The plan review was performed under the State of Oregon Structural Specialty Code (OSSC) 2004 edition
and 2004 Oregon Fire Code as amended by TVF&R.,.The submitted plans are approved subject to the
following conditions.
1. The structural review was reviewed and approved by Miller Consulting Engineers, August 16, 2006.
(review doc. attached).
2. Special inspection is required for comah ete,, we n' forcing stee4 bolting, welded studs,
embed plater bolts in concrete, masonry, soils, structural steel fabrication and erection,
adhesive and expansion anchor installation, (as items listed on sheet A0.1 and Miller Consulting
Engineers approved review letter)1be special inspection agency of record shall furnish inspection
reports to the Engineer of,Record,- Afghan Associates, the General Contractor, and the City of Tigard,
Building Division, attention Hap Watkins. All discrepancies shall be brought to the immediate
attention of the general contractor for correction. The special inspector shall submit a final signed
report stating whetlierthe work requiring special inspection was, to the best of the inspector's
knowledge; in conformance with the approved plans and specifications and the applicable workmanship

3. Structural Observations is required per section 1709 Oregon Structural Specialty Code (OSSC).
The ownershall employ a registered design professional to perform structural observations as defined
in Section 1702, OSSC. Structural observations is required for concrete foundation, retaining
walls, reinforcing steel in shear walls and P/T slab, before concrete placement at second floor,
placement of floor joist and before stressing P/T slab (as items listed on Miller Consulting
Engineers approved review letter). Deficiencies shall be reported in writing to the owner and to the City
of Tigard, Building Division, attention Hap Watkins. FYI The structural observations do not waive
the framing inspection required by code. OSSC 1709

4. Live Loads Posted: The live loads for which each floor has been designed to exceed 50 psf shall be
conspicuously posted by the owner in that part of each story in which they apply, using durable metal
signs, and it shall be unlawful to remove or deface such notices. The occupant of the building shall be
responsible for keeping the actual load below the allowable limits. 1603.3 OSSC

5. Access to exits shall be marked by readily visible exit signs in cases where the exit or the path of egress
travel is not immediately visible to the occupants. Sec. 1011.1 OSSC
Exit signs and the means of egress shall be illuminated at all times the building isloecupied,and s hall be
provided with a second source of power that will provide 1 1/2 hours of illumination in of primary
power loss. Sec 1006.2 & 3 OSSC FYI Additional exit signs may be requiredsby xhe city field inspector

3. Partial or complete closing of required opening in exterio walls by tarpaulins or any other
means. ;
4. Dispensing of fuel.
8. Fire extinguishers shall be selected, installed,and maintained in accordance with OFC 906 and NFPA 10.
Extinguishers shall be located in conspicuous loons where they will be readily accessible and
immediately available for use. These locations shall be along normal paths of travel, unless the fire code
official determines that the hazard posed i dicates the need for placement away from normal paths of
travel. Sec. 906 OFC
9. Fire extinguishers shall not be obstructed or obscured from view. In rooms or areas in which visual
obstruction cannot be completely avoided, means shall be provided to indicate the locations of .
extinguishers.

10. Deferred submittals minimum fee shall be $200.00. This fee is in addition to the project plan review
fee based on the total;project value. (OAR 918 -050 -0150) Documents for deferred submittal items
shall be subinitted to the registered design professional in responsible charge who shall review them
and fowaid them�to the building official with a notation indicating that the deferred submittal
documents have been reviewed and been found to be in general conformance to the design of the
building: The deferred submittal items shall not be installed until the design and submittal documents
have been approved by the building official. Oregon Structural Specialty Code Section 106.3.4.2"

Approved Plans: 1 set of approved plans, bearing the City of Tigard approval stamp, shall be maintained
on the jobsite. The plans shall be available to the Building Division inspectors throughout all phases of

Certificate of Occupancy: No building or structure shall be used or occupied until the Building Official
has issued a certificate of occupancy 109.1 OSSC
Premises Identification: Approved numbers or addresses shall be provided for all new buildings in such
a position as to be plainly visible and legible from the street or road fronting the property.
