Nicoll Engineering is providing this report to describe the resolution of the noncompliance items. ITEM NUMBER 1 (Resolved) • Summary: No inspection performed for the embeds at roofline of office (Ref. Detail 1/5.2). Embeds installed. Action: Mr. Duncan, the building owner and contractor has provided a letter with Mill Certification (enclosed) verifying the rebar . grade of the No. 5 bar used for these embeds. The contractor exposed several areas where the welds between the rebar and embed plate could be inspected. Carlson Testing visually inspected these welds and found them to be 3/16" fillet vs. the 1/4" fillet welds required by the approved documents. Upon further review of our calculations, a 3/16" fillet weld will develop the required strength of the No. 5 bar. Therefore the existing condition is adequate for the design requirements. Action: Carlson Testing, Inc. located the rebar with a metal detecting device. Nicoll Engineering accepts the results and is satisfied that the rebar was placed according to approved documents. ITEM NUMBER 4 (Resolved) Summary; No reports for rebar inspection. Action: Same as item 3. ITEM NUMBER 5 (Resolved) Summary: Contractor could not get vibrator into first grout lift. Searnster 09- 16 -99: CTI representative was onsite for periodic inspection of structural steel and welding. The following was noted., 1. Performed visual inspection of (8) tube steel columns_ Welds inspected include base plate to tube steel column connection and tube steel to column buckets. All welding appears to conform to details and AWS D1,1. 2. Performed visual inspection of ledger clips at roof level of the north and south walls in warehouse area. All welding appears to conform to details and AWS D1.1. 3. Reviewed mill certificates for rebar used for embeds, item #1 on CTI non compliance list. Contractor is in compliance and item is signed off. ` 4. Need engineer approval for placement of roof line buckets with rebar embeds into CMU wall of office area. This inspector checked rebar in 8" CMU wail to top in the following locations: 8/2.4 -3; 3 /A -B and A/2.7 -3. Approximately (5) cubic yards of Lone Star 6 1 /2 sack 3/8" aggregate grout mix #0884 was pumped into wall and mechanically vibrated. No samples were made. Embeds set were grade 60 not A706 per 1/S5.2. Contractor will set them in this pour. Contractor said he had paperwork for these embeds, but could not find them. See non- compliance list. Watson, COP #643 / WABO (WAT 23 3239) 08- 06 -99: CTI representative (S. Fox) was present onsite as requested to inspect placement of reinforcing steel in the warehouse interior CMU wall at height of 0' to 26'. This wall was laid up without any inspection beyond height of 4'0 ". This inspector was able to, by use of flashlight from top, and clean -outs at bottom, see down the vertically reinforced cells. The walls had bond beams at approximately 4' on center with (1) #5 bar in each bond beam, and three bond beams at bolt lines. Vertical bars consist of (1) #5 bar at 32" on center with (2) #5 bars at sides and above openings. Reinforcement is in accordance with project drawings. 08- 06 -99: CTI representative (G. Watson) was present onsite for a periodic masonry inspection. This inspector inspected 8" CMU wall grout placement at line 2, 0' to 27'4 ". Contractor provided clean -outs at every vertical bar. Beam seats at top of wall as noted in prior inspections have the wrong type of rebar welded to seat. Plans call for A706. Grade 420 was used, and owner and engineer were notified. Contractor did not consolidate grout per UBC. (18) cubic yards of a Lone Star 6 %2 sack Kurtz mix grout was placed in walls. Our reports pertain to the material tested /inspected only. They conform to the permit documents, We are unable to verify welds of rebar to buckets, which have been embedded at top of wall. We understand that you had visually inspected the rebar to buckets welds and will take full responsibility for the size and placement of the welds. The buckets appear to conform to detail 1 / 5.2. The welds for the buckets were visually inspected and the weld position and size appear to be per the directions of this office. CTI Permit No. BUP99 -00048 FIELD INSPECTION REPORT DATES COVERED: August 25, 1999 PROJECT: Duncan Building ADDRESS: 16135 SW 74th- Tigard, OR _INSPEC `TOR- J_Hietpas -COP#343_/ VNABO (HIP-5 -- 08-25-99: CTI representative was onsite for a periodic masonry inspection. This inspector observed contractor place grout at Tine B/1 -2 and line 1/A.4 -B. (18) cubic yards of a 6 % sack grout mix #0884 was placed at the above mentioned locations. Contractor grouted full height of walls this date. This inspector could only see the top 4' of wall for rebar conformance. Contractor placed grout in greater than 6' lifts. Contractor could not get vibrator into lower sections of the wall. See attached non - compliance list. Non - compliance items were brought to the attention of Gordon with NW Masonry and added to the non- compliance list as items 1 -5 (see attached).
