1704.1.2 of the State Building Code representatives from our firm have performed special inspection during
construction for the following project:

Our project files indicate that PSI special inspections activities listed below were conducted as scheduled from
December 30. 2010 through March I I, 2011. This letter represents a summary of work observed and reported by
PSI representatives and the resolution of any documented noncompliance.

Remarks:
PSI was on site as requested to perform special inspection of installation of epoxied dowels for cmu infill wall. Per details, #5 dowels were
to be installed at 16" on center vertically with 6" embedment, and two #5 bond beam horizontal dowels were to be epoxied at 4' on each
side of wall with 8" embedment. Additional dowels were to be epoxied horizontally above bond beam for joints, but due to existing doorway
frame, these were not needed. 3 /4" diameter holes were drilled to 8" on bond beams, and 6" on verticals. (3/6) verticals encountered rebar
during drilling and could only achieve 5 " -5.5" embedment. Holes were drilled into existing concrete of unknown PSI at 49F, and were blown
and brushed until clean and dry. Hilti RE 500 SD epoxy (expires 02/2011) was used. No issues encountered. See attached report for
additional info on cmu wall.
