THE CONTRACTOR IS REQUIRED TO TAKE DUE PRECAUTIONARY MEASURES
TO PROTECT UTILITY LINES ON THESE DRAWINGS.
THE CONTRACTOR AGREES THAT THEY SHALL ASSUME SOLE AND COMPLETE
RESPONSIBILITY FOR THE JOB SITE CONDITIONS DURING THE COURSE OF
CONSTRUCTION OF THIS PROJECT, INCLUDING SAFETY OF ALL PERSONS AND
PROPERTY. THIS SHALL APPLY CONTINUOUSLY AND NOT BE LIMITED TO
NORMAL WORKING HOURS; AND THE CONTRACTOR SHALL DEFEND,
INDEMNIFY AND HOLD THE OWNER AND THE ENGINEER HARMLESS FROM ANY
AND ALL LIABILITY, REAL OR ALLEGED, IN CONNECTION WITH THE
PERFORMANCE OF WORK ON THIS PROJECT, EXCEPT THE LIABILITY ARISING
FROM THE SOLE NEGLIGENCE OF THE OWNER OR THE ENGINEER.

1.THE PERMIT HOLDER MUST COMPLY WITH ALL APPLICABLE PROVISIONS OF FEDERAL AND STATE
LAW, THE TIGARD MUNICIPAL CODE, AND THE TERMS OF ANY AGREEMENT WITH THE CITY OF TIGARD
REGARDING WORK TO BE DONE PURSUANT TO THIS PERMIT.
2.THE CITY'S STANDARD SPECIFICATION FOR TRAFFIC CONTROL IS "MANUAL ON UNIFORM TRAFFIC
CONTROL DEVICES FOR STREETS AND HIGHWAYS", U.S. DEPT. OF TRANSPORTATION, FHWA, CURRENT
EDITION.
3.TRAFFIC CONTROL SHALL BE PROVIDED FOR BY THE CONTRACTOR IN ACCORDANCE WITH THE
CITY'S STANDARD SPECIFICATIONS AND, ALSO, IN ACCORDANCE WITH A CITY (JOB SPECIFIC) APPROVED
TRAFFIC CONTROL PLAN. A COPY OF THE APPROVED TRAFFIC CONTROL PLAN SHALL BE AVAILABLE AT
THE WORK AREA.
4.PUBLIC ROADWAY SHALL NOT BE CLOSED TO TRAFFIC, AT ANY TIME, WITHOUT HAVING FIRST
OBTAINED WRITTEN APPROVAL FROM THE CITY ENGINEER. THE PERMIT HOLDER IS RESPONSIBLE FOR
PROVISION OF TIMELY NOTIFICATION OF TRAFFIC FLOW DISRUPTIONS TO AREA WIDE EMERGENCY
SERVICES (TIGARD POLICE DEPT., TUALATIN FIRE & RESCUE) AND TO TRI-MET AND TIGARD SCHOOL
DISTRICT AND OTHER SERVICE PROVIDERS.
5.ADVANCE WARNING OF IMMINENT TRAFFIC DISRUPTION SHALL BE PROVIDED TO THE GENERAL
MOTORING PUBLIC BY PLACEMENT OF AN ADVANCE NOTIFICATION SIGN AT EACH END OF THE
CONSTRUCTION AREA 72 HOURS (MIN.) BEFORE INITIATION OF CONSTRUCTION WORK.
6.ACCESS TO EXISTING PROPERTIES SHALL BE MAINTAINED AT ALL TIMES, INCLUDING NORMAL
DELIVERY SERVICE AND MAIL SERVICE AND IF NOT, SHALL BE CAUSE FOR WORK STOPPAGE UNTIL
EFFECTIVE ACCESS IS ESTABLISHED.
7.TRAFFIC CONTROL DEVICES, FLAG PERSONS, ETC., SHALL BE IN PLACE PRIOR TO INITIATION OF
CONSTRUCTION WORK AND SHALL BE EFFECTIVELY MAINTAINED.
