information provided by the construction contractor, field survey of
manholes, mainlines and service lateral location markers for both sanitary
and storm utilities, construction staking field notes (when applicable) and
field changes noted during construction inspections. The location and
elevation of underground utilities shown herein are approximate only and
must be field verified prior to any connection thereto or excavation in the
immediate vicinity thereof.

101
1. ALL PUBLIC STREETS SHALL BE CONSTRUCTED AS PER THE APPLICABLE SECTIONS OF THE CITY OF
TIGARD PUBLIC IMPROVEMENT DESIGN STANDARDS AND OREGON STANDARD SPECIFICATIONS FOR
CONSTRUCTION, 2008.
2. THE CONTRACTOR SHALL DO ALL THE WORK SHOWN ON THE DRAWINGS AND ALL INCIDENTAL WORK
CONSIDERED NECESSARY TO COMPLETE THE PROJECT IN AN ACCEPTABLE MANNER.
3. THE CONTRACTOR AND/OR SUBCONTRACTOR SHALL HAVE A MINIMUM OF ONE (1) SET OF APPROVED
CONSTRUCTION PLANS ON THE JOB SITE AT ALL TIMES DURING THE CONSTRUCTION PHASES.
4. ALL MATERIAL SUPPLIER(S) SHALL SUBMIT TO THE ENGINEER PROOF OF MATERIAL(S) TESTED IN
ACCORDANCE WITH THE ABOVE SPECIFICATIONS AND SHALL CERTIFY THAT ALL MATERIAL DELIVERED
TO THE JOB SITE MEET OR EXCEED WASHINGTON COUNTY SPECIFICATIONS PRIOR TO DELIVERY OF
MATERIAL TO THE SITE. ANY MATERIAL NOT CERTIFIED BY THE SUPPLIER SHALL BE REMOVED FROM
THE JOB SITE AT NO ADDITIONAL COST TO THE OWNER.
5. A PRE CONSTRUCTION MEETING MUST BE HELD PRIOR TO COMMENCING WORK, WITH YOUR CWS
INSPECTOR. (INSPECTION HOT LINE # (503) 681-4444) & CITY OF TIGARD ENGINEERING &
CONSTRUCTION SERVICES INSPECTOR.
6. ABANDON WELLS AND DRAIN FIELDS ONSITE MUST COMPLY WITH OREGON DEPT. OF WATER
RESOURCES' REQUIREMENTS.
7. CONSTRUCTION RELATED TO CWS PERMITTING TO BE IN ACCORDANCE WITH R&O 19-22 (CWS).
8. NO STORAGE OF EQUIPMENT OR MATERIALS IN RIGHT-OF -WAY, MUST FOLLOW CONSTRUCTION
PARKING PLAN PER CONDITIONS OF APPROVAL.
9. TRENCHES IN ROADWAY LEFT OVERNIGHT REQUIRE A 2" TEMPORARY HOT MIX SURFACE OR LEVEL STEEL
PLATE.
GENERAL
1. ALL TREES, BRUSH AND DEBRIS WITHIN LIMITS OF THE RIGHT-OF-WAY AND ON THE LOTS TO BE
FILLED SHALL BE REMOVED AND DISPOSED OF BY THE CONTRACTOR.
2. ALL AREAS OF CONSTRUCTION SHALL BE STRIPPED. STRIPPING SHALL CONSIST OF REMOVING
THE TOPSOIL HUMUS. STRIPPING MATERIALS SHALL BE DISPOSED OF ON THE SITE IN AREAS
DESIGNATED BY THE ENGINEER. PER GEOTECH'S RECOMMENDATIONS.
3. IF SPRINGS OR GROUND WATER IS ENCOUNTERED DURING CONSTRUCTION, THE CONTRACTOR
SHALL ADVISE THE SOILS AND CIVIL ENGINEER OF THE CONDITIONS FOUND AND COORDINATE
HIS ACTIVITIES IN A MANNER THAT WILL ALLOW THE ENGINEERS TIME TO REVIEW THE
SITUATION AND PREPARE A PLAN TO PROPERLY DISPOSE OF THE WATER ENCOUNTERED.
4. MATERIAL IN SOFT SPOTS WITHIN THE ROADWAY SHALL BE REMOVED TO THE DEPTH REQUIRED
TO PROVIDE A FIRM FOUNDATION AND SHALL BE REPLACED WITH 1 1/2"-0" CRUSHED ROCK.
THE ENTIRE SUBGRADE SHALL BE THOROUGHLY COMPACTED PER GEOTECHNICAL
RECOMMENDATIONS.
5. CONTRACTOR SHALL NOTIFY THE ENGINEER WHEN SUBGRADE IS COMPLETE AND 24 HOURS
PRIOR TO PLACEMENT OF ROCK BASE MATERIAL AND 24 HOURS PRIOR TO FINAL PAVING FOR
AN INSPECTION OF THE WORK. FAILURE TO DO SO WILL MAKE ANY SUBGRADE FAILURE OR
DRAINAGE PROBLEMS THE RESPONSIBILITY OF THE CONTRACTOR.
STREETS
1.ALL JOINTS TO BE RESTRAINED.
2.OPERATION OF VALVES BY TIGARD WATER PERSONNEL ONLY.
3.OPERATION OF VALVES IN THE TIGARD WATER SERVICE AREA IS PROHIBITED.
4.THE CONTRACTOR SHALL MAINTAIN ONE SET OF STAMPED APPROVED PLAN ON THE
CONSTRUCTION SITE AT ALL TIMES.
5.ALL WORK SHALL BE IN ACCORDANCE WITH AWWA (AMERICAN WATER WORKS ASSOCIATION)
STANDARDS.
6.ALL WORK SHALL BE APPROVED BY THE CITY PROJECT INSPECTOR.
