I didn't know the other 2 duplexes will be available to slip-sheet, so I will have to print those revisions and
come back to slip-sheet them.$will provide the yellow transmittal form with details for each permit when I

Omar,
I see that (1) of the (3) sets of duplex plans were slip sheeted. Are you coming back today to finish the others today? If
not, when will you return? Also, per my previous email, we will need you to complete a Transmittal Letter for each
permit with changes—this form is also available in the lobby—it's the yellow one.

Hi Omar,
There was some confusion with regard to slip sheeting plans, and now I see that you are referring to slip sheeting two
different sets of permits. We will have the duplex/ADU permits ready for you to slip sheet at 1:00 today in the open
Permit Center conference room.
Regarding lots 154, 167, 168, 169 and 171, we received what appeared to be new submittals for lots 167 and
169 -- not 168. Any time you need to make revisions, we ask that you complete the attached Transmittal
Letter so that we know what changed. The submittals for lots 167 and 169 that we received on Monday did
not have a Transmittal Letter, and they appeared to be entirely new permit submittals,so we completed additional
review that may have not been necessary. In this case, we do not know if your intention is to change the house plan and
void the original submittal, or to make minor changes. Communication before you submit would be much
appreciated. I checked with our Residential Plans Examiner,Allyson Armstrong, and she was not aware that you were
going to slip sheet these plans or submit replacements. What is changing? Please complete the attached Transmittal
letters for each permit.
Additionally,we have already completed plan review for lots 167 and 169. Theses permits are basically ready to issue,
but we have not completed our final review. For reference, the permit numbers are as follows:
• Lot 167 = MST2020-00068.
• Lot 169 = MST2019-00450.
Lastly, we would appreciate it if you can provide us with a more thorough Description of Work on the applications when
necessary. I noticed that the duplex/ADU permits simply say"New Single Family." Thank you for your cooperation.

Hi Tina,
After looking at some previous emails, it looks like lots 154, 167, 168, 169 and 171 were kept open because we
are going to use the original permit#. The new plans that I submitted yesterday were supposed to be a slip-
sheet not new submittal, sorry for the confusion!
Please let me know if you can discard the original plans for lot 167 & 168 and consider the new plans and
calculations as slip-sheet under the original permit#.

Hi Omar,
I was attempting to create permits for Polygon at Roshak Ridge Lot 167 &Lot 169, but I see there is older
submittals for these lots in our system already. If the older submittals need to be voided a request for action form
will need to filled out for each address. This needs to be done prior to us creating a permit for the newer submittal.
If this is not the case please let us know how you would like us to proceed.