8.PURSUANT TO TIGARD MUNICIPAL CODE SECTION 7.40.180, WORK IS PERMITTED BETWEEN THE
HOURS OF 7:00 A.M. TO 7:00 P.M. DAILY UNLESS OTHERWISE AUTHORIZED BY THE CITY MANAGER.
9.HOURS OF CONSTRUCTION WORK ON COLLECTOR AND ARTERIAL ROADS WILL BE LIMITED TO 9:00
A.M.-3:00 P.M. UNLESS AUTHORIZED BY THE CITY ENGINEER. NO WORK WILL BE PERMITTED ON
COLLECTOR AND ARTERIAL ROADS BETWEEN 3:30 P.M. AND 9:00 A.M. UNLESS AUTHORIZED BY THE CITY
ENGINEER.
10.TEMPORARY TRENCH PATCHING ON ARTERIALS AND COLLECTORS SHALL BE PAVED WITH AT
LEAST 2” OF HOT MIX ASPHALT.
11.MINIMUM TRAVEL LANE WIDTH SHALL BE TWELVE (12) FEET; PEDESTRIAN TRAVEL SHALL ALSO BE
PROVIDED FOR.
12.THE CITY RESERVES THE RIGHT TO ADD TO OR MODIFY TRAFFIC CONTROL REQUIREMENTS AS
MAY BE NECESSARY TO EFFECTIVELY CONTROL TRAFFIC AND TO ASSURE PUBLIC SAFETY.
13.BEFORE INITIATING ANY CONSTRUCTION ACTIVITY, THE PERMIT HOLDER SHALL BE CONTACTED
BY THE CITY'S PRIVATE DEVELOPMENT INSPECTOR, (503/639-4171) TO ESTABLISH A PLACE, TIME, AND
DATE FOR A PRECONSTRUCTION MEETING.
14.THE PERMIT HOLDER OR HIS CONTRACTOR SHALL NOTIFY THE CITY'S INSPECTOR AT 503/639-4171
TWENTY-FOUR (24) HOURS PRIOR TO COMMENCING WORK, TWENTY-FOUR (24) HOURS PRIOR TO ANY
STAGED INSPECTION AND AFTER COMPLETING WORK COVERED BY THE PERMIT.
15.A COPY OF THE PERMIT AND ALL ATTACHMENTS, AND A COPY OF THE APPROVED CONSTRUCTION
PLAN AND ALL AMENDMENTS SHALL BE AVAILABLE AT THE WORK AREA. ALL WORK SHALL CONFORM TO
THE PERMIT TERMS, CONDITIONS AND PROVISIONS AND TO THE CITY APPROVED PERMIT PLANS, AND
APPROVED PLAN AMENDMENTS AND TO THE CITY'S STANDARDS AND SPECIFICATIONS AND TO THESE
GENERAL CONDITIONS. CHANGES TO ANY OF THE AFORESAID
MUST BE APPROVED BY THE CITY, IN ADVANCE OF WORK PERFORMANCE.
16.MAINTENANCE OF THE WORK AREA AND APPROACH ROADS IS THE RESPONSIBILITY OF THE
PERMIT HOLDER. THE WORK AREA AND APPROACH ROADS SHALL BE MAINTAINED IN A CLEAN
CONDITION, FREE FROM OBSTRUCTIONS AND HAZARDS. A COPY OF THE PERMIT HOLDERS
CERTIFICATE OF INSURANCE SHALL BE AVAILABLE AT THE WORK AREA.
17.THE SPREADING OF MUD OR DEBRIS OR STORAGE OF MATERIALS OR EQUIPMENT OF ANY KIND
UPON ANY PUBLIC ROADWAY IS STRICTLY PROHIBITED AND VIOLATION SHALL BE CAUSE FOR
IMMEDIATE CANCELLATION OF THE PERMIT THE CITY MAY AT ANY TIME ORDER IMMEDIATE CLEAN-UP
AND THE STOPPAGE OF WORK TO ACCOMPLISH CLEAN-UP.