7.ALL PIPE AND FITTINGS SHALL BE DUCTILE IRON, CEMENT LINED, OF NEW MANUFACTURE AND MADE
IN THE USA. PIPE SHALL BE “TYTON” JOINT DUCTILE IRON, CLASS 52. ALL FITTINGS SHALL BE MJ
(MECHANICAL JOINT) UNLESS OTHERWISE SPECIFIED.
8.COVER FOR ALL WATER MAINS IN THE STREET AND RIGHTS-OF-WAY SHALL BE 36”. BACKFILL SHALL
BE ¾”-0” GRAVEL.
9.ALL VALVE-OPERATING NUTS SHALL BE WITHIN 36” OF FINISHED GRADE, OTHERWISE VALVE
OPERATING NUT EXTENSIONS WILL BE REQUIRED.
10.POURED CONCRETE THRUST BLOCKS OF AT LEAST EIGHT SQUARE FEET OF BEARING SURFACE ARE
REQUIRED AT EACH TEE, CROSS AND BEND LOCATIONS (SEE DETAILS FOR MINIMUM BEARING AREAS.)
11.A 4X4X8'-0”, PAINTED BLUE, SHALL BE INSTALLED IN FRONT OF EVERY 2” WATER SERVICE LOCATION
AND REMAIN THERE UNTIL THE WATER METER IS INSTALLED. ALL 2-INCH WATER SERVICES SHALL
CONSIST OF A MJ X 2” IPT TEE AND 2” IPT X 110 COMPRESSION FITTING AS MANUFACTURED BY
MUELLER COMPANY.
12.FIRE HYDRANT ASSEMBLY CONSISTS OF A MJ X 6” FLG. (FLANGE) TEE, 6” FLG. X MJ GATE VALVE, 6”
MJ X MJ HOLDING SPOOL AND A MODERN MUELLER CENTURION FIRE HYDRANT, A-442, 6” MJ, 5 1/4”
MVO, 3 PORT (2-2 1/2” NST HOSE CONNECTION, 1-4 1/2” NST PUMPER,) 1 ½” PENTAGON
OPERATING NUT OPENING LEFT, COLOR BEING YELLOW OR APPROVED EQUAL: MUELLER CENTURION
A-423, WATERIOUS PACER 6790, CLOW MEDALLION.
13.EACH FIRE HYDRANT SHALL BE INSTALLED UPON A PRE-FORMED CONCRETE BLOCK WITH 1 ½ CUBIC
YARDS OF CRUSHED 2”-3/4” DRAIN ROCK. TARPAPER WILL BE LAID ON TOP OF THE DRAIN ROCK TO
SEPARATE THE ROCK FROM THE EARTH COVER.
14.ALL SANITARY SEWER LINES WITHIN 10 FEET LATERALLY OR 3 FEET VERTICALLY OF A WATER MAIN
SHALL BE INCASED IN A REINFORCED CONCRETE JACKET 6” THICK FOR A DISTANCE OF 10 FEET ON
BOTH SIDES OF THE CROSSING. WHERE CROSSINGS ARE NECESSARY, THEY MUST BE MADE AT
APPROXIMATELY 90 DEGREES WITH AT LEAST 18” OF SEPARATION BELOW THE WATER LINE.
15.ALL MAINS WITH A STATIC PRESSURE UP TO 100 PSI (POUNDS PER SQUARE INCH) SHALL BE TESTED
AT 150 PSI FOR 1 HOUR WITH A MAXIMUM LOSS OF 5 PSI. WATER MAINS WITH A STATIC PRESSURE
GREATER THAN 100 PSI SHALL BE PRESSURE TESTED AT 1.5 TIMES THE STATIC PRESSURE FOR 1
HOUR WITH A MAXIMUM LOSS OF 5 PSI.
16.UPON SATISFACTORY COMPLETION OF TESTING, THE NEW MAINS AND CONNECTIONS TO EXISTING
MAINS SHALL BE CLEANED AND FLUSHED WITH POTABLE WATER PRIOR TO DISINFECTION. FLUSHING
VELOCITIES SHALL BE AT LEAST 2.5 FEET PER SECOND. DISINFECTION SHALL BE IN ACCORDANCE
WITH AWWA STANDARD C651-92, THE STATE HEALTH DIVISION AND CITY REQUIREMENTS. THE
CONTINUOUS FEED METHOD OF DISINFECTION SHALL BE USED. DISINFECTING MIXTURE SHALL BE A
CHLORINE-WATER SOLUTION HAVING A FREE CHLORINE RESIDUAL OF 40 - 50 MG/L (MILLIGRAMS
PER LITER.) THE DISINFECTION MIXTURE WILL BE PREPARED BY INJECTING A CALCIUM/SODIUM
HYPOCHLORITE AND WATER SOLUTION INTO THE PIPELINE AT A MEASURED RATE WHILE FRESH
(POTABLE) WATER IS ALLOWED TO FLOW THROUGH THE PIPELINE SO THAT THE CHLORINE WATER
SOLUTION IS OF THE SPECIFIED STRENGTH. TREATED (CHLORINATED) WATER SHALL BE RETAINED IN
THE PIPELINE LONG ENOUGH TO DESTROY ALL NONSPORE-FORMING BACTERIA. TYPICAL RETENTION
PERIOD IS 24 HOURS. AT THE END OF THE 24 HOUR PERIOD, THE PIPELINE IS TO HAVE A FREE
CHLORINE RESIDUAL OF AT LEAST 10 MG/L. AFTER SATISFACTORY CHLORINATION, FLUSH THE
WATER FROM THE LINE UNTIL THE WATER THROUGHOUT THE PIPELINE IS EQUAL CHEMICALLY AND
BACTERIOLOGICALLY TO THE PERMANENT SOURCE OF SUPPLY.
