& Table 104.3. Performed continuous visual inspection of (16) 3/4" threaded anchors installed in
7/8" holes, with 4" embedment. Contractor furnished an RFI response from engineer
approving the use of Redhead *Epcon A7 in place of Hilti Hy150 (attached).
Holes were brushed & blown clean prior to installation. Anchors were installed in conformance

REMARKS:
On this date a PSI representative was at the above referenced site to perform special inspections as requested or
required in accordance with job specifications, IBC, and applicable codes.
PSI representative was on site as requested at Nike Golf to provide special inspection of structural steel related work
on the two ton crane installation. The following is noted:
I. Witnessed the installation of the ' x 2 '/2 A325 bolts. All bolts were tensioned by the turn of the nut
method and conform with the ICC section 1704 and detail requirements.
2. Witnessed the installation of four 5/8" all thread epoxy anchors at tie in to wall south, west, and east
Kenway columns. Minimum embedment of 7 ''/2" achieved, Red Head Epcon A7 epoxy used. Installation
conforms with ICBO ER5560 and detail requirements.
To the best of my knowledge, the work was in accordance with the building department approved design drawings,
specifications, and applicable workmanship provisions of the IBC.