18.EFFECTIVE EROSION CONTROL IS REQUIRED. EROSION CONTROL DEVICES MUST BE INSTALLED
AND MAINTAINED MEETING THE CLEAN WATER SERVICES AND DEQ REQUIREMENTS. THE CITY MAY AT
ANY TIME ORDER CORRECTIVE ACTION AND STOPPAGE OF WORK TO ACCOMPLISH EFFECTIVE EROSION
CONTROL.
19.PROPERTY DISTURBED BY CONSTRUCTION ACTIVITY SHALL BE SEEDED WITH A STANDARD
GRASS MIX; SHRUBS, FLOWERS, BARK DUST, EXISTING SIGNS, PAVEMENT MARKINGS, MAILBOXES, ETC.
SHALL BE REESTABLISHED, REINSTALLED OR REPLACED, WITH LIKE KIND AND MATERIAL.
20.EFFECTIVE DRAINAGE CONTROL IS REQUIRED. DRAINAGE SHALL BE CONTROLLED WITHIN THE
WORK SITE AND SHALL BE SO ROUTED THAT ADJACENT PRIVATE PROPERTY, PUBLIC PROPERTY AND
THE RECEIVING SYSTEM IS NOT ADVERSELY IMPACTED. THE CITY MAY AT ANY TIME ORDER
CORRECTIVE ACTION AND STOPPAGE OF WORK TO ACCOMPLISH EFFECTIVE DRAINAGE CONTROL.
21.OREGON LAW REQUIRES FOLLOWING THE RULES ADOPTED BY THE OREGON UTILITY
NOTIFICATION CENTER. SAID RULES ARE SET FORTH IN OAR 952-001-0010 THROUGH OAR 952-001-0080.
COPIES OF SAID RULES MAY BE OBTAINED FROM THE CENTER BY CALLING 503-246-1987. IF YOU HAVE
ANY QUESTION ABOUT THE RULES, CONTACT THE CENTER. DAMAGE TO UTILITIES SHALL BE
CORRECTED AT THE PERMIT HOLDERS EXPENSE.
22.CONTRACTOR MUST VERIFY ALL EXISTING UTILITIES FOR BOTH VERTICAL ELEVATION AND
HORIZONTAL LOCATION PRIOR TO START OF WORK (POTHOLE BEFORE DIGGING IF NECESSARY).
SHOULD CONFLICTS ARISE AND REDESIGN OR RELOCATION OF FACILITIES BE NECESSARY, IT SHALL BE
DONE AT THE PERMIT HOLDERS EXPENSE. CHANGES MUST BE APPROVED BY THE CITY IN ADVANCE OF
WORK PERFORMANCE. CONTRACTOR SHALL COORDINATE THE WORK WITH AFFECTED UTILITY
AGENCIES.
23.A TEMPORARY HARD-SURFACE PATCH (COLD MIX A.C. OR HOT MIX BASE PAVING) SHALL BE
PLACED ON TRENCHES WITHIN ROADWAYS AT THE END OF EACH DAYS WORK. NO TRENCH, ON SITE OR
OFF-SITE, SHALL BE LEFT AT ANY TIME IN AN UN-SAFE CONDITION. THE PERMIT HOLDER IS
RESPONSIBLE FOR AND IS LIABLE FOR HAZARDS OR DAMAGE RESULTING FROM THE PROSECUTION OF
THE WORK.
24.WORK PROVIDED FOR UNDER THE PERMIT SHALL INCLUDE REPAIR OF EXISTING FACILITIES
(ROADS, DITCHES, ETC.) AS MAY BE NECESSARY, IN THE CITY INSPECTOR'S OPINION, TO OVERCOME
DETERIORATION OR DAMAGE WHICH OCCURRED IN CONJUNCTION WITH THE WORK AUTHORIZED BY
THE PERMIT. CORRECTIVE WORK SHALL BE DONE AT THE PERMIT HOLDERS EXPENSE.