DISPOSE OF THE DISINFECTION WATER IN AN APPROVED MANNER. DO NOT ALLOW DISINFECTION
WATER TO FLOW INTO A WATERWAY WITHOUT ADEQUATE DILUTION OR OTHER SATISFACTORY
METHODS OF REDUCING CHLORINE RESIDUALS TO A SAFE LEVEL AS MANDATED BY DEQ. AFTER
DISPOSAL AND FLUSHING OF THE DISINFECTION SOLUTION, THERE WILL BE ANOTHER 24 HOUR
PERIOD PRIOR TO BACTERIOLOGICAL TESTING. BACTERIOLOGICAL TESTS WILL BE TAKEN BY A
TRAINED TIGARD WATER WORKER.
17.PROVIDE AN ECLIPSE NO. 88 SAMPLING STATION MANUFACTURED BY KUPFERTE FOUNDRY, ST.
LOUIS, MO 63102. THE SAMPLING STATION SHALL BE 2”-0” BURY, WITH A ¾” FIPT (FEMALE IRON
PIPE THREAD) INLET, AND A ¾” UNTHREADED HOSE NOZZLE. ALL SAMPLING STATIONS SHALL BE
ENCLOSED IN A LOCKABLE, NON-REMOVABLE, ALUMINUM-CAST HOUSING. WHEN OPENED, THE
SAMPLING STATION SHALL REQUIRE NO KEY FOR OPERATION AND THE WATER WILL FLOW IN AN
ALL-BRASS WATERWAY. ALL WORKING PARTS WILL BE OF BRASS AND BE REMOVABLE FROM ABOVE
GROUND WITH NO DIGGING. A COPPER VENT TUBE (STANDARD) WILL ENABLE THE SAMPLING
STATION TO BE PUMPED FREE OF STANDING WATER TO PREVENT FREEZING AND TO MINIMIZE
BACTERIA GROWTH. THE EXTERIOR PIPING WILL BE BRASS AND A ¼” BALL VALVE SHALL BE
PROVIDED IN PLACE OF THE ¼” PET COCK ON THE VENT PIPE.
18.WATER METERS WILL BE INSTALLED BY TPW UPON INDIVIDUAL REQUEST AND PAYMENT BY OTHERS
(OWNERS.) ALL WATER METERS CONNECTED TO AN IRRIGATION SYSTEM MUST HAVE THE PROPER
OREGON STATE APPROVED BACKFLOW PREVENTION DEVICE, MINIMUM OF A DCVA (DOUBLE CHECK
VALVE ASSEMBLY) INSTALLED ON THE PROPERTY SIDE OF THE WATER METER. IN ADDITION, EVERY
METER FOR COMMERCIAL, MULTI-FAMILY, INDUSTRIAL AND INSTITUTIONAL SERVICE, REGARDLESS
OF SIZE, SHALL HAVE THE PROPER OREGON STATE APPROVED BACKFLOW PREVENTION DEVICE
MINIMUM OF A DCVA INSTALLED ON THE PROPERTY SIDE OF THE METER. THE BACKFLOW
PREVENTION DEVICE SHALL BE INSTALLED AND TESTED BY THE OWNER, WITH THE RESULTS
FORWARDED TO THE TPW WATER DIVISION BEFORE WATER SERVICE CAN BEGIN.
WATER CONSTRUCTION NOTES
1.THE GRADING PERMIT IS NOT TO BE CONSTRUED AS FINAL APPROVAL OF SITE GRADING,
EMBANKMENT OR STRUCTURAL FILL WORK; IT MERELY PROVIDES FOR INITIATION OF WORK
AND IS SUBJECT TO CHANGE PENDING CITY APPROVAL OF THE ENTIRE
CONSTRUCTION-DEVELOPMENT PLAN SET.
2.SPREADING OF MUD OR DEBRIS UPON ANY PUBLIC ROAD IS PROHIBITED. THE CITY MAY ORDER
STOPPAGE OF WORK TO EFFECT CORRECTIVE ACTION, AT ANY TIME.
3.EFFECTIVE EROSION CONTROL, DUST CONTROL, AND DRAINAGE CONTROL IS REQUIRED AT ALL
TIMES. THE CITY MAY ORDER STOPPAGE OF WORK TO EFFECT CORRECTIVE ACTION, AT ANY
TIME.
4.EMBANKMENTS OR STRUCTURAL FILLS FOR ROADWAY CONSTRUCTION OR FILLS TO BE
CONSTRUCTED ON BUILDABLE LOTS SHALL BE CONSTRUCTED FROM EXCAVATED MATERIALS
ACCEPTABLE TO THE SOILS ENGINEER AND SHALL BE BROUGHT TO GRADE IN LIFTS NOT TO
EXCEED 8" LOOSE MEASURE. EACH LIFT SHALL BE COMPACTED TO 90 PERCENT OF MAXIMUM
DENSITY AS OBTAINED BY AASTHO T_180 COMPACTION TEST.
5.STRUCTURAL FILLS SHALL COMPLY WITH CHAPTER 70APPENDIX CHAPTER 33 OF THE UNIFORM
BUILDING CODE. CONSTRUCTION ON EXISTING SLOPES GREATER THAN 5:1 SHALL BE
ACHIEVED BY BENCHING INTO THE EXISTING BANK A MINIMUM OF TEN FEET. IF THE BENCH
EXPOSES SAND SOIL AN UNDERDRAIN MUST BE PROVIDED. THE UNDERDRAIN, IF REQUIRED,
WILL BE CONSTRUCTED BY LYING INSTALLING NONWOVEN FILTER FABRIC, EQUIVALENT TO
EXXON GTF 12500, ALONG THE BOTTOM OF THE STARTING BENCH, THEN PLACING A SIX INCH
LAYER OF 1½" -- 03/4" DRAIN ROCK, AND FINALLY COVERING THE DRAIN ROCK WITH THE
NONWOVEN FILTER FABRIC. SUCCESSIVE BENCHES WILL BE CONSTRUCTED WITH VERTICAL
FILL BETWEEN TWO TO FIVE FEET AND BE COMPACTED IN LAYERS NOT TO EXCEED 8 INCHES.
