SW 72 Avenue indicates fill has been placed over this site. We are unable to
0 / determine if the fill was placed in accordance with Appendix Chapter 3.
A. FiII placed on the property which is to support the foundation of a building
shall be placed in accordance with accepted engineering practice. Submit
a soils investigation report and a report from an approved testing agency
C of satisfactory placement of fill [OSSC, Section 3301.1].
Complete the enclosed Soils Special Inspection form designating an
D Approved Testing Laboratory [Line B] and signed by the owner of the
project [Line D].
i. The completed form must be returned to this office before a Site
permit can be issued.
ii. Copies of all special inspection reports shall be filed with this
office continually during construction [OSSC, Section 3305].
iii. A final signed report must be on file before an occupancy
rn certificate will be issued [OSSC, Section 3318].
V . Catch basins shall be trapped by using the inverted one - quarter bend or welded
baffle. Provide catch basins constructed to OPSC, Section 1108 specifications.
Submit revised plans with details and specifications for the Lynch -type catch
basin. The design shall be in accordance with OPSC, Section 1108
/ _ requirements.
. The pipe size from the new catch basin is not adequate to handle volume /area.
Provide not less than a 6 inch pipe at a 1 percent slope.

. The pipe size from the junction of the catch basin lateral is not adequate to
handle the volume of the parking lot and roof drainage. • Provide not Tess than an
8 inch pipe at a 1 percent slope.
6. rm drain piping size and slope continuing down stream of the junction of all
new roof drains shall be no less than an 8 inch diameter with a 1 percent slope.
Plans show an existing 6 inch storm drain receiving flow from this project. There
is some confusion as the sanitary sewer line is shown as 8•inches along the line.

7) DESCRIBE FIRE AREA :*
* If there is more than one fire area in the structure, include (attach) a diagram on 8 -1/2 X 11 or 11 X 17 paper
indicating areas.
CALCULATING REQUIRED FIRE FLOW
Complete line A if the building has only a single occupancy hazard. Skip to line B if the building has
multiple occupancy hazards. Round off calculations to nearest whole number. NOTE: THE
REQUIRED FIRE FLOW IN LINE A, B OR C MUST BE AT LEAST 1,500 GPM AND CANNOT
EXCEED LINE D OR 3,000 GPM. CORRECTIONS WILL HAVE TO BE MADE IF THE
CALCULATED FLOW EXCEEDS LINE D OR 3,000 GPM (See Fire Flow Calculation Instruction
Section 5 for explanation)

B) MULTIPLE OCCUPANCY HAZARD
Use the following when a single fire flow area contains multiple occupancy hazards. Each occupancy
will be broken down as a percent of the total fire floor area they occupy. The fire flow for each
occupancy will be calculated by multiplying the occupancy factor, fire flow and the percent of the area
that the occupancy occupies. Add the fire flow of all occupancies together to obtain the required fire
flow for the building.
