Stoner, COP #611, WABO #ST0707291 11- 22 -99: CTI representative was onsite and the following was noted: Summary of non compliance list resolution process to be read in conjunction with correspondence from the engineer of record Nicholi Engineering and Construction Services, Inc. CTI representatives, S. Stoner and A. Ewing had discussed over the phone and in person, the procedure to resolve non compliance items on report dated 09 -17 -99 by CTI representative G. Seamster, and accompanying non compliance list dated 09- 20 -99. It was agreed that the following method would be used. 1. Scan CMU walls with magnetic rebar locator at random locations to determine if the walls are reinforced at the required spacing. 2. Tap on the walls, and if necessary use drill to judge adequacy. • 3. Review all evidence supplied by the contractor to verify as much as possible that the office roof line welded embeds were as specified. On 11 -03 -99 CTI representatives began the process described above and the following was noted: 1. Welded embeds had not been inspected. Owner /contractor supplied mill certificates to the structural engineer stating that A -706 rebar had been used. There is no way to verify this. 2. Magnetic rebar locator scans at various locations indicated that walls were reinforced at specified spacing with added bars at required locations i.e. jambs and headers of openings and at mezzanine floor line. 3. Walls were grouted both partially (at reinforced cells) and solidly: A. Wall section at A/1 -2 grouted solid above the 4' bond beam. Partial below. B. Wall section at A -B /2 grouted solid above mezzanine floor bond beam. Partial below. C. Wall section at A/2 -3 grouted solid full height. D. Wall section at A -B /3 grouted partially to top of openings. E. Wall section at B/2 -3 grouted partially to top of openings, solid above top of openings. F. Wail section at A -B/1 grouted partially to first 4' bond beam, solidly grouted above the full height. G. Wall section at B/1 -2 partially grouted to 8' bond beam, solidly grouted full height above. Owner /contractor had chipped out CMU at two office roof line embeds for visual inspection of welds. See report dated 11 -12 -99 by CTI representative CM Yuckert. Structural engineer had reviewed CTI's findings and issued a letter dated 11- 12 -99. Engineer has accepted as built condition and items 1 -7 on the CTI non compliance list have been resolved. Nicoli Engineering is providing this report to describe the resolution of the noncompliance items. ITEM NUMBER 1 (Resolved) Summary: No inspection performed for the embeds at roofline of office (Ref. Detail 1/5.2). Embeds installed. Action: Mr. Duncan, the building owner and contractor has provided a letter with Mill Certification (enclosed) verifying the rebar tirade of the No 5 bar used for these embeds. The contractor exposed several areas where the welds between the rebar and embed plate could be inspected. Carlson Testing visually inspected these welds and found them to be 3/16" fillet vs. the 1 /4" fillet welds required by the approved documents. Upon further review of our calculations, a 3/16" fillet weld will develop the required strength of the No. 5 bar. Therefore the existing condition is . adequate for the design requirements. • ITEM NUMBER 2 (Resolved) Summary: Walls not consolidated Action: Carlson Testing, Inc. performed a harmonic impact test on the structures walls and found that is appears the grout was consolidated in the masonry walls. Nicoll Engineering is satisfied with the test performed and the results. ITEM NUMBER 3 (Resolved) Summary: Walls not inspected prior to grout. Action: Carlson Testing, Inc. located the rebar with a metal detecting device. Nicoli Engineering accepts the results and is satisfied that the rebar was placed according to approved documents. Welds inspected include base plate to tube steel column connection and tube steel to column buckets. All welding appears to conform to details and AWS D1.1. 2. Performed visual inspection of ledger clips at roof level of the north and south walls in warehouse area. All welding appears to conform to details and AWS D1.1. 3. Reviewed mill certificates for rebar used for embeds, item #1 on CTI non compliance list. Contractor is in compliance and item is signed off. 4. Need engineer approval for placement of roof line buckets with rebar embeds into CMU wall of office area. Rebar to bucket welds have not been signed off. Also need weld detail for buckets fabricated at Emmert Machining. All non compliance items were brought to the attention of: John Duncan, owner.