EACH 8-INCH LAYER SHALL BE COMPACTED TO A MINIMUM OF 90 PERCENT AASHTO T-180
DENSITY. THE FILL SLOPES SHALL NOT EXCEED 2:1 AT FINISH GRADE. NO ROCK OR SIMILAR
MATERIAL EXCEEDING A 12-INCH DIAMETER SHALL BE ALLOWED IN THE STRUCTURAL FILL.
6.THE GEOTECHNICAL ENGINEER FOR STRUCTURAL FILL SHALL BE NOTIFIED 24 HOURS IN
ADVANCE, BY THE CONTRACTOR, OF STARTING BENCHWORK TO DETERMINE THE NEED FOR
AN UNDERDRAIN LAYER AND TO VERIFY EXISTING CONDITIONS.
7.THE CITY SHALL BE PROVIDED WITH A COPY OF THE GEOTECHNICAL ENGINEER'S
REPORT/RECOMMENDATION CHANGES.
8.IF SPRINGS OR GROUND WATER ARE ENCOUNTERED DURING CONSTRUCTION, THE
CONTRACTOR SHALL NOTIFY THE SOILS AND CIVIL ENGINEERS OF THE CONDITIONS FOUND
AND COORDINATE HIS ACTIVITIES IN A MANNER THAT WILL ALLOW THE ENGINEERS TIME TO
REVIEW THE SITUATION AND PREPARE A PLAN TO PROPERLY MITIGATE THE WATER
ENCOUNTERED.
9.THE CONTRACTOR SHALL HAVE THE SOILS ENGINEER TAKE COMPACTION TESTS. A MINIMUM
OF THREE TESTS WILL BE REQUIRED FOR EACH 2 FEET OF FILL.
10.EXCAVATOR MUST COMPLY WITH ORS 757.541 THROUGH 757.571 (UTILITY PRE-NOTIFICATION,
ETC.).
GRADING NOTES
1.ALL STORM DRAIN CONNECTIONS ARE TO BE FACTORY TEES UNLESS OTHERWISE NOTED.

2.THE CITY'S STANDARD SPECIFICATION FOR TRAFFIC CONTROL IS "MANUAL ON UNIFORM
TRAFFIC CONTROL DEVICES FOR STREETS AND HIGHWAYS", U.S. DEPT. OF
TRANSPORTATION, FHWA, CURRENT EDITION.
3.TRAFFIC CONTROL SHALL BE PROVIDED FOR BY THE CONTRACTOR IN ACCORDANCE WITH
THE CITY'S STANDARD SPECIFICATIONS AND, ALSO, IN ACCORDANCE WITH A CITY (JOB
SPECIFIC) APPROVED TRAFFIC CONTROL PLAN. A COPY OF THE APPROVED TRAFFIC
CONTROL PLAN SHALL BE AVAILABLE AT THE WORK AREA.
4.PUBLIC ROADWAY SHALL NOT BE CLOSED TO TRAFFIC, AT ANY TIME, WITHOUT HAVING
FIRST OBTAINED WRITTEN APPROVAL FROM THE CITY ENGINEER. THE PERMIT HOLDER IS
RESPONSIBLE FOR PROVISION OF TIMELY NOTIFICATION OF TRAFFIC FLOW DISRUPTIONS
TO AREA WIDE EMERGENCY SERVICES (TIGARD POLICE DEPT., TUALATIN FIRE & RESCUE)
AND TO TRI-MET AND TIGARD SCHOOL DISTRICT AND OTHER SERVICE PROVIDERS.
5.ADVANCE WARNING OF IMMINENT TRAFFIC DISRUPTION SHALL BE PROVIDED TO THE
GENERAL MOTORING PUBLIC BY PLACEMENT OF AN ADVANCE NOTIFICATION SIGN AT
EACH END OF THE CONSTRUCTION AREA 72 HOURS (MIN.) BEFORE INITIATION OF
CONSTRUCTION WORK.
6.ACCESS TO EXISTING PROPERTIES SHALL BE MAINTAINED AT ALL TIMES, INCLUDING
NORMAL DELIVERY SERVICE AND MAIL SERVICE AND IF NOT, SHALL BE CAUSE FOR WORK
STOPPAGE UNTIL EFFECTIVE ACCESS IS ESTABLISHED.
7.TRAFFIC CONTROL DEVICES, FLAG PERSONS, ETC., SHALL BE IN PLACE PRIOR TO
INITIATION OF CONSTRUCTION WORK AND SHALL BE EFFECTIVELY MAINTAINED.
8.PURSUANT TO TIGARD MUNICIPAL CODE SECTION 7.40.180, WORK IS PERMITTED
BETWEEN THE HOURS OF 7:00 A.M. TO 7:00 P.M. DAILY UNLESS OTHERWISE AUTHORIZED
BY THE CITY MANAGER.
9.HOURS OF CONSTRUCTION WORK ON COLLECTOR AND ARTERIAL ROADS WILL BE LIMITED
TO 9:00 A.M.-3:00 P.M. UNLESS AUTHORIZED BY THE CITY ENGINEER. NO WORK WILL BE
PERMITTED ON COLLECTOR AND ARTERIAL ROADS BETWEEN 3:30 P.M. AND 9:00 A.M.
UNLESS AUTHORIZED BY THE CITY ENGINEER.