25.ONE AS-BUILT DRAWING AND PDF SHOWING ALL NEW PUBLIC IMPROVEMENTS, INCLUDING ANY
REVISION MADE TO THE PREVIOUSLY APPROVED CONSTRUCTION PLANS AND, ALSO, ANY
IMPROVEMENT WHICH MAY IMPACT AN EXISTING PUBLIC SYSTEM OR FACILITY, SHALL BE PROVIDED TO
THE CITY BY A REGISTERED CIVIL ENGINEER ALONG WITH AN ENGINEER’S CERTIFICATION OF
INSTALLATION COMPLIANCE (FORM ATTACHED). IN ADDITION, THE APPLICANT’S AS-BUILT DRAWINGS
SHALL BE TIED TO THE GPS NETWORK. THE APPLICANT’S ENGINEER SHALL PROVIDE THE CITY WITH AN
ELECTRONIC FILE WITH POINTS FOR EACH STRUCTURE (MANHOLES, CATCH BASINS, WATER VALVES,
HYDRANTS AND OTHER WATER SYSTEM FEATURES) IN THE DEVELOPMENT, AND THEIR RESPECTIVE X
AND Y STATE PLANE COORDINATES, REFERENCED TO NAD 83 (91). INCLUDE ACROBAT (***.PDF) FILE AND
ONE 11X17 COPY OF AS-BUILTS.
26.A SEWER SYSTEM AIR-TEST AND A VIDEO TEST REPORT AND ONE SET OF BLUELINE "AS-BUILTS"
(OF EITHER OR BOTH THE STORM AND SANITARY SEWER SYSTEMS) SHALL BE REQUIRED BY THE CITY
FOR REVIEW AND APPROVAL PRIOR TO CONNECTION OF ANY BUILDINGS TO THE SEWER SYSTEM.
26.THE PERMIT HOLDERS ENGINEER/INSPECTOR SHALL SUBMIT DAILY INSPECTION REPORTS, ON A
WEEKLY BASIS, TO THE CITY'S INSPECTOR. (SEE DEVELOPER - ENGINEER AGREEMENT NOTE # 5.)
27.THE CITY'S INSPECTOR MAY, AT HIS DISCRETION, REQUIRE PROVISION OF TESTS AND OR
REPORTS FROM THE PERMIT HOLDER, THE PERMIT HOLDERS ENGINEER OR CONTRACTOR TO VALIDATE
CLAIMS OF MATERIAL OR CONSTRUCTION ADEQUACY/COMPLIANCE. SUCH TESTS/REPORTS SHALL BE
PROVIDED AT THE PERMIT HOLDER’S EXPENSE.
28.THE PERMIT HOLDER SHALL PROVIDE A COPY OF A PROPERLY EXECUTED RELEASE AND WAIVER
DOCUMENT TO THE CITY FOR EACH OWNERSHIP DISTURBED BY CONSTRUCTION ACTIVITY, AS
EVIDENCE OF DISTURBANCE RESOLUTION AND OWNER SATISFACTION.
29.EXISTING MONUMENTS, PROPERTY CORNERS, AND SURVEY MARKERS SHALL BE PROTECTED.
REPLACEMENT SHALL BE AT THE PERMIT HOLDERS EXPENSE.
30.THE ENGINEER SHALL NOTIFY THE WASHINGTON COUNTY SURVEYOR WHEN THE INITIAL AND
FINAL LIFT OF ASPHALT HAVE BEEN PLACED.
31.THE PERMIT HOLDER SHALL PROVIDE TO THE CITY INSPECTOR, IN WRITING, THE NAMES AND 24
HOUR EMERGENCY TELEPHONE NUMBER OF TWO (2) PERSONS WHO HAVE AUTHORITY TO RESOLVE
PROBLEMS, TAKE CORRECTIVE ACTION AND, IN GENERAL, WILL BE RESPONSIBLE IN CASE OF ANY
EMERGENCY. THE PERMIT HOLDER SHALL NOTIFY THE CITY INSPECTOR, IN WRITING, OF ANY/ALL
ASSIGNMENT CHANGES.
