Hi Trevor,
I called the Oregon Building Codes Division to verify LT Electric's supervisor license for Larry Mitchell, but I could not
speak with anyone. I left a voicemail for Gina Cadile to call me back. Unfortunately, I have not been able to verify Larry
Mitchell's Supervisor license 44245 is active. Per our earlier phone conversation, CCB license 223257 was updated, but
the Supervisor license 44245 has not been. I should receive a call back this week by the Building Codes Division. I will
get back to you once I know more. If LT Electric happens to have another Supervising license, I could always replace the
license for Larry Mitchell.

Hi Allyson,
I have not seen you for a very long time I hope all is well in your world. Brady brought up this
Hollingsworth project for approval and I found conditions related to building construction for
an alternate materials and method (AM&M) that I want you to be sure to remind you to be
aware of in the plan review process. They are to provide residential fire sprinklers in lieu of a
turn around. This was approved by Mark as per UAC requirements.

Hi Jim,
We will accept the Uniform Alternate Design Standard of providing fire sprinklers in lieu of access standards.The utility
work in the tract and laterals will be inspected under your PFI permit by our Engineering department. If there is site
utility plumbing work on the individual lots, a plumbing permit for each lot would be required.

Good Morning Mark/Tom—
I wanted to follow up with you regarding Jon's email below regarding the Hollingsworth Partition project on SW 121st
Ave. There are two items I'm hoping to get your help on:
1. As Jon mentioned below,we received the OK from the Fire Marshall to use Fire Sprinklers as an alternate
method for the length of the existing driveway, but in order to meet the Condition of Approval it looks like we
need your approval as well
2. The Site also has a small amount of private utility work for the storm and sewer services for lots 2 & 3 and we
would like to handle that application at the same time (I have attached a sheet for reference)
Can you clarify for me which applications you would like to see so we can get formal approval for the Fire Sprinklers? I
can submit a plumbing application as well for the private utilities.

Your proposed AM&M appears that it would be acceptable to TVF&R. You are
correct, this approval, since it involves the interior construction of the home, has
to ultimately be approved by the Building Official.

In regards to the Hollingsworth Partition project at 12580 SW 121st Ave, I have attached an Alternate
Material and Methods Request for the length of the existing driveway on site. This is in regards to
Condition of Approval#15 and the requirements in letter from your site review in April. See the
attached Staff Report for reference.
We are proposing Fire Sprinklers so I know the Building Official needs to approve this as well but I
wanted to start with you.