10.TEMPORARY TRENCH PATCHING ON ARTERIALS AND COLLECTORS SHALL BE PAVED WITH
AT LEAST 2” OF HOT MIX ASPHALT.
11.MINIMUM TRAVEL LANE WIDTH SHALL BE TWELVE (12) FEET; PEDESTRIAN TRAVEL SHALL
ALSO BE PROVIDED FOR.
12.THE CITY RESERVES THE RIGHT TO ADD TO OR MODIFY TRAFFIC CONTROL
REQUIREMENTS AS MAY BE NECESSARY TO EFFECTIVELY CONTROL TRAFFIC AND TO
ASSURE PUBLIC SAFETY.
13.BEFORE INITIATING ANY CONSTRUCTION ACTIVITY, THE PERMIT HOLDER SHALL BE
CONTACTED BY THE CITY'S PRIVATE DEVELOPMENT INSPECTOR, (503/639-4171) TO
ESTABLISH A PLACE, TIME, AND DATE FOR A PRECONSTRUCTION MEETING.
14.THE PERMIT HOLDER OR HIS CONTRACTOR SHALL NOTIFY THE CITY'S INSPECTOR AT
503/639-4171 TWENTY-FOUR (24) HOURS PRIOR TO COMMENCING WORK, TWENTY-FOUR
(24) HOURS PRIOR TO ANY STAGED INSPECTION AND AFTER COMPLETING WORK
COVERED BY THE PERMIT.
15.A COPY OF THE PERMIT AND ALL ATTACHMENTS, AND A COPY OF THE APPROVED
CONSTRUCTION PLAN AND ALL AMENDMENTS SHALL BE AVAILABLE AT THE WORK AREA.
ALL WORK SHALL CONFORM TO THE PERMIT TERMS, CONDITIONS AND PROVISIONS AND
TO THE CITY APPROVED PERMIT PLANS, AND APPROVED PLAN AMENDMENTS AND TO THE
CITY'S STANDARDS AND SPECIFICATIONS AND TO THESE GENERAL CONDITIONS. CHANGES
TO ANY OF THE AFORESAID MUST BE APPROVED BY THE CITY, IN ADVANCE OF WORK
PERFORMANCE.
16.MAINTENANCE OF THE WORK AREA AND APPROACH ROADS IS THE RESPONSIBILITY OF
THE PERMIT HOLDER. THE WORK AREA AND APPROACH ROADS SHALL BE MAINTAINED IN
A CLEAN CONDITION, FREE FROM OBSTRUCTIONS AND HAZARDS. A COPY OF THE PERMIT
HOLDERS CERTIFICATE OF INSURANCE SHALL BE AVAILABLE AT THE WORK AREA.
17.THE SPREADING OF MUD OR DEBRIS OR STORAGE OF MATERIALS OR EQUIPMENT OF ANY
KIND UPON ANY PUBLIC ROADWAY IS STRICTLY PROHIBITED AND VIOLATION SHALL BE
CAUSE FOR IMMEDIATE CANCELLATION OF THE PERMIT. THE CITY MAY AT ANY TIME
ORDER IMMEDIATE CLEAN-UP AND THE STOPPAGE OF WORK TO ACCOMPLISH CLEAN-UP.
18.EFFECTIVE EROSION CONTROL IS REQUIRED. EROSION CONTROL DEVICES MUST BE
INSTALLED AND MAINTAINED MEETING THE CLEAN WATER SERVICES AND DEQ
REQUIREMENTS. THE CITY MAY AT ANY TIME ORDER CORRECTIVE ACTION AND
STOPPAGE OF WORK TO ACCOMPLISH EFFECTIVE EROSION CONTROL.
19.PROPERTY DISTURBED BY CONSTRUCTION ACTIVITY SHALL BE SEEDED WITH A STANDARD
GRASS MIX; SHRUBS, FLOWERS, BARK DUST, EXISTING SIGNS, PAVEMENT MARKINGS,
MAILBOXES, ETC. SHALL BE REESTABLISHED, REINSTALLED OR REPLACED, WITH LIKE
KIND AND MATERIAL.
20.EFFECTIVE DRAINAGE CONTROL IS REQUIRED. DRAINAGE SHALL BE CONTROLLED WITHIN
THE WORK SITE AND SHALL BE SO ROUTED THAT ADJACENT PRIVATE PROPERTY, PUBLIC
PROPERTY AND THE RECEIVING SYSTEM IS NOT ADVERSELY IMPACTED. THE CITY MAY AT
ANY TIME ORDER CORRECTIVE ACTION AND STOPPAGE OF WORK TO ACCOMPLISH
EFFECTIVE DRAINAGE CONTROL.
21.OREGON LAW REQUIRES FOLLOWING THE RULES ADOPTED BY THE OREGON UTILITY
NOTIFICATION CENTER. SAID RULES ARE SET FORTH IN OAR 952-001-0010 THROUGH
OAR 952-001-0080. COPIES OF SAID RULES MAY BE OBTAINED FROM THE CENTER BY
CALLING 503-246-1987. IF YOU HAVE ANY QUESTION ABOUT THE RULES, CONTACT THE
CENTER. NOTE: DAMAGE TO UTILITIES SHALL BE CORRECTED AT THE PERMIT HOLDERS
EXPENSE.
22.CONTRACTOR MUST VERIFY ALL EXISTING UTILITIES FOR BOTH VERTICAL ELEVATION AND
HORIZONTAL LOCATION PRIOR TO START OF WORK (POTHOLE BEFORE DIGGING IF
NECESSARY). SHOULD CONFLICTS ARISE AND REDESIGN OR RELOCATION OF FACILITIES
BE NECESSARY, IT SHALL BE DONE AT THE PERMIT HOLDERS EXPENSE. CHANGES MUST
BE APPROVED BY THE CITY IN ADVANCE OF WORK PERFORMANCE. CONTRACTOR SHALL
COORDINATE THE WORK WITH AFFECTED UTILITY AGENCIES.
