Axiom Pest Control will be here Thursday,August 26th to do an initial treatment. We will be doing their 18 month program which has them come out for the initial treatment, again about 4 weeks later and then every three months ( a total of 7 treatments.) I am attaching the service agreement to show you that it is scheduled and what their treatment covers, but I will send the invoice after I get it for the initial treatment. If,for any reason, at any time the service agreement is canceled before its completion,and the initial service has been performed, customer agrees to pay a $250 cancellation fee. Upon completion of this agreement treatments will continue at the same frequency until canceled by the customer with a 30-day written notice. If customer fails for any reason to make a payment in 30 days from the due date Axiom Eco-Pest Control at its option may discontinue the service and start collection proceedings.
