This is to certify that VLMK Engineers has reviewed the installation of the two water
quality ponds located at the above mentioned project and has found them to be in
compliance with the design indicated on the permit drawings dated June 30, 1998.
There are two water quality ponds located on the subject property. One is located on the
north property line, the other on the south. The project was completed approximately
• four months ago. Both water quality facilities have been in operation since that time. At
the present time, the vegetation is fully established and the ponds appear to be
functioning as the design intended.

This is to notify you that the Creekview project is substantially complete. During the
course of the construction for the project, our office has provided periodic site
observations and special inspection of the lateral load systems. Copies of our field
reports pertaining to those inspections have been forwarded to your office. We have also
reviewed inspection and testing reports from the geotechnical engineer and from the
testing agency.
To the best of our knowledge, the buildings have been constructed in accordance with the
permit drawings and with the requirements of the Uniform Building Code.
If you need any additional information from our office to process an occupancy permit

1 W ater Quality Facility observation by Engineer of record.
The owner shall employ the Engineer of record, responsible for the design and
specifications of the Water Quality Facility, to perform construction and visual observation
of the Water Quality Facility for compliance of the design and specifications, at significant
stages, and at completion of the construction.
Prior to final occupancy approval of the construction under permit for the site, the
Engineer of record shall provide the City of Tigard, "Attention Plans Examiner" and
"Supervising Inspector' with written confirmation that the Water Quality Facility is in
compliance with the design and specifications of same.
2. Pursuant to TVFR amendments to UFC, Section 903.4.2.1, the site requires 5 hydrants,
provide details.
3. On the loading dock side of Building "B" at Grade 140.3 B.O.C. down to the grade break
at 74th, provide an additional C.B. at grade 139.65. Slots in curbs for offsite drainage are
not allowed.
4. The storm drainage plan shall adequately address the number of catch basins required to
handle the parking lot and hard surface runoff. The Uniform Plumbing Specialty Code
[UPSC, Section 1108] allows a maximum 6" outlet on each catch basin, and Table 11 -2
limits the maximum surface area one catch basin can serve to 7,133 square feet at 1%

slope of the horizontal line from the catch basin. Additional catch basins are required to
adequately serve the area.
A. Sizing of all storm drain piping is determined by OPSC, Table 11 -2. If an
engineered system is to be used in lieu of a table 11 -2, two sets of plans
stamped by an engineer licensed in Oregon and the hydrodynamic
calculations must be submitted for review and approval.
5. Cleanouts are required on all drainage piping every 100 feet and at each aggregate
change of direction exceeding 135 degrees [OPSC, Section 707.4 and 707.5].
6. Catch basins shall be trapped by using the inverted one - quarter bend or welded baffle.
Provide catch basins constructed to OPSC, Section 1108 specifications.
7. Submit revised plans with details and specifications for the Lynch -type catch basin. The
design shall be in accordance with OPSC, Section 1108 requirements.
Complete the enclosed Soils Special Inspection form designating an Approved Testing Laboratory
[Line B] and signed by the owner of the project [Line D].
A. The completed form must be retumed to this office before a Site permit can be
issued.
B. Copies of all special inspection reports shall be filed with this office continually
during construction [OSSC, Section 3305].
C. A final signed report must be on file before an occupancy certificate will
be issued [OSSC, Section 3318].
Based on the concems issued on Page 10 of Geo- Designs analysis "Deep Foundations° we will
require structural observation pursuant to OSSC, 1702(4). Please advise the owner of this
request.
