A visit was made to observe concrete placement in progress. At this time, the east 40% +/- of the
first pour had been completed.
Claus (Carlson Testing) indicated that the concrete slump was running at about 6 ", though a 5"
slump was being requested from the plant to better facilitate placement on the sloping ramp.
Carlson Testing was monitoring batch tickets, keeping track of mix revolutions, and noting any
added water (within limits of water not added at plant).
Observed workmen cleaning off screeds with a hose above the concrete pour, with water running
down into the wet concrete. Kris was asked to have this practice stopped immediately and ensure
that it did not occur again.

Placement of P.T. slab reinforcing and tendons were reviewed for the first tier slab (Pours 1 and 2).
Overall, the layout was in good order and final adjustment were in progress to prepare for the
pours on the 29 and 30 The•following observations and /or recommendations were noted and
discussed at the site for prompt resolution:
The following was noted:
1. Typical mat steel needs additional chairs and ties. A number of bars are sagging and tie
wires are missing or do not hold when walked upon.
2. Need bearing pad across elevator head. Also place 4" piece of insulation on ties. Knock
down high spots in concrete before placing neoprene bearing pad.
3. Verify that dowels are loose along all walls between grids 7 and 9. (Reference details 2, 3,
11, 12, and 13/S5.2) If bars are not loose and cannot be freed with a hammer, provide 7" +
piece of pipe insulation on bar.
4. Review tendon anchorage heads for overlap. Heads cannot overlap.
5. Provide U -bars on swept tendons adjacent to the drain at D3. Also provide trim bars on
each of 4 sides around drain. •

6. Uniform tendons need to be tied tight to the underside of the banded tendons across the
columns. Note particularly on pour 2 (south).
_ i . inter : joint at gild 6.2 needs expanded metal foi stop. See 5/S5.1. Okay to
place in segments between tendon anchorages rather than running continuous.
8. Clean all loose debris from forms prior to pour.
9. As discussed, curing compound will be placed on the slab after pour. The slab will be
sprinkled intermittently on Friday and over the weekend. Baugh will monitor the weather
and provide as needed.
