Reviewed reinforcing tendons and shoring for 3 P.T. slab four (2 tier, NW portion). In general,
layout appears to be in good order. The following items were noted and discussed with Jim and
Marvin for correction:
1. Wedge covers along the intermediate stress joint (grid 6.25) need to be correctly secured.
2. The tendon dead ends at G -9 are not chaired properly and need to be corrected.
3. Provide U -bars at the swept tendons located adjacent to the drain.
4. Provide (2) #4 bars at tendon anchoring — note particularly at grid C.2 -9. Where typical
running bars are more than 1W from anchorage, provide extra #4 x regd.
5. Noticed several tom tendon casings. Make sure these areas are clean and properly
greased. Wrap with tape.
6. The bottom reinforcing in front of the elevator is generally too high. Please adjust to
provide 1" to 1'/2" max. clear. Note that several bars will be higher due to conflict with
spandrel embeds.
7. Provide compressible foam or 3/8" gap at west side of elevator door header to allow slab
movement. See sketches provided to Baugh.
8. The #4 chair bar doubles as typical mat reinforcing at some locations. However, along B -5,
these bars do not always have correct lap. Provide #4x4' -0" bars at these locations to
create proper bar lap splice.

9. Richmond anchors at the edge of the crane opening are to be straight (see detail 171S -5.2).
Hooked anchors were installed and need to be corrected.