23.A TEMPORARY HARD-SURFACE PATCH (COLD MIX A.C. OR HOT MIX BASE PAVING) SHALL
BE PLACED ON TRENCHES WITHIN ROADWAYS AT THE END OF EACH DAYS WORK. NO
TRENCH, ON SITE OR OFF-SITE, SHALL BE LEFT AT ANY TIME IN AN UN-SAFE CONDITION.
THE PERMIT HOLDER IS RESPONSIBLE FOR AND IS LIABLE FOR HAZARDS OR DAMAGE
RESULTING FROM THE PROSECUTION OF THE WORK.
24.WORK PROVIDED FOR UNDER THE PERMIT SHALL INCLUDE REPAIR OF EXISTING FACILITIES
(ROADS, DITCHES, ETC.) AS MAY BE NECESSARY, IN THE CITY INSPECTOR'S OPINION, TO
OVERCOME DETERIORATION OR DAMAGE WHICH OCCURRED IN CONJUNCTION WITH THE
WORK AUTHORIZED BY THE PERMIT. CORRECTIVE WORK SHALL BE DONE AT THE PERMIT
HOLDERS EXPENSE.
25.ONE AS-BUILT DRAWING AND PDF SHOWING ALL NEW PUBLIC IMPROVEMENTS, INCLUDING
ANY REVISION MADE TO THE PREVIOUSLY APPROVED CONSTRUCTION PLANS AND, ALSO,
ANY IMPROVEMENT WHICH MAY IMPACT AN EXISTING PUBLIC SYSTEM OR FACILITY, SHALL
BE PROVIDED TO THE CITY BY A REGISTERED CIVIL ENGINEER ALONG WITH AN
ENGINEER'S CERTIFICATION OF INSTALLATION COMPLIANCE (FORM ATTACHED). IN
ADDITION, THE APPLICANT'S AS-BUILT DRAWINGS SHALL BE TIED TO THE GPS NETWORK.
THE APPLICANT'S ENGINEER SHALL PROVIDE THE CITY WITH AN ELECTRONIC FILE WITH
POINTS FOR EACH STRUCTURE (MANHOLES, CATCH BASINS, WATER VALVES, HYDRANTS
AND OTHER WATER SYSTEM FEATURES) IN THE DEVELOPMENT, AND THEIR RESPECTIVE X
AND Y STATE PLANE COORDINATES, REFERENCED TO NAD 83 (91). INCLUDE ACROBAT
(***.PDF) FILE AND ONE 11X17 COPY OF AS-BUILTS.
26.A SEWER SYSTEM AIR-TEST AND A VIDEO TEST REPORT AND ONE SET OF BLUELINE
"AS-BUILTS" (OF EITHER OR BOTH THE STORM AND SANITARY SEWER SYSTEMS) SHALL BE
REQUIRED BY THE CITY FOR REVIEW AND APPROVAL PRIOR TO CONNECTION OF ANY
BUILDINGS TO THE SEWER SYSTEM.
26.THE PERMIT HOLDERS ENGINEER/INSPECTOR SHALL SUBMIT DAILY INSPECTION REPORTS,
ON A WEEKLY BASIS, TO THE CITY'S INSPECTOR. (SEE DEVELOPER - ENGINEER
AGREEMENT NOTE # 5.)
27.THE CITY'S INSPECTOR MAY, AT HIS DISCRETION, REQUIRE PROVISION OF TESTS AND OR
REPORTS FROM THE PERMIT HOLDER, THE PERMIT HOLDERS ENGINEER OR CONTRACTOR
TO VALIDATE CLAIMS OF MATERIAL OR CONSTRUCTION ADEQUACY/COMPLIANCE. SUCH
TESTS/REPORTS SHALL BE PROVIDED AT THE PERMIT HOLDER'S EXPENSE.
28.THE PERMIT HOLDER SHALL PROVIDE A COPY OF A PROPERLY EXECUTED RELEASE AND
WAIVER DOCUMENT TO THE CITY FOR EACH OWNERSHIP DISTURBED BY CONSTRUCTION
ACTIVITY, AS EVIDENCE OF DISTURBANCE RESOLUTION AND OWNER SATISFACTION.
29.EXISTING MONUMENTS, PROPERTY CORNERS, AND SURVEY MARKERS SHALL BE
PROTECTED. REPLACEMENT SHALL BE AT THE PERMIT HOLDERS EXPENSE.
30.THE ENGINEER SHALL NOTIFY THE WASHINGTON COUNTY SURVEYOR WHEN THE INITIAL
AND FINAL LIFT OF ASPHALT HAVE BEEN PLACED.
31.THE PERMIT HOLDER SHALL PROVIDE TO THE CITY INSPECTOR, IN WRITING, THE NAMES
AND 24 HOUR EMERGENCY TELEPHONE NUMBER OF TWO (2) PERSONS WHO HAVE
AUTHORITY TO RESOLVE PROBLEMS, TAKE CORRECTIVE ACTION AND, IN GENERAL, WILL
BE RESPONSIBLE IN CASE OF ANY EMERGENCY. THE PERMIT HOLDER SHALL NOTIFY THE
CITY INSPECTOR, IN WRITING, OF ANY/ALL ASSIGNMENT CHANGES.
32.THE PERMIT HOLDER SHALL CAUSE HIS CONTRACTOR TO PROVIDE TO THE CITY
INSPECTOR, IN WRITING, THE NAME AND 24 HOUR EMERGENCY TELEPHONE NUMBER OF
A DESIGNATED "COMPETENT PERSON" RESPONSIBLE FOR CONSTRUCTION SAFETY AS PER
OR-OSHA, CHAP. 437, DIV. 3 CONSTRUCTION, SUB-DIVISION P - EXCAVATIONS. THE
CONTRACTOR SHALL NOTIFY THE CITY INSPECTOR OF ANY/ALL ASSIGNMENT CHANGES.
