08 -23-00: CT1 representative was on site, at the request of Scott with Howard S. Wright
Construction, and observed cleanout and epoxy procedures for placement of 1 /2" all-thread rods
with 4W •embedment at new tube steel door frame. Unable to achieve required depth on (6)
locations. Depths achieved were: (2) at 4W, (2) at 4 ", (1) at 31/2", and (1) at 3 °. These depths
written on columns at locations. Hilts Hy -150 cleaning guidelines were followed.
• Note:. As per Keith with Eastern Steel, directed to observe and report without building permit or .
City - approved plans. Required permit and plans detail to be sent to CTl by Scott with Howard
S. Wright Construction.