32.THE PERMIT HOLDER SHALL CAUSE HIS CONTRACTOR TO PROVIDE TO THE CITY INSPECTOR, IN
WRITING, THE NAME AND 24 HOUR EMERGENCY TELEPHONE NUMBER OF A DESIGNATED "COMPETENT
PERSON" RESPONSIBLE FOR CONSTRUCTION SAFETY AS PER OR-OSHA, CHAP. 437, DIV. 3
CONSTRUCTION, SUB-DIVISION P - EXCAVATIONS. THE CONTRACTOR SHALL NOTIFY THE CITY
INSPECTOR OF ANY/ALL ASSIGNMENT CHANGES.
33.IT IS THE SOLE RESPONSIBILITY OF THE PERMIT HOLDER TO PROVIDE FOR PROPER
RIGHT-OF-ENTRY AND/OR EASEMENTS PRIOR TO STARTING WORK. PROOF OF RIGHT-OF-ENTRY OR
PROPERLY EXECUTED EASEMENTS, SHALL BE PROVIDED TO THE CITY. THE CITY SHALL IN NO WAY BE
CONSTRUED TO BE LIABLE FOR THE PERMIT HOLDERS FAILURE TO OBTAIN OR PROVIDE FOR PROOF OF
RIGHT-OF-ENTRY OR EASEMENTS.
34.BEFORE PLACEMENT OF ASPHALT, THE PERMIT HOLDER SHALL CLEAN AND COMPLETE A
PRE-ACCEPTANCE VIDEO INSPECTION OF ALL NEW SEWER LINES. THE PERMIT HOLDER SHALL PROVIDE
THE CITY WITH 30 DAYS OF NOTICE FOR THE VIDEO INSPECTION. ANY DEFICIENCIES SHALL BE
REPAIRED BEFORE PLACEMENT OF FINAL LIFT.
35.(PRIVATE SYSTEMS) THE DEVELOPER SHALL MAINTAIN THE WATER QUALITY FACILITY AND
CORRECT ANY DEFECTIVE WORK, AND SHALL PERFORM ALL SCHEDULED MAINTENANCE TO ASSURE
PROPER FUNCTIONING OF THE FACILITY.
36.(PUBLIC SYSTEMS) THE DEVELOPER SHALL CREATE AND PROVIDE TO THE CITY A SCHEDULE FOR
PERIODIC (QUARTERLY) MAINTENANCE OF THE WATER QUALITY FACILITY. THE DEVELOPER SHALL
MAINTAIN SAID FACILITY FOR A PERIOD OF THREE (3) YEARS FROM THE DATE OF CITY CONDITIONAL
ACCEPTANCE OF THE PUBLIC IMPROVEMENTS. ON OCTOBER 1ST OF EACH YEAR DURING THIS THREE
(3) YEARS PERIOD, THE DEVELOPER SHALL REPORT, IN WRITING, MAINTENANCE ACTIVITIES TO THE
DIRECTOR OF PUBLIC WORKS. CITY CREWS SHOULD MONITOR PROPER REMOVAL AND DISPOSAL OF
SILT AND DEBRIS. PRIOR TO CITY FINAL ACCEPTANCE OF THE FACILITY, THE DEVELOPER SHALL
PROVIDE A
WRITTEN EVALUATION OF THE OPERATION AND MAINTENANCE NEEDS OF THE FACILITY AND CORRECT
ANY DEFICIENCIES IDENTIFIED IN THE EVALUATION.
GENERAL CONDITIONS
STORM AND SANITARY NOTES
1.ALL STORM DRAIN CONNECTIONS ARE TO BE FACTORY TEES UNLESS OTHERWISE NOTED
2.ALL RAIN DRAINS OR FOOTING DRAINS SHALL BE PIPED OR CONNECTED TO A DRAINAGE
SYSTEM, DRAINAGE BLOCKOUTS SHALL BE PLACED IN CURBING FOR THOSE LOTS ONLY
INDICATED ON THE PLANS. BLOCKOUTS ARE TO BE LOCATED 5’ FROM PROPERTY LINES.
3.ALL STORM SEWER PIPE SHALL BE INSTALLED WITH WATERTIGHT JOINTS IN EASEMENT
AREAS.
