Dear Jared, As per our conversation and my site visit, I have the following for your review and consideration. CCB#45970 www.fisherroof.com 503.227.ROOF 503.227.7663 With the above information in mind and the goal to provide a new long-term roofing solution we have the following scope of work and proposal for your review and consideration. General Scope 1. Provide a pre-roofing conference with all necessary parties involved with this project. This may include the owner or owner's representative, Fisher Roofing project manager and site foreman, sub-contractors, tenant representative, building engineer, site manager and any others that are critical to a smooth and successful project. Our goal is to have a clear understanding of expectations and address any concerns before we begin. 2. The Fisher Roofing foreman assigned to this project will remain with this project until completion. Contact information will be provided during the pre-roofing conference. 3. The site will be cleaned and free of debris, on a daily basis, and the roof system will be watertight at the end of the shift. 4. Roofing materials and equipment will be secured at all times, whether on the ground or on the roof. 5. If so required, a portable toilet will be set onsite in an agreed upon location. It is our goal to remain outside the building's interior as much as possible. 6. All OR-OSHA and/or WISHA safety standards will be strictly adhered to, including fall and ground protection. 7. Upon request, MSDS information will be provided for the products used and stored on this project. At any time, MSDS information is always available through our main office. 8. Upon request, we will provide our standard certificates of insurance. 9. Any dry rot or otherwise damaged decking or framing will be replaced on a time and materials basis. Our standard rates are $87.00 per man hour and materials plus 20% markup on materials for overhead and profit. This will be invoiced in addition to the sum of the contract. Tear off Scope 1. Remove the existing roofing assembly down to the last removable layer and dispose of offsite in a legal manor. It may be unfeasible to remove the last layer of roofing when hot asphalt or adhesive has been installed directly to the decking or substrate. Prior to the project commencement, roofing core sample/s will be taken and analyzed for asbestos content. If the results of the testing are positive, special methods for removal and disposal may be required, which can affect the pricing below. Test results have shown "None Detected" for all layers of roofing as related to asbestos. These results allow for normal removal and disposal methods. 2. Any dry rotted or otherwise damaged decking or structure will be replaced on a time and materials basis, as outlined above. 3. The dumpster or dump truck will receive cones and barrier tape to alert pedestrians and workers on the ground of possible hazard from the debris overhead. Tarps will be placed on the exterior walls for protection and if necessary, an enclosed trash chute will be installed. 4. We will not be responsible for cleanup of falling dust and debris inside the building. Roofing and Sheet Metal Scope 1. Sweep or power blow substrate clean in preparation for the new roofing system. 2. Identify any abandoned penetrations to be removed. Remove and infill decking, as required, on a time and materials basis as outlined above. Items to be identified by owner or owner's representative prior to project commencement. 3. Furnish and install '/4 ' Densdeck Fire Rated Protection Board mechanically attaching to the substrate with threaded fasteners and 3" steel insulation plates as required by the manufacturer. 4. Furnish and install a new 60 mil PVC mechanically attached, heat welded grey, UL Class "A" Fire Rated roofing system with the manufacturer's standard 20-year membrane only warranty. 5. Furnish and install new PVC base flashings on all roof curbs. 6. Furnish and install new base flashings on all walls running the membrane up and over the top of the walls creating a watertight seal prior to the installation of the new sheet metal cap flashings. 7. Clean the existing drain bowl assembly in preparation for the new flashings. Please note: Any damaged or missing parts will be replaced on a time and materials basis. 8. Furnish and install new PVC roof drain flashings set in compression sealant. 9. Fabricate and install one new oversized PVC Clad sheet metal drop drain with new strainer. 10. Furnish and install new factory PVC pipe boot flashings with new stainless-steel clamping ring and caulking sealant. When factory boots cannot be installed, field fabricated flashings will be necessary. 11. Furnish and install new factory pitch pockets at line sets, flex conduits and small diameter pipes. This includes the electrical guy wire mast. When factory pitch pockets cannot be used, we will fabricate and install custom clad sheet metal pockets as required. Prime and install factory pitch pocket filler. 12. Furnish and install new T-joint covers at all T-joints, as required by the manufacturer. 13. After probing all seams, furnish and install cut edge sealant on all exposed scrim edges. 14. Furnish and install one new walk pad at the located at the back of the building. 15. Furnish and install one additional layer of roofing membrane beneath the grease trap. 16. Fabricate and install new 26 ga. pre-painted baked enamel finished standing seam coping cap sheet metal flashings set in a continuous sheet metal face clip. 17. Fabricate and install new 26 ga. pre-painted sheet metal wall and curb counter flashings as required. 18. Upon completion, clean up and haul away all related construction debris. 19. Upon completion and payment, provide warranties as agreed upon in this contract including our 10-year workmanship warranty and the manufacturer's 20-year membrane only warranty.
