This is to certify that in accordance with Section 1701 of the Uniform Building Code, Title 24, we have
performed special inspection of the following item(s) per our inspection reports only:
Installation of Adhesive Anchors
All inspections and tests were performed and reported according to the requirements of Project Documents
and, to the best of our knowledge, the work was in conformance with the approved plans and
specifications, approved change orders and applicable workmanship provisions of the State Building Code
and Standards, as well as the structural engineer's design changes, approvals and verbal instructions.
Our reports pertain to the material tested /inspected only. Information contained herein is not to be
reproduced, except in full, without prior authorization from this office.

The plan review was performed under the State of Oregon Structural Specialty Code (OSSC)
1998 edition; and the Tualatin Valley Fire & Rescue Ordinance 99 -01 (TVFR99 -01) 1999
edition. The submitted plans are approved subject to the following conditions.
1. NORTH, SOUTH & WEST walls shall be 1 hour rated due to location on property.
2. Openings are not permitted in the North & South walls. The structure will have
openings framed in but will be in- filled until the property line has been adjusted to
permit openings.
3. Parapet walls are required on the North & South sides of the structure.
4. The "Covered Entry" will require 2 layers of gypsum applied to the underside of the
roof joist to comply with section 709.4.1 Exception 5.2 OSSC See attached item 21-
1.1, from Table 7 -C. No vents within 5 feet of property line.
5. The means of egress shall be provided with illumination of not less than 1 foot candle
at the floor level anytime the building is occupied. The means of egress illumination
shall be provided with a second source of power in case of primary power loss.

6. Provide one 2 -A rated fire extinguisher in the new area. The extinguisher shall be
located so that no usable space has a distance greater than 75 feet to the extinguisher.
Standard 10 -1, Chapter 3 TVFR99 -01.
Deferred submittals such as Mechanical, Truss Joist Shop Drawings, etc, will be charged a
} deferred submittal fee based on the valuation of the portion of the work being deferred. The
minimum fee shall be $200.00.

American with Disabilities Act (ADA): It shall be the responsibility of the Architect,
Engineer, Designer, Contractor, Owner and Lessee to research the applicability of the ADA
requirements for the structure. The City of Tigard reviews the plans and inspects the structure
only for compliance with Chapter 11 of the OSSC which may not include all of the
requirements of the ADA.
Approved Plans: 1 set of approved plans, bearing the City of Tigard approval stamp, shall
be maintained on the jobsite. The plans shall be available to the Building Division inspectors
throughout all phases of construction. 106.4.2 OSSC
Certificate of Occupancy: No building or structure shall be used or occupied until the
Building Official has issued a certificate of occupancy 109.1 OSSC
Premises Identification: Approved numbers or addresses shall be provided for all new
buildings in such a position as to be plainly visible and legible from the street or road
fronting the property.
When submitting revised drawings or additional information, please attach a copy of the
enclosed City of Tigard, Letter of Transmittal. The letter of transmittal assists the City of
Tigard in tracking and processing the documents.