4.STORM SEWER PIPE, UNLESS OTHERWISE NOTED ON PLANS, SHALL BE PVC PER ASTM D-3034
(SDR 35).
5.BACKFILL MUST BE COMPACTED TO A DENSITY OF 95% PER ASSHTO T-99 IN PAVED OR
STRUCTURAL FULL AREAS. MINIMUM COMPACTION IN UNPAVED OR NON-STRUCTUAL
FILL AREAS IS 90%.
6.STORM DRAIN LINES SHALL BE WHITE IN COLOR AND TESTED ACCORDING TO THE
REQUIREMENTS OF CLEAN WATER SERVCES. TESTING SHALL BE PERFORMED AFTER
CONSTRUCTION IS COMPLETED INCLUDING BACKFILLING AND COMPACTION OF BASE
ROCK. THE DEFLECTION TEST AS DESCRIBED IN THE AMERICAN PUBLIC WORKS
ASSOCIATION MANUAL – OREGON CHAPTER IS REQUIRED. ALL STORM SEWER LINES
MUST BE THOROUGHLY CLEANED PRIOR TO DOING ANY TESTING OR VIDEO TAPING.
7.THE ENDS OF STORM DRAIN SERVICE LATERAL OR PIPE STUBS SHALL BE MARKED WITH
A ONE PIECE 2”X4” MARKER EXTENDING A MINIMUM 12” ABOVE THE GROUND WITH
THE END PAINTED IN WHITE. ATTACH MAGNETIC TAPE ALONSIDE THE 2X4 MARKER

OF THE GRADING PERMIT AND PLAN SHALL BE PROVIDED TO THE CLEAN WATER SERVICES (CWS) FOR
CONSTRUCTION INSPECTION AT THE PRE-CONSTRUCTION MEETING. ANY WORK OUTSIDE THE PUBLIC
RIGHT OF WAY, INCLUDING CUTS FILLS GRADING AND CLEARING MAY REQUIRE PERMITS FROM CITY OF
TIGARD.
8.UNANTICIPATED CONTAMINATION ENCOUNTERED DURING CONSTRUCTION IN CITY RIGHT OF WAY.
A)THE APPLICANT IS RESPONSIBLE FOR ALL COSTS ASSOCIATED WITH PROPER CHARACTERIZATION,
IDENTIFICATION ANALYSIS, MANAGEMENT AND DISPOSAL OF CONTAMINATED MEDIA ENCOUNTERED
WITHIN THE CITY RIGHT-OF-WAY OR ON CITY PROPERTY. THE APPLICANT IS ALSO RESPONSIBLE FOR
ALL RESULTANT DELAYS.
B) THE APPLICANT WILL PROVIDE THE CITY (ENGINEER AND INSPECTORS) WITH COPIES OF ALL
DISPOSAL PERMITS FROM THE PERMITTED DISPOSAL FACILITY, ANALYTICAL RESULTS USED TO GAIN
ACCEPTANCE OF THE CONTAMINATED MEDIA AND DISPOSAL RECEIPTS/DAILY WEIGH SLIPS. DAILY
WEIGH SLIPS AMOUNTS SHALL BE CHECKED AGAINST INSPECTOR'S DAILY REPORTS. THE
APPLICANT MUST USE AN OREGON DEPARTMENT OF ENVIRONMENTAL QUALITY- APPROVED
DISPOSAL FACILITY FOR DISPOSAL OF THE CONTAMINATED MEDIA.
9.FOUNDATION STABILIZATION MAY BE NECESSARY AS PER CLEAN WATER SERVICES (CWS) STANDARDS
10.MAINTAIN MINIMUM OF 36" OF COVER OVER ALL PIPE IN UNPAVED EASEMENT AREAS.

15.CITY OF TIGARD PLUMBING DIVISION APPROVALS AND PERMITS ARE REQUIRED FOR PRIVATELY
MAINTAINED SEWER, INLETS, INLET LEADS & SERVICE LATERALS CONSTRUCTED OUTSIDE OF PUBLIC
