The submitted plans are approved subject to the following conditions. SITE (received 6/5/06) 1. Our files show no site permit was issued. Please submit site plan with permit application along with valuation of site work. Complete form to show all impervious area, excavation, grading and fill volumes. Please complete both sides of the form. Forms attached. 2. Provide attached TVF&R form with permit STRUCTURAL 3. The structural review was completed by Miller Consulting.
