Gentlemen:
The following items were nonconformance issues developed by Carlson Testing, Inc. and noted
on the enclosed incomplete items list, dated September 20, 1999. Nicoli Engineering is
providing this report to describe the resolution of the noncompliance items.
ITEM NUMBER 1 (Resolved)
Summary: No inspection performed for the embeds at roofline of office (Ref.
Detail 1/5.2). Embeds installed.
Action: Mr. Duncan, the building owner and contractor has provided
a letter with Mill Certification (enclosed) verifying the rebar

The contractor exposed several areas where the welds
between the rebar and embed plate could be inspected.
Carlson Testing visually inspected these welds and found
them to be 3/16" fillet vs. the 1 /4" fillet welds required by the
approved documents. Upon further review of our
calculations, a 3/16" fillet weld will develop the required
strength of the No. 5 bar. Therefore the existing condition is
adequate for the design requirements.
ITEM NUMBER 2 (Resolved)
Summary: Walls not consolidated
Action: Carlson Testing, Inc. performed a harmonic impact test on
the structures walls and found that is appears the grout was
consolidated in the masonry walls. Nicoli Engineering is
satisfied with the test performed and the results.
ITEM NUMBER 3 (Resolved)
Summary: Walls not inspected prior to grout.
Action: Carlson Testing, Inc. located the rebar with a metal detecting
device. Nicoli Engineering accepts the results and is
satisfied that the rebar was placed according to approved
documents.
ITEM NUMBER 4 (Resolved)
Summary: No reports for rebar inspection.
Action: Same as item 3.
ITEM NUMBER 5 (Resoiveci)
Summary: Contractor could not get vibrator into first grout lift.
Action: See item 2.
At this time the nonconformance items 1 -5 have been resolved to the satisfaction of Nicoli
Engineering.

FIELD INSPECTION REPORT

09- 16 -99: CTI representative was onsite for periodic inspection of structural steel and welding. The following
was noted.
1. Performed visual inspection of (8) tube steel columns. Welds inspected include base plate to tube steel
column connection and tube steel to column buckets. All welding appears to conform to details and AWS
D1.1.
2. Performed visual inspection of ledger clips at roof level of the north and south walls in warehouse area. All
welding appears to conform to details and AWS D1.1.
3. Reviewed mill certificates for rebar used for embeds, item #1 on CTI non compliance list. Contractor is in
compliance and item is signed off.
4. Need engineer approval for placement of roof line buckets with rebar embeds into CMU wall of office area.
Rebar to bucket welds have not been signed off. Also need weld detail for buckets fabricated at Emmert
Machining.
All non compliance items were brought to the attention of: John Duncan, owner.