33.IT IS THE SOLE RESPONSIBILITY OF THE PERMIT HOLDER TO PROVIDE FOR PROPER
RIGHT-OF-ENTRY AND/OR EASEMENTS PRIOR TO STARTING WORK. PROOF OF
RIGHT-OF-ENTRY OR PROPERLY EXECUTED EASEMENTS, SHALL BE PROVIDED TO THE
CITY. THE CITY SHALL IN NO WAY BE CONSTRUED TO BE LIABLE FOR THE PERMIT
HOLDERS FAILURE TO OBTAIN OR PROVIDE FOR PROOF OF RIGHT-OF-ENTRY OR
EASEMENTS.
34.BEFORE PLACEMENT OF ASPHALT, THE PERMIT HOLDER SHALL CLEAN AND COMPLETE A
PRE-ACCEPTANCE VIDEO INSPECTION OF ALL NEW SEWER LINES. THE PERMIT HOLDER
SHALL PROVIDE THE CITY WITH 30 DAYS OF NOTICE FOR THE VIDEO INSPECTION. ANY
DEFICIENCIES SHALL BE REPAIRED BEFORE PLACEMENT OF FINAL LIFT.
35.(PRIVATE SYSTEMS) THE DEVELOPER SHALL MAINTAIN THE WATER QUALITY FACILITY
AND CORRECT ANY DEFECTIVE WORK, AND SHALL PERFORM ALL SCHEDULED
MAINTENANCE TO ASSURE PROPER FUNCTIONING OF THE FACILITY.
36.(PUBLIC SYSTEMS) THE DEVELOPER SHALL CREATE AND PROVIDE TO THE CITY A
SCHEDULE FOR PERIODIC (QUARTERLY) MAINTENANCE OF THE WATER QUALITY FACILITY.
THE DEVELOPER SHALL MAINTAIN SAID FACILITY FOR A PERIOD OF THREE (3) YEARS
FROM THE DATE OF CITY CONDITIONAL ACCEPTANCE OF THE PUBLIC IMPROVEMENTS.
ON OCTOBER 1ST OF EACH YEAR DURING THIS THREE (3) YEARS PERIOD, THE DEVELOPER
SHALL REPORT, IN WRITING, MAINTENANCE ACTIVITIES TO THE DIRECTOR OF PUBLIC
WORKS. CITY CREWS SHOULD MONITOR PROPER REMOVAL AND DISPOSAL OF SILT AND
DEBRIS. PRIOR TO CITY FINAL ACCEPTANCE OF THE FACILITY, THE DEVELOPER SHALL
PROVIDE A WRITTEN EVALUATION OF THE OPERATION AND MAINTENANCE NEEDS OF THE
FACILITY AND CORRECT ANY DEFICIENCIES IDENTIFIED IN THE EVALUATION.

APPROVAL PRIOR TO THE APPLICANT RECEIVING A PUBLIC FACILITY IMPROVEMENT (PFI) PERMIT AND COMMENCING ANY SITE WORK.
7.PRIOR TO COMMENCING ANY SITE WORK, THE APPLICANT MUST SUBMIT A PUBLIC FACILITY IMPROVEMENT (PFI) PERMIT TO COVER ALL

10.PRIOR TO COMMENCING ANY SITE WORK, THE APPLICANT MUST PROVIDE A CONSTRUCTION VEHICLE ACCESS AND PARKING PLAN FOR
APPROVAL BY THE CITY ENGINEER. THE PURPOSE OF THIS PLAN IS FOR PARKING AND TRAFFIC CONTROL DURING THE PUBLIC
IMPROVEMENT CONSTRUCTION PHASE. ALL CONSTRUCTION VEHICLE PARKING MUST BE PROVIDED ONSITE. NO CONSTRUCTION
VEHICLES OR EQUIPMENT WILL BE PERMITTED TO PARK ON THE ADJOINING RESIDENTIAL PUBLIC STREETS. CONSTRUCTION VEHICLES
INCLUDE THE VEHICLES OF ANY CONTRACTOR OR SUBCONTRACTOR INVOLVED IN THE CONSTRUCTION OF SITE IMPROVEMENTS OR
BUILDINGS PROPOSED BY THIS APPLICATION AND MUST INCLUDE THE VEHICLES OF ALL SUPPLIERS AND EMPLOYEES ASSOCIATED WITH

11.PRIOR TO COMMENCING SITE IMPROVEMENTS, THE APPLICANT MUST PROVIDE A PHOTOMETRIC ANALYSIS FOR THE REVIEW AND
APPROVAL. THE APPLICANT MUST SUBMIT PLANS SHOWING THE LOCATION OF STREETLIGHTS AND THE TYPE AND COLOR OF POLE AND
LIGHT FIXTURE FOR REVIEW AND APPROVAL. PHOTOMETRIC ANALYSIS WILL FOLLOW THE RECOMMENDED VALUES AND REQUIREMENTS
DESCRIBED IN ANSI/IESNA. ALL PUBLIC STREETLIGHTS MUST BE PGE OPTION B.
12.PRIOR TO COMMENCING ANY SITE WORK, THE APPLICANT MUST SUBMIT SITE PLANS AND A FINAL STORM DRAINAGE REPORT AS PART
OF THE PFI PERMIT INDICATING HOW RUN-OFF GENERATED BY THE DEVELOPMENT WILL BE COLLECTED, CONVEYED, TREATED, AND
DETAINED FOR REVIEW AND APPROVAL. THE STORM DRAINAGE REPORT MUST BE PREPARED AND INCLUDE A MAINTENANCE PLAN IN
ACCORDANCE WITH CWS DESIGN AND CONSTRUCTION STANDARDS AND THE CITY OF TIGARD STANDARDS.
