent with the project specifications, and they are therefore approved. The target area is from zero to 75 feet east, west and north. 2. There are several trees along the east and southeast side of the wooded area that are hazardous. One of these trees is Tree Number 374 (Cherry tree) Located on the west side of the gravel entrance off Bull Mountain Road. This has been in the TPZ reports since January 15, 2005.