13.PRIOR TO COMMENCING ANY SITE WORK, THE APPLICANT MUST OBTAIN A CWS STORMWATER CONNECTION AUTHORIZATION PRIOR TO
ISSUANCE OF THE CITY OF TIGARD PFI PERMIT. PLANS MUST BE SUBMITTED TO THE CITY OF TIGARD FOR REVIEW. THE CITY WILL
FORWARD PLANS TO CWS AFTER PRELIMINARY REVIEW.
14.PRIOR TO COMMENCING ANY SITE WORK, THE APPLICANT MUST SUBMIT SITE PLANS AS PART OF THE PFI PERMIT SHOWING THE
PROPOSED SANITARY SEWER SYSTEM AND ASSOCIATED FACILITIES TO BE DESIGNED AND CONSTRUCTED IN ACCORDANCE WITH THE
CITY OF TIGARD AND CWS DESIGN AND CONSTRUCTION STANDARDS.
15.PRIOR TO COMMENCING ANY SITE WORK, THE APPLICANT MUST SUBMIT SITE PLANS AS PART OF THE PFI PERMIT SHOWING ALL
PROPOSED AND/OR EXTENSIONS OF PUBLIC WATER LINES, HYDRANTS, AND WATER SERVICES TO BE DESIGNED IN ACCORDANCE WITH
THE CITY OF TIGARD STANDARDS FOR REVIEW AND APPROVAL.
16.PRIOR TO COMMENCING ANY SITE WORK, THE APPLICANT MUST PROVIDE WRITTEN APPROVAL FROM TVF&R FOR FIRE FLOW, HYDRANT
PLACEMENT, AND EMERGENCY VEHICULAR ACCESS AND TURN AROUND.
17.PRIOR TO COMMENCING ANY SITE WORK, THE APPLICANT MUST SUBMIT ALL PUBLIC UTILITY EASEMENTS, JOINT ACCESS EASEMENTS,
PUBLIC ACCESS EASEMENTS AND MAINTENANCE AGREEMENTS FOR REVIEW AND APPROVAL.
18.PRIOR TO COMMENCING ANY SITE WORK, THE APPLICANT MUST SUBMIT AN EROSION CONTROL PLAN AS PART OF THE PFI PERMIT. THE
PLAN MUST CONFORM TO THE "CWS EROSION PREVENTION AND SEDIMENT CONTROL DESIGN AND PLANNING MANUAL” (CURRENT
EDITION).
19.PRIOR TO COMMENCING ANY SITE WORK, THE APPLICANT MUST SUBMIT A FINAL GRADING PLAN SHOWING THE EXISTING AND
PROPOSED CONTOURS. THE PLAN MUST DETAIL THE PROVISIONS FOR SURFACE DRAINAGE OF ALL LOTS AND SHOW THAT THEY WILL
BE GRADED TO ENSURE THAT SURFACE DRAINAGE IS DIRECTED TO THE STREET OR A PUBLIC STORM DRAINAGE SYSTEM APPROVED BY
THE ENGINEERING DIVISION. THE DESIGN ENGINEER MUST INDICATE, ON THE GRADING PLAN, WHICH LOTS WILL HAVE NATURAL
SLOPES BETWEEN 10 PERCENT AND 20 PERCENT, AS WELL AS LOTS THAT WILL HAVE NATURAL SLOPES IN EXCESS OF 20 PERCENT.
THIS INFORMATION WILL BE NECESSARY IN DETERMINING IF SPECIAL GRADING INSPECTIONS AND/OR PERMITS WILL BE NECESSARY.

2.PROTECTION FENCING. TREE PROTECTION FENCING SHALL CONSIST OF 5-FOOT TALL
2-INCH MESH CHAIN LINK OR WELDED WIRE FENCING SECURED TO 1.5-INCH DIAMETER
STEEL OR ALUMINUM POSTS PLACED NO FURTHER THAN 8-FEET ON CENTER AND SHALL BE

3.PRECONSTRUCTION CONFERENCE. THE PROJECT ARBORIST SHALL BE ON SITE TO DISCUSS
METHODS OF TREE REMOVAL AND TREE PROTECTION PRIOR TO ANY CONSTRUCTION.
4.TREE PROTECTION ZONE MAINTENANCE. THE PROTECTION FENCING SHALL NOT BE
MOVED, REMOVED, OR ENTERED BY EQUIPMENT EXCEPT UNDER DIRECTION OF THE
PROJECT ARBORIST.
5.STORAGE OF MATERIAL OR EQUIPMENT. THE CONTRACTOR SHALL NOT STORE MATERIALS
OR EQUIPMENT WITHIN THE TPZ.
6.TREE PROTECTION INSPECTION. THE PROJECT ARBORIST SHALL INSPECT AND VERIFY THE
LOCATION OF PROTECTION MEASURES PRIOR TO CONSTRUCTION, MONITOR TREE
PROTECTION MEASURES AT LEAST ONCE EVERY OTHER WEEK WHEN WORK IS ON-GOING
NEAR THE TREE PROTECTION ZONE, AND PROVIDE WRITTEN REPORTS TO THE CITY
FOLLOWING EACH INSPECTION.
7.FINAL REPORT. AFTER THE PROJECT HAS BEEN COMPLETED, THE PROJECT ARBORIST
SHALL PROVIDE A FINAL REPORT THAT DESCRIBES THE MEASURES NEEDED TO MAINTAIN
AND PROTECT THE REMAINING TREES AND CONFIRM THAT NEW TREES HAVE BEEN
